We are seeking a compassionate *Speech-Language Pathologist (SLP)* to join our *in-house therapy team* and help residents improve communication, cognition, swallowing safety, independence, and quality of life.
*CCC-SLPs and Clinical Fellowship Year (CFY) candidates are encouraged to apply.*

As an SLP, you will help residents:
* Improve speech, language, voice, and functional communication
* Strengthen memory, attention, problem-solving, sequencing, and safety awareness
* Improve swallowing function and mealtime safety
* Reduce the risk of aspiration, dehydration, and malnutrition
* Use compensatory strategies and communication supports more effectively
* Communicate needs, preferences, and concerns with greater independence
* Maintain meaningful cognitive-communication abilities as they age
You will complete evaluations, develop individualized treatment plans, provide skilled interventions, educate residents and caregivers, and collaborate with nursing, dietary, PT, OT, physicians, and the interdisciplinary care team.
*What We’re Looking For*
* Master’s degree in Speech-Language Pathology from an accredited program
* Current *ASHA Certificate of Clinical Competence (CCC-SLP)* or eligibility to complete a *Clinical Fellowship Year*
* Current *South Carolina Speech-Language Pathologist license* or eligibility for provisional licensure
* Current CPR certification
* Strong clinical assessment and treatment-planning skills
* Excellent communication and interpersonal skills
* A compassionate, resident-centered approach
* Ability to work effectively with geriatric and medically complex populations
Experience in skilled nursing, long-term care, rehabilitation, dysphagia management, or geriatrics is preferred.

*Qualifications*

* Prior experience working in skilled nursing facilities or long-term care environments is highly preferred
* Demonstrated knowledge of anatomy, physiology, medical imaging terminology, and treatment plan implementation
* Experience working with individuals with disabilities, neurodevelopmental disorders such as autism or childhood developmental delays, geriatrics, or in inpatient/outpatient settings
* Strong skills in caregiver and client communication to ensure clarity and support throughout therapy sessions
* Ability to perform manual handling safely during patient transfers and mobility assistance
* Familiarity with occupational therapy techniques for clients with neurodiverse needs and occupational health considerations
* Excellent documentation skills aligned with healthcare standards and medical terminology familiarity

2. Maintains current BLS certification
3. Maintains current ACLS certification
4. Respond to emergency situations when a physician is not immediately available by providing airway management, delivering emergency drugs and fluids and following cardiovascular life support guidelines. The CRNA initiates these activities and summons a physician as soon as possible.
5. Maintains thorough knowledge of pharmacological properties, indications and contraindications of anesthesia - their untoward effects and the management thereof.
6. Maintains thorough knowledge of sterile technique and infection control.
7. Maintains working knowledge of all equipment associated with delivery of anesthesia. Apply appropriate non-invasive and invasive monitoring devices for continuous evaluation of the patient.
8. Maintains and readily retrieves all necessary paperwork and patient records related to anesthesia.
9. Assist Direct of Nursing in maintenance of drug supply and inventory.
10. Provides in-service education to keep operating room staff abreast of anesthesia changes.
11. Participate with the staff in the initiation of the anesthetic.
12. Assists Physician in physical assessment of surgical patients.
13. Oversees delivery of preoperative, operative and postoperative anesthesia care. Manage patient's airway and ventilation during anesthesia care
14. Monitor regional and local anesthesia and provide adjunctive treatment as necessary.
15. Perform the pre-anesthesia evaluation.
16. Obtain and prepare all drugs necessary to initiate and maintain the anesthetic and to treat emergencies.
17. Develop and implement an anesthetic plan.
18. Order appropriate laboratory test, medical clearances necessary to aid in the evaluation of the patient preoperative, operatively or postoperative.
19. Administers IV sedation and peribulbar block and maintains under the direction of the Surgeon.
20. CRNAs should consult with surgeon concerning any abnormal conditions discovered during the pre-anesthetic evaluation, and any abnormal patient conditions arising during the anesthetic or during the recovery period.
21. Maintains surgical/anesthesia logbook, complete checklists of emergency equipment.
22. Maintains confidentiality of patient records

Cooperate and collaborate with other medical providers and support staff at the practice site to support effective communications and continuous improvement of the site’s clinical operation
Maintain accurate and concise documentation of all patient encounters on a timely basis, in such manner and form as determined by CHC, third party payers, federal funding and national accreditation agencies with which CHC has agreements and/or obligations
Provide services to a full patient panel, with a minimum of 1,500 active patients per provider FTE (Active patients shall be defined as those individuals who have had at least one completed medical encounter in the past two years. If the physician wishes to “close” practice to new patients, the minimum full panel of patients shall be attained, and agreement with the CMO and Chief Executive Officer must be obtained in writing)
Comply with the Federal Standards as outlined in the Bureau’s Provider Productivity Initiative Review (PPIR) as it pertains to the provider aspects of productivity/performance (In particular, for a full-time physician: a minimum of 4,200 encounters per fiscal year [as established by Medicare], cost per medical encounter compliance under the federal threshold as calculated each fiscal year)
Provide coordinated and comprehensive patient care throughout all major life cycles (except in the case of a provider specialty that limits the practice to a specific age, gender, or patient type), in all applicable care settings, including but not limited to: ambulatory outpatient; consultation; assistance to other providers, including mentoring and supervision; community medicine, such as sports physicals or services performed as a “Good Samaritan”; and services as defined/required by hospitals for admitting privileges or by contracts/agreements, i.e., third party payers, in which CHC participates
Perform other duties and services which may benefit patients and members of the community, or who could be patients of CHC, or which facilitates the provision of services to patients of the Corporation
Adhere to and provides services in accordance with the: a) mission of the Corporation; b) established and accepted standards of care; c) any clinical protocols and operational guidelines established by the Corporation; and c) terms and conditions as set forth in the Federal Grant, accrediting and licensing bodies (i.e. credentialing)
Cooperate with the Department of Justice in the defense of claims (including access to all pertinent documents, patient information and records). Furthermore, as the department of Justice requires, cooperate in providing information related to all previous malpractice claims as permitted by law and court action
Collaborate with Administration in the development and implementation of practice-specific strategic and operational plans

The Director of Family Medicine, in conjunction with the CMO, will provide ongoing feedback as well as formal reports on all aspects of performance including: productivity, efficiency; knowledge and techniques of medical diagnosis and treatment, including knowledge of medications and tests; referral patterns; patient rapport; supervisory ability; record maintenance; adherence to Corporate policies, procedures and directives; and interaction with peer, support and administrative staffs.

Facilitates communications among patient/family, multidisciplinary team, medical management team, community resources and other disciplines to anticipate, identify, evaluate, and act to resolve any potential barriers and constraints to delivery of care in a timely manner. Understands and interprets multiple contracts and contractual obligations in order to enable the care management team to achieve maximum clinical and financial outcomes.
Collaborates with the patient/family and inter-professional team including the primary care team, hospital care team, post acute care managers, and other care partners to provide a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services. May work embedded within a provider office or telephonically working with a care team.
Uses evidenced-based approaches to increase patient and family activation and engagement in their own care. As appropriate to the population, partners with patient and family to develop SMART (specific, measurable, attainable, relevant, time-bound) goals. Assists in the development, procurement, and adoption of patient self-management educational resources.
Identifies potential barriers to learning and/or to the optimal delivery of care. Reports abnormal findings to the responsible provider/care team, and collaborates to develop a plan.
Independently manages CM caseload according to department expectations. Ensures timely completion of tasks and documentation related to MCO, regulatory and contractual requirements.
Partners with identified at-risk patients throughout the diagnosis, treatment and follow-up in order to deliver continuity of care. Anticipates the needs of the patient, recognizes and responds to changes in a patient’s status and determines priorities of patient care based on essential patient needs.
Coordinates patient information and communication between and among the patient/family, the referring/accepting facilities and physicians, community caregivers (as applicable) and other members of ACM to ensure smooth transitions of care.
Coordinates referrals to other internal AAH departments and/or external community resources as necessary.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
- Registered Nurse license issued by the state in which the team member practices.
Education Required:
- Bachelor's Degree in Nursing or related field.
Experience Required:
- Typically requires 5 years of experience in clinical nursing or 1-2 years of care management experience.

This position is responsible for assisting clinicians and nursing staff in providing medical care, as well as, implementing and evaluating direct patient care. This position utilizes specialized knowledge, judgment and skills to provide an excellent patient experience.
CORE FUNCTIONS
1. Prepares patient for exam and treatment by taking and recording vital signs, symptoms and other necessary measurements and recording chief complaint; documents pain scores as appropriate. Reports condition of patient which may be indicative of change in the patient's condition to the clinician.
2. Assists providers with exams and minor in-office procedures which could include taking patients’ blood pressure, documenting respiration and heart rate/pulse, taking temperature, as well as performing phlebotomies, ear irrigation and EKG’s. Acts as chaperon as needed. Uses universal blood and body fluid precautions at all times and personal protective equipment as needed. Responsible for prioritization of patient flow through clinical process.
3. Coordinates and manages patient care including pre-certifications, referrals, authorizations, prescriptions and scheduling of patients for clinic, hospital and other ancillary medical treatments and tests. Responsible for communicating test results to patient. Acts as a resource to clinician in order to provide optimal patient care. May administer medications in accordance with established policies and procedures or as directed.
4. Provides clerical support with patient/client notifications and documentation to include but not limited to complete medical records including labs, authorizations, X-Rays, hospital records and any previous care pertaining to each patient visit. Provides complete and accurate information necessary for billing related to the patient encounter in a timely fashion.
5. Maintains a clean, functional environment including cleaning and disinfection of equipment, exam rooms and storage areas. Disinfects equipment and instruments using appropriate solutions following sterilization procedures. Follows manufacturer's recommendations and OSHA guidelines in handling of hazardous substances. Performs basic maintenance of equipment and coordinates needed maintenance for prompt repairs.
6. Orders supplies as directed, stocks patient care areas. Medication ordering and tasks related to all medications (e.g. expired medications/equipment and recalls).
7. Develops and maintains effective communication mechanism with all external and internal customers. Exemplifies service orientation and the team concept in all contacts.
8. This position will complete all necessary on-boarding/orientation activities, including simulation training, as needed or directed.
9. This position works under the direct supervision of the providers. This position interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Also interacts with physician’s offices in order to report, ask for or clarify information. Prioritizes data from multiple sources to provide support for the response of the patient and family to changes in health status.
MINIMUM QUALIFICATIONS
High school diploma/GED or equivalent working knowledge. Completion of an approved medical assistant training program as defined by state regulations.
Active Medical Assistant Certification is required. Certification or additional training must meet the requirements for Meaningful use. BLS certification required. Active Medical Assistant certification such as: Registered Medical Assistant (RMA) with proof of current membership from American Medical Technologists (AMT) or Certified Medical Assistant (CMA) with proof of current membership from American Association of Medical Assistants (AAMA) or Certified Clinical Medical Assistant (CCMA) with proof of current membership from National Health Career Association (NHA) (Test administered after 7/1/2017 only) or National Certified Medical Assistant (NCMA) with proof of current membership from National Certified Competency Testing (NCCT).
Requires strong computer skills, including the ability to work with medical software. Requires exceptional interpersonal and communication skills. Requires the ability to manage multiple changing priorities in an effective and organized fashion.
