Job Description:
Under the direction of the manager, the Patient Equipment Care Coordinator I - Facility Campus / Office is accountable for intaking, coordinating, and ensuring timely implementation of Home Medical Equipment and supply orders
Schedule: Full-time
Hours: 9:30am - 6:00pm
This position fills patient orders, coordinates deliveries, and responds to patient requests in a timely manner. As a valued member of the team, this position provides important patient education and will facilitate patient set ups or fittings in a one on one situation and provides instruction to clients and caregivers in the use, care, and safety of supplies and equipment provided.
The essential job duties may also include helping customers that come into our facilities as well as supportive office work. It includes exhibiting exceptional customer service on incoming and outgoing phone calls, helping customers in a retail setting, and in fulfilling orders directly with patients. Facilitates the set-up of medical equipment and supplies to the patient by assigning the appropriate delivery method to meet customer and company needs and verifies the client specific needs and selects appropriate equipment based on assessment while adhering to physician orders. Processes Point of Sale (POS) sales request by completing computer order entry, receiving payment, and coordinating delivery or patient pick-up. Communicates any problems regarding the patient's condition and use of supplies or equipment to the appropriate staff. Demonstrates keen problem identification skills and resolution abilities and addresses concerns in a timely manner and maintains an understanding of Medicare, Medicaid, and third-party billing requirements and all components of Homecare's equipment order processing system.
The Patient Equipment Care Coordinator I – Facility Campus / Office is responsible for all of the duties assigned to the Patient Equipment Care Specialist – Facility Campus / Office, listed above, with additional responsibilities of general Office / Operations Support: Supports office operations by ordering, stocking, and tracking inventory. (Requires specific training and use of SCO software and processes) and working with SCO and local site to coordinate the acquisition of appropriate medical equipment and supplies with the HME store to maintain acceptable par levels in stock. The Coordinators work on more complex assignments and service recovery initiatives and is expected to demonstrate keen problem identification and resolution skills and addresses concerns in a timely manner, while aiding other caregivers in problem solving and resolution skills.
Minimum Qualifications
- Demonstrated experience and expertise in a customer service role
- CDL (required for LOX transportation only)
- Current HAZMAT endorsement (required for LOX transport only)
- Have completed all requirements for the established career progression program for the next level within the Home Medical Equipment (HME) titles.
- Experience in a role requiring strong attention to detail, accuracy and dependability.
- Experience performing a role requiring effective verbal, written, and interpersonal communication skills.
- Organizational skills and ability to set priorities.
- Demonstrated ability to work independently and is self-motivated.
- Demonstrated ability to understand and speak English clearly, follow verbal and written instructions, and understand customer communications.
- Experience using basic word processing, spreadsheets, databases, internet, e-mail, and scheduling applications.
Preferred Qualifications
- More than two years of customer service experience.
- Greater than six months of Intermountain Homecare Home Medical Equipment or a minimum of six months as Patient Equipment Care Specialist title.
- Prior experience working with medical equipment
- Office Coordination experience.
- Patient care experience.
Ongoing need for employee to see and read information, labels, assess patient needs, operate and identify HME equipment and supplies.
Frequent interactions with both patients/customers and /or patient care providers, that require employee to verbally communicate as well as hear and understand spoken information, customer/patient needs, and issues quickly and accurately.
May be required to stand and walk to retrieve lift and carry different types of HME supplies and equipment.
Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care; accessing needed information; dialing in settings on equipment; changing filters; and threading of connectors and other equipment accessories.
Will also bend to retrieve, lift, and carry supplies and equipment. Expected to utilize full range of movement in doing so. Typically includes items of varying weights, up to and including heavy items up to 70 lbs.
Need to stand and walk to assist with distributing supplies and equipment. This includes pushing/pulling all types of HME/Respiratory equipment including oxygen cylinders and other heavy items. If applicable, will also bend to retrieve, lift, and carry supplies and equipment - expected to utilize full range of movement in doing so. Typically includes items of varying weights, up to and including heavy items.

Key Responsibilities:
Referral Management: Receive and process referrals for patient care, ensuring timely and consistent admissions.

Collaboration: Work closely with referring hospitals, physicians, board and care facilities, skilled nursing facilities, and other healthcare providers.

1. Answers internal and external calls with proper telephone etiquette. Communicates well both verbally and in written form. Provides information to callers in a helpful and courteous manner following HIPAA and Confidentiality Policies and guidelines, processing calls to proper location promptly.
2. Maintains an excellent working knowledge of the PBX telephone system, AMCOM, and all paging protocols and proprietary software programs.
3. Instructs callers to use long distance authorization code where applicable and assists others in making long distance calls, and conference calls when necessary and appropriate.
4. Follows processes and guidelines for overhead paging, while maintaining confidentiality at all times and communicating in a professional manner.
5. Responds appropriately to CODE calls and emergencies. Records pertinent information on Alarm & Code databases.
6. Knows and follows procedures for TDD/TTY Hearing impaired phone when applicable.
7. Adheres to the release of information, HIPAA, and confidentiality policies and procedures. Releases information to customers or co-workers according to policy and guidelines. Does not violate HIPAA or talk about patient information with co-workers unless necessary for the care of that patient.
8. Customer Service oriented. Gives and receives constructive criticism. Takes control of a conversation without becoming discourteous or upset with callers.
9. Stays calm under pressure and makes decisions independently, while following HIPAA and Confidentiality Policies and guidelines.

This position is responsible for assisting clinicians and nursing staff in providing medical care, as well as, implementing and evaluating direct patient care. This position utilizes specialized knowledge, judgment and skills to provide an excellent patient experience.
CORE FUNCTIONS
1. Prepares patient for exam and treatment by taking and recording vital signs, symptoms and other necessary measurements and recording chief complaint; documents pain scores as appropriate. Reports condition of patient which may be indicative of change in the patient's condition to the clinician.
2. Assists providers with exams and minor in-office procedures which could include taking patients’ blood pressure, documenting respiration and heart rate/pulse, taking temperature, as well as performing phlebotomies, ear irrigation and EKG’s. Acts as chaperon as needed. Uses universal blood and body fluid precautions at all times and personal protective equipment as needed. Responsible for prioritization of patient flow through clinical process.
3. Coordinates and manages patient care including pre-certifications, referrals, authorizations, prescriptions and scheduling of patients for clinic, hospital and other ancillary medical treatments and tests. Responsible for communicating test results to patient. Acts as a resource to clinician in order to provide optimal patient care. May administer medications in accordance with established policies and procedures or as directed.
4. Provides clerical support with patient/client notifications and documentation to include but not limited to complete medical records including labs, authorizations, X-Rays, hospital records and any previous care pertaining to each patient visit. Provides complete and accurate information necessary for billing related to the patient encounter in a timely fashion.
5. Maintains a clean, functional environment including cleaning and disinfection of equipment, exam rooms and storage areas. Disinfects equipment and instruments using appropriate solutions following sterilization procedures. Follows manufacturer's recommendations and OSHA guidelines in handling of hazardous substances. Performs basic maintenance of equipment and coordinates needed maintenance for prompt repairs.
6. Orders supplies as directed, stocks patient care areas. Medication ordering and tasks related to all medications (e.g. expired medications/equipment and recalls).
7. Develops and maintains effective communication mechanism with all external and internal customers. Exemplifies service orientation and the team concept in all contacts.
8. This position will complete all necessary on-boarding/orientation activities, including simulation training, as needed or directed.
9. This position works under the direct supervision of the providers. This position interacts with all levels of staff in a variety of departments, physicians, patients, families and external contacts, such as employees of other health care institutions, community providers and agencies, concerning the health care of the patient. Also interacts with physician’s offices in order to report, ask for or clarify information. Prioritizes data from multiple sources to provide support for the response of the patient and family to changes in health status.
Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. The Registered Nurse (RN) with Homecare is a professional caregiver who assumes responsibility and accountability for assessing, planning, implementing and evaluating care of Home Health adult and pediatric patients and their families in their home setting. Minimum Qualifications:
- Current RN License in state of practice.
- Basic Life Support (BLS) Certification for healthcare providers.
