About Speak Up Speech Therapy
Speak Up Speech Therapy is an established pediatric private practice located in Clackamas, conveniently located just off I-205.
Our clinic serves children and adolescents with a wide range of communication and feeding needs, including:
* Speech sound and phonological disorders
* Expressive and receptive language delays
* Late language emergence
* Autism and social communication differences

Responsibilities
* Complete comprehensive speech and language evaluations for pediatric clients.
* Develop individualized treatment plans and functional goals.
* Provide evidence-based speech, language, communication, and/or feeding therapy based on your areas of competency.
* Complete timely clinical documentation.
* Educate and collaborate with parents and caregivers.
* Communicate with physicians, educators, therapists, and other professionals as appropriate.
* Monitor progress and modify treatment plans based on each child's needs.
* Maintain professional and ethical standards in accordance with Oregon licensure requirements and ASHA guidelines.
Qualifications
* Master's degree in Speech-Language Pathology or Communication Sciences and Disorders from an accredited program.
* Current or eligible Oregon Speech-Language Pathologist license.
* Pediatric experience preferred.
* Two or more years of experience preferred, although strong candidates with less experience are encouraged to apply.
* Strong Clinical Fellowship candidates may be considered.

DEPARTMENTAL SUMMARY: The Rehabilitation Department provides therapy and nursing services to patients, families and patient’s caregivers including: Rehabilitation Nursing Care, Physical Therapy, Occupational Therapy and Speech Language Pathology. Our department operates as a system department and provides services to Acute Care, Inpatient Rehabilitation Facility, Home Health and Outpatient settings across four campuses. Our Rehabilitation departments offer a variety of specialty services across the various services delivery areas.
POSITION OVERVIEW: The Occupational Therapistevaluates, plans, and provides treatment and rehabilitation as prescribed by a referring physician. Works with family and team members regarding patient’s abilities and goals.
This position does not directly manage any other caregivers.
ESSENTIAL FUNCTIONS AND DUTIES: RESPONSIBILITIES:
Adheres to Oregon State Occupational Therapy Licensing Board.
Provides occupational therapy evaluation and treatment to achieve desired outcomes.
Provides staff consultation and training.
Provides community resource knowledge and referral services.
Provides occupational therapy services to other hospital service areas.
May be required to drive Rehabilitation van and meet all related SCHS vehicle policy requirements.
Functions as a member of the Interdisciplinary Care Team to assure patient care is accomplished effectively and efficiently in a cost effective manner.
Participates in clinical care conferencing.
Gathers data and accurately documents information in a timely manner demonstrating compliance with departmental and professional regulations.
Participates with team members in facilitating patient’s and family’s learning throughout the continuum of care. Reinforces patient’s continued health care through teaching and/or referral to community agency follow-up.

QUALIFICATIONS

Your standard week consists of three days of scheduled cases (7:00 AM – 5:00 PM), one call day, and one post-call day off. Responsibilities
Responsible for helping patients maintain, restore and improve movement, activity and function.
Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning.

Qualifications
Current license to practice physical therapy specific to the state you're assigned to work in.
Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
Current CPR certification.
