*Position Highlights:*
· Hours: Monday-Friday ~7am to 4pm (flexible start time)
· No evenings, weekends, or call.
· Part-Time (2-3 days/week) with growth potential to Full-Time (4-5 days/week)
· Uncapped W-2 earning potential, directly proportionate to provider availability, EMR documentation, number of patient encounters per month, number of facilities covered, etc. ($215k-$225k annually projected for full time Physicians)
· Instruments provided by AmeriWound, such as, debridement kits & hurricane spray.
· Dedicated nurse to round with at each facility.
· Company laptop provided w/ EMR Aprima.
· Malpractice provided for both full and part-time providers.
· Mileage reimbursement based on standard IRS guidelines.
· We serve as your office and administration so you can spend more time with your family!
*Responsibilities:*
· Round weekly at assigned facilities within territory.
· Diagnose patient conditions via physical examination and/or telemedicine.
· _Physical Demands: Standing and walking for long periods of time, bending, kneeling, proximal and distal dexterity, lifting, carrying, pushing, or pulling up to 50 lbs._
· Perform medical procedures i.e., debridement as necessary.
· Communicate and collaborate on findings and treatments with facility clinical staff.
· Communicate information to patients regarding diagnosis and treatment goals.
· Document each patient encounter in EMR - APRIMA and maintain patient medical records on company assigned laptop computer.
· Facilitate referrals to other healthcare professionals and medical facilities as necessary.
*Qualifications:*
· MD, DO, PA, CRNP.
· Board Certified/Board Eligible.
· Active, unrestricted state medical license.
· DEA license.
· Valid driver license.
· Ability to build and maintain rapport with facility staff and patients.
· We require a high degree of technical skill utilizing software, including proficiency documenting EMR w/ strong attention to detail.

Latitude Acupuncture & Wellness is seeking a licensed acupuncturist for a busy and growing clinic South Burlington, Vermont.
We are looking for a candidate to grow with our expanding practice. Candidate should be self-motivated and able to perform independently in a relatively fast-paced environment. The clinic has 4 treatment rooms with 2 providers on shift concurrently. Experience with functional or integrative medicine is valued, but we recognize that practitioners come from different educational and clinical backgrounds. Knowledge of Chinese herbs is preferred but not required.
The practice is owned and led by an RN, licensed acupuncturist, and Chinese herbalist with approximately 25 years of healthcare experience. The clinical environment is designed to support collaboration between different disciplines while maintaining high standards of professional care.
You will have the opportunity to work alongside another acupuncturist as well as our massage therapy team, with the broader goal of creating an integrated experience for our patients.
Our practice sees patients with a wide range of concerns, including:
* Acute and chronic pain
* Autoimmune and inflammatory conditions
* Fertility and reproductive health
* Women's health
* Digestive and gastrointestinal concerns
* Stress and trauma-related concerns
* General wellness and preventative care
Prospective candidates should be:

* Interested and willing to learn new treatment techniques when appropriate

*Job Description Summary*
Responsible for assessments and treatment of patients. The therapist develops a plan of treatment which is approved by the physician. In the delivery of treatment, the therapist uses independent judgment based on common principles of pathophysiology and accepted standards of care. ABOUT THE ROLE:
The Licensed Mental Health Clinician specializing in the pediatric population will provide mental health treatment to children in the Southern-Central Vermont area. FUNCTIONS OF THE POSITION:
Provide Emotional Support: Offer emotional support and counseling to pediatric patients and their families, helping them navigate the challenges associated with illness, hospitalization, and social needs.
Assess Social Needs: Conduct comprehensive assessments to determine the social, environmental, and psychosocial needs of pediatric patients and their families. This includes evaluating their home environment, support systems, financial resources, and community resources / social determinants of health.
Develop Care Plans: Collaborate with medical professionals, including doctors, nurses, and other behavioral health team members, to develop individualized care plans that address the specific needs of each pediatric patient and their family. These plans should incorporate social, emotional, and developmental interventions.
Conduct individual, family and group therapy sessions using evidence-based therapeutic techniques.
Provide Education: Deliver education and support to pediatric patients and their families about coping strategies, disease management, and community resources available to them. This may involve conducting group sessions or providing individualized education.
Collaborate with Multidisciplinary Team: Work closely with a multidisciplinary team, including physicians, nurses, psychologists, and care managers, to develop and implement comprehensive treatment plans that address the holistic needs of pediatric patients.

EDUCATION/ SKILLS REQUIRED FOR SUCCESS:
Master's degree in social work (MSW), marriage and family therapy (MFT), clinical mental health counseling (CMHC) or similar from an accredited institution is required.
Licensed clinician required (LMHC, LADC, LICSW, LMFT).
Prior experience in pediatric social work or a related field is highly desirable.
Experience in primary care settings is a plus.

Major Responsibilities:
- Evaluates, diagnoses, treats, and manages acute, critical and chronic patients illness and diseases within both an outpatient clinic and inpatient hospital setting.

- Provides care coordination, which includes managing a variety of ancillary services, pre-operative plans, discharge planning, provider referrals and medical devices such as insulin pumps, defibrillators, and pacemakers.
- Prescribes, recommends and regulates pharmacologic and non-pharmacologic therapies across multiple care settings, including controlled medications that are warranted by patient's diagnosis or changing medical condition.
- Prepares, dictates and signs patient care plans and summaries of patient care. Maintains records according to established standards and guidelines.

$18.85 - $28.30Major Responsibilities:- Cleans patient care areas, surgical areas, public/private restrooms, conference rooms, lobbies, offices, elevators, and stairwells.
- Cleans all environmental surfaces including ceilings and vents, walls, doors, furniture, beds, bathroom fixtures, and windows. Assists in cleaning emergency spills as requested.
- Dusts and damp mops floors in patient and exam rooms, corridors, stairwells, restrooms, and other areas of the facility.
- Replaces cubicle and window coverings.
- Gathers and disposes recyclables, trash, and medical waste. Collects hazardous trash following established infection control guidelines.
- Sorts, stocks and delivers linens. Gathers and prepares soiled linen for transportation.
- Cleans and prepares dismissal and transfer units and certain equipment used in the care of patients and distributes non-medical supplies to specific areas.
- Fills and cleans supply dispensers.
- Inventories linens, cleaning supplies and paper supplies and orders additional in a timely and cost effective manner. Keeps housekeeping closet organized and clean.

- Must be able to bend, stoop, kneel, reach above shoulders, and crawl to complete tasks.

$20.80 - $31.20Major Responsibilities:- Reviews production sheets and recipes to ensure quality and quantity needed for items to prepare. Prepares food on daily production report according to established guidelines. Maintains time and production records.
- Measures and mixes ingredients according to recipe. Uses a variety of kitchen utensils and equipment including blenders, mixers, grinders, slicers and tenderizers to prepare soups, salads, gravies, desserts, sauces and casseroles.
- Bakes, roasts, broils and steams meat, fish, vegetables and other foods per recipes. Carves and slices meats and cheese.
- Garnishes, displays and serves food items according to standards and recipes.
- Ensures proper storage and rotation of inventories in refrigerators and freezers. Monitors food temperatures and utilizes established procedures to ensure safe food handling techniques and prevent bacteria contamination.
- Follows department standards for safe food handling, storage and sanitation. Maintain temperature logs for production areas per department standards.
- Monitors accurate portioning and promotes food waste control. Prepares in small batches to help control waste/cost.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Licensure, Registration, and/or Certification Required:
