The healthcare team at Horizon PACE will approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. Horizon has over 25 years’ experience providing care in Kentucky and is the largest provider of adult day health care services in the state.
Horizon has opened 4 PACE locations in Kentucky, and we are excited to expand services to older Kentuckians. Horizon has an excellent benefit package including competitive pay, medical, dental, vision, life coverages, as well as generous paid time off, holidays, 401K, and many more. Additional benefits to Nurse Practitioners, we provide an allowance of $2,500.00 annually for cost associated with continuing education. Retention bonus of $5,000 at 6 months, and an additional $5,000 retention bonus at 1 year, for a total of $10,000 in retention.
Under the supervision of the Medical Director, the Nurse Practitioner provides care and continuous medical coverage, directly provides services to Horizon PACE program participants. Demonstrates the knowledge and skills necessary to assess, plan, care for, and provide services to frail elder participants according to assigned responsibilities and Horizon PACE standards.
*Position Responsibilities:*
· Performs in person comprehensive history and physical on new Horizon PACE participants. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
· Conducts an in person reassessment semi annually and as needed.
· Integrates the primary care treatment plan into the overall plan of care developed by the Interdisciplinary Team. Interacts with team members to meet emergent and acute need of participants. Participates in discharge planning for acute and long-term placement.
· Evaluates and treats participants during acute illness.
· Oversees and manages participant’s use of medical specialist and inpatient care.
· May assist with participant to the hospital. May provides updates to Interdisciplinary Team. In conjunction with the Interdisciplinary Team plans and coordinates discharge.
· Refers participants to medical specialist as indicated, communicates and follows up with specialist regarding outcome of visit.
· Manages all care of participants in the nursing home: Provides regular visits as dictated by nursing home standards and participant need. Performs telephone contact with nursing home staff as required; admits nursing home participant to hospital when necessary.
· May participates in rotating night and weekend call.
· May function as a member of the Interdisciplinary Team.
· At the direction of the Medical Director, reviews medical outcomes/statistical and utilization data, including reviewing of necessity of hospitalizations, and use of specialists. When problems are identified he/she works with the Medical Director to establish best practices and works with clinical staff and Interdisciplinary Team to make appropriate procedural and or operational changes to avoid unnecessary hospitalizations and improve the health of participants.
· Completion and documentation of appropriate diagnostic coding.
· Advises the Medical Director in ways and means to establish better accountability of Horizon PACE services to participants and referral sources, keeps Medical Director aware of needed material and human resources as program expands.
· Assists with the development of policies and procedure, standards of care; performs on-going monitoring and evaluation of patient care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues.
· Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
· Follows all Horizon PACE Policies and Procedures and applicable Occupational Safety and Health Administration (OSHA) safety guidelines.
· Participates in continuing education classes and any required staff and training meetings. Maintains professional licensure and certifications.
· Provides information about Horizon PACE Program to interested individuals and groups in adherence to PACE regulations.
· Participates in and supports Quality Improvement Initiatives.
· Performs other duties as required or requested.

* Assist supervisor with various tasks to support department operations as directed; may also be assigned special projects.
* Perform tasks which are supportive in nature to essential functions of the job, but which may be altered or re-designed depending on individual circumstances.
* Assists participants and caregivers in filing grievances
*Experience & Education:*
· NP or APRN Licensure with no restriction and ability to practice within the state of Kentucky. Meets requirements of 42 CFR Part 410.
· Required to have one year’s experience working with a frail or elderly population required

An EMT-Paramedic provides the highest level of medical care and transportation to patients who access the EMS System. The Paramedic brings the hospital Emergency Department to the patient’s side and acts as the eyes and ears of the emergency room physician. Job Duties:
The EMT-Paramedic will satisfactorily perform all skills and procedures as designed by the Kentucky Board of EMS curriculum.
The EMT-Paramedic will satisfactorily meet the requirements of Trans-Care’s Medical Direction for affiliation and clinical oversight.
While on duty, the EMT-Paramedic will practice his or her skills and fulfill his or her duties within the Standard of Care Protocol, any physician order, and any direction by a higher medical authority.
The EMT-Paramedic will make himself/herself familiar with, know and follow all Trans-Care’s policies and procedures.
The EMT-Paramedic will respond to EMS calls as assigned by the communications center observing the policies of Trans-Care on “out of chute” and “end of shift” time.
The EMT-Paramedic will operate and oversee the operation of his or her partner of the assigned vehicle under the rules of the Commonwealth of Kentucky and the policies of Trans- Care.
The EMT-Paramedic will know and follow all Trans-Care policies and procedures regarding normal operations and treat all Trans-Care customers, patients, fellow employees and/or vendors with honesty and respect according to Trans-Care’s Code of Conduct.
The EMT-Paramedic will observe a strict standard of patient confidentiality regarding the access, possession, or transmission of Personal health information (PHI) per Trans-Care’s privacy policies designed to protect such information from access by unauthorized persons.
The EMT-Paramedic will safeguard the privacy of all patients protected health information in accordance with Trans-Care privacy policies, procedures, and practices, as requested by federal and state law, and in accordance with Trans-Care policies of professionalism as a health care provider.
The EMT-Paramedic will report any concerns regarding real or possible breach of policies and procedures regarding patient confidential information and any observed practices by fellow employees which appear to violate these policies and/or procedures to the Privacy Officer at his/her earliest opportunity either in person, by email or anonymously as provided by federal regulation.
The EMT-Paramedic will be courteous and professional at all times while on duty. That shall include interactions with Trans-Care’s customers, its patients, and communications center, and his or her supervisor(s). Attitudes that demonstrate sarcasm, argumentativeness and hostility are not acceptable behaviors.
Prior to each shift and in accordance with Indiana law, the EMT-Paramedic will account for, check operability of, and be responsible for assuring that all required Advance Life Support equipment is operating, functional and located on her or her assigned unit.
The EMT-Paramedic will check in, check out, and account for all narcotics as required by state, DEA and/or EMS rules and Trans-Care policies and check all medication on board for expiration dates and replace as needed.
The EMT-Paramedic will maintain his or her in-service education as prescribed by the Indiana State EMT Agency.
The EMT-Paramedic will operate his or her assigned vehicle and personal conduct under the laws and rules of the Commonwealth of Kentucky and the safety policies of Trans-Care including but not limited to procedures designed to enhance and provide the crew and patient safety at all times.
The EMT-Paramedic will submit documentation that is complete, accurate and timely prior to leaving his/her shift at the designated time for departure.
The EMT-Paramedic will help maintain a clean and hazard free work environment by performing such cleaning chores as requested both on and in a vehicle and station.
The EMT-Paramedic will be responsible to precept Paramedics and EMT-Basic students and new employees as assigned.
The EMT-Paramedic will advise of all vehicle problems needing attention or repair and submit the proper report(s) necessary for correction per Trans-Care procedure.
The EMT-Paramedic will maintain himself/herself in a fit condition to be able to perform lifting, bending, stooping and other associated physical activity without injury.
The EMT-Paramedic will also perform any reasonably related duties as prescribed by his/her supervisor.

Required Qualifications:

Provides resident information needed for the resident assessment instrument according to facility policy such as signs and symptoms of swallowing disorders, speech and hearing patterns, cognitive patterns, Brief Interview for Mental Status (BIMS), speech language pathology and audiology minutes and distinct calendar days, etc. during the defined observation period(s).
Provides resident information needed for prior functioning in functional cognition as required by facility policy (RAI User’s Manual) for Medicare Part A, Medicare Advantage residents upon admission.
Required Qualifications:
Master’s Degree, or equivalent, in Speech Language Pathology
A valid, unrestricted license in his/her therapy discipline in the state.
Knowledgeable of skilled nursing home regulations, procedures, laws, regulations and guidelines pertaining to long-term care.
