Pain Consultants of Arizona, in collaboration with HonorHealth, is a team of highly experienced physicians who specialize in interventional pain management. Our main location is in North Phoenix-Desert Ridge, with satellite offices in North Scottsdale, Peoria, and Chandler. We have over 20 years of combined practice experience specifically focused on providing solutions to patients suffering from acute, chronic, cancer, and/or post-operative pain.

At Pain Consultants of Arizona, our mission is to provide compassionate, evidence-based, highly individualized, and comprehensive care to acute and chronic pain sufferers, where the unique needs of each patient are the center of a shared decision process.
*Responsibilities*
* Perform minimally invasive interventional procedures such as nerve blocks, epidural injections, and radiofrequency ablations to alleviate acute and chronic pain conditions.
* Conduct comprehensive patient assessments, including detailed medical histories and physical examinations, to develop tailored pain management plans.
* Order, interpret, and correlate laboratory and diagnostic procedures to formulate differential diagnoses and establish working diagnoses (MRI, CT, NCVs, etc.).
* Monitor treatment and adjust treatment plans as necessary to optimize pain relief and quality of life.
* Utilize electronic health record (EHR) systems and medical imaging tools to accurately document procedures and monitor patient progress.
* Collaborate with other health care professionals including primary care physicians, neurologists, orthopedics, physical therapists, APPs, mental health providers, etc., to provide comprehensive care for all patients.
* Educate patients and their families about their conditions, treatment options, and self-management techniques. Complete/Assist with patient's paperwork in a timely manner, including FMLA.
* Stay abreast of developments in the field through CMEs, research, and participation in professional organization.
* Adhere to all ethical and legal standards of practicing medicine (and per specialty designation).
* Ensure adherence to HIPAA regulations and informed consent. Maintain meticulous medical records for all procedures and patient interactions. Engage with patients, return phone calls/messages timely.
* Assist with coverage for other clinics/locations, as needed.
* Perform additional duties as assigned.
* Participate in On-Call rotation
*Qualifications*
* Medical Degree (MD/DO)
* Current, unrestricted medical license (MD/DO).
* Current, unrestricted DEA license.
* Fellowship in Pain Management preferred and Board certification required (Anesthesiology, Pain, PM&R).
* Strong decision-making skills and the ability to function effectively within a collaborative care model.
* Excellent written and verbal communication skills.
* Proven experience performing interventional pain procedures
* Familiarity with EMR systems like Epic or similar platforms for documentation and patient management.
* Strong knowledge of physiology, anatomy, and medical terminology relevant to pain management practices.
* Excellent patient service skills with the ability to communicate complex information clearly and compassionately.

All Care VNA Physical Therapists help patients get back on their feet and out of the hospital so they can heal where they are most comfortable – in their homes and communities. Our Physical Therapists design and deliver personalized 1:1 rehabilitative therapy programs for patients recovering from a surgery, illness, or accident and can assess progress by being a part of their day-to-day life.
Schedule: Monday to Friday. No weekend or holiday rotation required.

Position Type:Full TimeEssential Duties & Responsibilities:
* Practice independently in the community as part of an interdisciplinary care team.
* Conduct comprehensive assessment of patients by determining patient needs, develops courses of action including treatment plan and outcomes.
* Manages assigned caseload; provides physical therapy services to patients as directed per PT plan of care.
* Deliver personalized rehabilitation therapy to patients in their home or care facilities.
* Constantly evaluate evolving patient needs and respond with plan of care adjustments.
* Documents all care provided and maintain records per All Care policies and standards.
* Participates in All Care team meetings, including but not limited to, interdisciplinary team meetings, in-service and training programs, rehab services meetings and caseload reviews.
* Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient.

* Ability to use and assist patient with use durable medical equipment as a component of the plan of care, including but not limited to wheelchairs, patient transfer lifts, ambulatory devices, slings, splints, orthoses, prostheses, and exercise/strengthening equipment.
Qualifications:
* Valid driver’s license is required.
* Active Massachusetts Physical Therapist (PT) license is required
* Previous experience as a physical therapist, preferably in a home health care or outpatient setting.
* Experience working with patients recovering from surgery, strokes, or managing chronic conditions is an advantage.
* Strong knowledge of physical therapy techniques, modalities, and exercise prescription.

- Current provider certification in BLS and ACLS from the American Heart Association.
