Responsibilities:
Provide optometry services commensurate with community standards which include, but are not limited to:
Diagnosing and treating a wide range of eye diseases as well as helping to facilitate the management of related systemic conditions such as diabetes, high blood pressure, thyroid disorders, sexually transmitted diseases, and rheumatologic conditions.
Providing treatment of visual disorders through prescription of corrective lenses (to include contact lenses when appropriate according to policy and subject to approval of the Clinical Director) and vision training and pharmacological means.
Performing minor surgical procedures including, but not limited to, anterior segment foreign body removal and suture removal.
Completing ocular tonometry exams and full visual field tests.
Recognizing the need for referral to other providers, such as ophthalmologists, and following proper procedures for ordering consultations outside of the institution.
Minor repair, fitting, and adjustment of eyeglass frames.
Accomplishing all procedures within standard infection control protocols.
Assisting in patient dismissal from the clinic upon completion of treatment as well as ensuring follow up care instructions are understood.
Maintaining optical examination equipment through normal procedures including cleaning and sterilizing as well as other minor repairs. Any defective equipment should be properly reported for corrective action to be taken.
Properly storing and maintaining adequate levels of supplies in assigned rooms and operatories. If, at any time, the incumbent notices any supplies running low in the supply room, he/she immediately notifies the person responsible so that supplies can be reordered.
Ensuring instruments and unapproved items are not left in the clinic during non-patient care hours and that proper storage and security procedures are adhered to, as instructed.
Properly processing and documenting examination results into the patient electronic medical record as well as assisting in audits and reviews of records.
Properly containing and disposing of hazardous biological waste following all applicable laws, policies, and local procedures.
Completing clerical and support duties, including utilization of institutional-specific computer programs such as SENTRY, as necessary.

Individuals providing optometry services under the contract award must meet all the following requirements:
* Education: Applicants must have a Doctor of Optometry degree from an educational program that is accredited by the Accreditation Council on Optometric Education (ACOE) at the time the degree was obtained.
* Licensure: Applicants must have received a passing score on the National Board of Examiners in Optometry (NBEO) examinations and possess a current, full, and unrestricted license to practice optometry in a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
* Cardio-pulmonary resuscitation (CPR) certification through the American Heart Association or the American Red Cross
Preferred qualifications and experience include:
* Experience in providing healthcare in a correctional setting.

Qualifications:
Master’s degree in Counseling or Social Work from an accredited school
Current WV licensure in the respective fields (LPC, LICSW or higher)
Minimum of three (3) years experience with at-risk families, knowledge of and experience in behavioral family therapy and trauma focused treatment with knowledge of behavioral modification desired
Supervisory experience
Ability to travel; driver’s license and insurance statement

Responsibilities:
Clinically supervise and facilitate clinical development of assigned therapists and staff in order to assure quality services and supports provided are consistent with client needs and maintain fidelity of Crittenton Services’ trauma informed service model.
Provide on-going clinical supervision, support, training, and consultation to assigned therapists and staff.
Schedule and facilitate weekly individual and group clinical supervision and case consultations with assigned therapists and staff.
Oversight, review, and approval of assigned therapist/staff clinical documentation to assure both quality of services provided and compliance with Medicaid standards.
Participate in Quality Assurance and Quality Improvement processes including client chart reviews, implementation of corrective action clinical measures (i.e, training).
Provide clinical supervision for therapists/staff pursuing their professional clinical license, as permitted by licensing body
Act as the clinical expert at each site. Will conduct site intakes/admissions to best match the needs of the client with the strengths of the therapist. If Clinical Supervisor is not available to schedule or conduct intake, they will oversee the assignment of each referral.
Attend bi-weekly the Site Leadership Meeting with Director of Wellspring Family Services.
Schedule and facilitate weekly staff meetings
Responsible for taking appropriate action, in coordination with Director of Wellspring Family Services, and HR, regarding disciplinary action.
Complete yearly staff performance evaluations in collaboration with Community Outreach Supervisor and 90-day reviews of new staff.
Maintain a consistent caseload, which shall be determined individually according to each site’s specific needs and circumstances
Attend treatment plans in person/telehealth depending on diagnosis of client and/or review treatment plans acting as the credentialed LPC/LICSW (as determined by the Director).
Complete and submit to the Director of Wellspring any travel vouchers, time sheets, requests for time off.
Must possess current certifications for all core trainings (CPR, CPI, First Aid) and attend a minimum of 4 agency trainings through the continuing education program.
Maintain professional credentials as required by the respective State licensing boards and provide proof of licensure and completion of CEUS yearly.
Maintain credentialing in at least one insurance network to promote fiscal diversity at the site.
Salary: $57,000 to $68,000 (based upon licensure and experience)+ $7,500 Sign-On Bonus!

Creates the initial electronic health record that serves as the foundation of the patient medical record that is utilized by all members of the healthcare team. Prevents creation of duplicate medical records that can cause treatment safety issues and billing problems. Follows and ensures compliance with the mandate of the organization’s accrediting bodies to use identifiers to positively identify a patient prior to the delivery of patient care to ensure patient safety.
Checks in and registers patients; obtains and verifies complete demographic, guarantor, and insurance information; discusses and collects co-pays and other out-of-pocket patient responsibilities.
