*2. ESSENTIAL FUNCTIONS & RESPONSIBILITIES*
* Perform comprehensive physical therapy evaluations including mobility, strength, ROM, balance, transfer ability, and functional status assessment.
* Establish measurable, patient-centered therapy goals and develop individualized PT plans of care.
* Administer therapeutic exercise, neuromuscular re-education, therapeutic activities, gait training, and manual therapy as indicated.
* Complete and document GG motor scores within required timeframes in collaboration with the IDT.
* Supervise and direct PTAs and rehab aides in the delivery of physical therapy services within their scope of practice.
* Participate actively in weekly IDT meetings; communicate PT findings, goal progress, and discharge recommendations.
* Provide patient and family education on home exercise programs, assistive device use, fall prevention, and community reintegration.
* Recommend and fit assistive and adaptive equipment including orthotics, prosthetics, and mobility aids.
* Maintain accurate, timely documentation of evaluations, daily notes, progress notes, and discharge summaries in the EHR.
* Assist with or co-complete the IRF-PAI in collaboration with the Rehabilitation Program Director and attending physician.
* Coordinate with nursing on transfer techniques, positioning, and equipment needs for safe patient handling.
* Participate in QAPI activities and contribute to therapy department quality improvement initiatives.
* Maintain continuing education and competency in evidence-based rehabilitation practice.
* Adhere to APTA standards of practice, Code of Ethics, and Louisiana Physical Therapy Practice Act.
*3. EDUCATION & EXPERIENCE REQUIREMENTS*
* Required: Doctor of Physical Therapy (DPT) from a CAPTE-accredited institution.
* Preferred: One (1) or more years of physical therapy experience in an inpatient rehabilitation or acute care setting.
* Preferred: IRF, neurological, or orthopedic rehabilitation experience strongly preferred.
*4. LICENSURE, CERTIFICATION & REGISTRATION REQUIREMENTS*
* Required: Current, unrestricted Physical Therapist license issued by the Louisiana Physical Therapy Board (LAPTB).
* Required: Current BLS/CPR certification (AHA or Red Cross).
* Preferred: Board-Certified Clinical Specialist in Neurologic Physical Therapy (NCS) or Orthopaedic Physical Therapy (OCS) — APTA ABPTS.
* Required: Louisiana PT license must be verified through LAPTB prior to and throughout employment.
*5. KNOWLEDGE, SKILLS & ABILITIES*
* Expert clinical knowledge in physical therapy evaluation and treatment for stroke, TBI, SCI, hip fracture, joint replacement, and medically complex patients.
* Proficiency in GG motor scoring and IRF-PAI documentation requirements.
* Knowledge of therapeutic exercise prescription, neuromuscular facilitation, and gait training techniques.
* Ability to supervise and direct PTAs within Louisiana scope of practice guidelines.
* Proficiency with EHR documentation in an inpatient rehabilitation setting.
* Strong IDT communication skills including verbal and written reporting of patient progress.
* Knowledge of assistive and adaptive equipment prescription and fitting.
* Understanding of CMS IRF 60% Rule admissions criteria and medical necessity requirements.
* Commitment to evidence-based, patient-centered physical therapy practice.
* Knowledge of patient and family education techniques for functional recovery and home preparation.

Essential Functions:
Prepare and properly set up treatment rooms for hygiene procedures.
Review and update patient’s health history, chart progress, treatments, and home care compliance.
Clean calculus deposits, accretions, and stains from teeth and beneath margins of gums, using dental instruments, including scaling and root planning for patients with periodontal disease
Apply fluorides and other cavity preventing agents to arrest dental decay.
Examine gums, using probes, to locate periodontal recessed gums and signs of gum disease; communicate findings to patient and doctor.
Selecting, mixing, and placing materials on instruments and in the patient’s mouth, as needed.
Educate and motivate patient on proper oral hygiene and corrective/preventative measures.
Schedule appropriate re-care appointments.
Assist hygiene assistant in sterilization duties.

* Stay current with medical terminology, physiology knowledge, anatomy insights, and best practices in speech-language pathology to deliver high-quality care.
*Responsibilities:*
* Maintains a safe and effective environment.
* Verifies physician’s orders for treatment prior to performing evaluation as well as conducts full chart review. Verifies critical / abnormal lab values with nursing staff prior to patient treatment.
* Establishes, initiates and updates an appropriate plan of care (POC) in collaboration with the patient and / or family in accordance with physician orders.
* Establishes specific, measurable, attainable, realistic and timely long and short term goals relative to the patient’s age and lifestyle.
* Performs evaluations and treatments of cognitive / linguistic deficits, motor speech disorders and dysphagia.
* Communicates diet changes, need for MBSS and swallow precautions with the medical director directly or via the Director of Therapy.
* Conducts bedside swallow exams.
* Responsible for scheduling MBSS with physician order.
* Recommends and implements appropriate diet consistency change by writing an order with approval of medical director.
* Recommends design, fabrication, application or training in the use of assistive technology.
* Works with the patient and / or family as well as the interdisciplinary team to establish an appropriate discharge plan.
* Initiates patient / family education when appropriate in a timely manner in accordance with physician orders.
* Updates POC every 5-7th day of admission in accordance with IRF-PAI standards.

Education
Required - Graduate of an accredited school of nursing.
Preferred - Associate's or Bachelor’s degree in nursing.
Work Experience
Required - 2 years of nursing experience.
Certifications
Required - Current registered nurse (RN) license in state of practice.
Basic Life Support (BLS) from the American Heart Association.
Preferred - Certification in clinical specialty area.
Knowledge Skills and Abilities (KSAs)
Proficiency in using computers, software, and web-based applications.
Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
Excellent conflict resolution skills.
Ability to work a flexible work schedule (e.g. 24/7, weekend, holiday, on call availability) and travel throughout and between facilities.
Job Duties
Provides continuity of care by ensuring smooth transitions between care settings.
Develops relationships with patients and their multidisciplinary team to facilitate and/or navigate through subsequent treatment and follow-up to reflect continuity of care.
Manages high risk, complex patient care with the goal of minimizing readmission.
Works closely with physicians to coordinate patient’s care plan communication; works with multidisciplinary team to maintain and implement up-to-date coordinated patient centered care plan; communicates with all members of the healthcare team as patient advocate.
Prepares, executes, and reinforces post-discharge care plan.
Identifies barriers to care in an effort to elicit changes in processes for patients navigating the continuum of care.
Collaborates with leadership to review processes with the goal of improving the clinical experience for referred patients and the referring physician.
Adapts behavior to the specific patient population, including but not limited to respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
Performs other related duties as required.
