Required Qualifications
Must hold a current RN license in the state of employment.
Working knowledge of the MDS 3.0 (current version of minimum data set).
The MDS Coordinator participates in the Resident Assessment Process through completion of the pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs). Participates in developing individualized resident Care Plans, identifying the needs of the residents and projected outcomes as required by Federal and State regulations.
Major Duties and Responsibilities:
Resident Assessment & Care Planning: Conduct and coordinate MDS assessments, develop individualized care plans, and ensure timely updates for any significant changes in residents' conditions.
Compliance & Documentation: Ensure accurate completion and submission of MDS assessments in accordance with Medicare/Medicaid guidelines and state/federal regulations.
Interdisciplinary Collaboration: Facilitate care plan meetings, maintain effective communication across departments, and support continuous improvements in resident care outcomes.

Minimum of one (1) year of experience within the past two years, working as an Emergency Medicine Physician.
*Schedule:* The shifts are 0700-1900 or 1900-0700, and the schedule has both shifts available each day. Physician can pick the dates and shifts they want to work. They can only work 4 days in a week and there must be 12 hours’ break in between one shift and the next.
*Responsibilities*:
* Examines, diagnoses, stabilizes, and treats, patients presenting to the Emergency Department to include urgent and emergent conditions. Respond to codes, perform life saving measures and participate in mass casualty events.

Complies with facility policies and procedures for appropriately documenting, billing and coding for residents receiving speech language pathology and audiology services.
Provides resident information needed for the resident assessment instrument according to facility policy such as signs and symptoms of swallowing disorders, speech and hearing patterns, cognitive patterns, Brief Interview for Mental Status (BIMS), speech language pathology and audiology minutes and distinct calendar days, etc. during the defined observation period(s).
Provides resident information needed for prior functioning in functional cognition as required by facility policy (RAI User’s Manual) for Medicare Part A, Medicare Advantage residents upon admission.
Required Qualifications:
Master’s Degree, or equivalent, in Speech Language Pathology
A valid, unrestricted license in his/her therapy discipline in the state.

Key Responsibilities
Assess patients' physical conditions and develop individualized treatment plans.
Implement therapeutic exercises and modalities to improve mobility, strength, and overall function.
Educate patients and their families on treatment plans, exercises, and lifestyle modifications.
Monitor and document patient progress, adjusting treatment plans as necessary.
Collaborate with a multidisciplinary team to ensure comprehensive care.
Stay current with best practices and evidence-based treatments in physical therapy.
Participate in community outreach programs to promote health and wellness.
Qualifications
Doctor of Physical Therapy (DPT) degree from an accredited program.
Valid state license to practice physical therapy.
Strong interpersonal and communication skills.
Ability to work effectively in a team environment.
Commitment to continuous professional development.

The Heartbeat of Journey

Required Qualifications:
Associate degree from an accredited Occupational Therapy Assistant program.
Current state licensure as a Certified Occupational Therapist Assistant (COTA).
Strong interpersonal and communication skills, with a passion for working in long-term care settings.
Ability to work collaboratively within a multidisciplinary team.
Major Duties and Responsibilities:
Assist the Occupational Therapist in implementing individualized treatment plans to enhance residents’ physical, emotional, and cognitive functioning.
Provide hands-on therapeutic interventions, including exercises, activities of daily living (ADL) training, and adaptive equipment use.
Document residents’ progress and communicate updates to the care team to ensure continuity of care.
Promote a safe and supportive environment while fostering meaningful relationships with residents.
