*Responsibilities*
· Provides patient care through advanced health assessment, diagnosis, intervention, and evaluation; conducts physical exams; completes health history summary; documents patients’ progress; assists with patient care management.
· Develop treatment plans, treat patients with acute problems including illnesses and minor injuries following a predetermined protocol in a collaborative practice agreement with a physician.
· Prescriptive responsibilities are reviewed by collaborative physician.
· Supervise stress testing in the office
· Participate in professional development and lifelong learning.
· Communicating effectively with the providers and clinical staff regarding the assessment and ongoing treatment and care of our patients. Compliance with all applicable policies and procedures including, but not limited to, OSHA, PQRS, HR, CPT and ICD-10 compliance.
· Responsible for documenting accurately and completely in all E.H.R. systems in office settings.
· Working with office clinical staff in the regulation and management of INR patients.
· Responsible for authorization process if a peer to peer may be required in participation with insurance authorization team.
· Other job responsibilities as assigned.
Minimum Qualifications:
Master’s degree, license and valid certification as a Nurse Practitioner and/or Physician Assistant:
· Must meet all state requirements and be licensed in good standing to practice medicine
· Previous experience with the use of an Electronic Medical Records system; eClinical Works and EPIC is preferred
· Maintain ACLS/BLS certification
· Good organizational and communication skills
· Responsible for maintaining annual continuing education as required by professional licensing
· Construct and Interpret medical history
· Able to work independently
· 2+ years of professional clinical experience preferred

* Strong clinical judgment, communication, and decision-making skills.
* Ability to work effectively in a fast-paced, high-acuity environment.
