This job is responsible for the provisions of all facets of pharmaceutical care related to intravenous homecare patients. Assists with the referral process, including but not limited to the provision of medical information to patients and care givers relative to home infusion therapy. Performs patient assessment and care. Oversees the Patient Advocate team.To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education
Required – Bachelor of Science in Pharmacy
Preferred – Doctor of Pharmacy (PharmD)
Completion of a PGY1 or PGY2 Pharmacy Practice Residency
Work Experience
Required – None
Preferred – 2 years of experience as Pharmacist
Experience in specialty pharmacy
Certifications
Required – License to practice Pharmacy in the state of practice
Knowledge Skills and Abilities (KSAs)
Must have computer skills and dexterity required for data entry and retrieval of information.
Effective verbal and written communication skills and the ability to present information clearly and professionally.
Must be proficient with Windows-style applications, various software packages specific to role and keyboard.
Strong interpersonal skills.
Ability to use computer programs necessary for the successful review, authorization, preparation, dispensing and management of home infusion center patients.
Ability to remain familiar with literatures, references, and resources for therapies and care of patients serviced by the home infusion center.
Ability to maintain the confidentiality of patients and their personal information at all times.
Ability to maintain and foster an environment of teamwork and engagement.
Job Duties
Compounds sterile medications and infusions for the home, outpatient clinics, skilled nursing facilities, and physician/clinic environments.
Initiates infusion therapies with successful on-going evaluation of care and completion of required documentation relative to processing therapies, both fiscal and legal.
Assists in the teaching of patients prior to discharge to home.
Coordinates the deliveries with nursing and support services.
In the absence of the PIC, acts as a pharmacist in charge and makes decisions while on duty.
Participates in inventory control, ordering, warehouse organization, packing/shipping, equipment maintenance, and hazardous waste control.
When on-call, responsible for emergency on-call duties on weekends and weeknights as required by center scheduling.
Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
Performs other related duties as assigned.

Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Physical demand requirements are in excess of those for Sedentary Work. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work: (1) when it requires walking or standing to a significant degree; or (2) when it requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or (3) when the job requires working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of those materials is negligible. NOTE: The constant stress and strain of maintaining a production rate pace, especially in an industrial setting, can be and is physically demanding of a worker even though the amount of force exerted is negligible.
Normal routine involves no exposure to blood, body fluid or tissue and as part of their employment, incumbents are not called upon to perform or assist in emergency care or first aid.
The incumbent has no occupational risk for exposure to communicable diseases.
Because the incumbent works within a healthcare setting, there may be occupational risk for exposure to hazardous medications or hazardous waste within the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning and/or disposal of contaminated waste. The risk level of exposure may increase depending on the essential job duties of the role.

Certifications
- Required - Current Clinical Lab Scientist Generalist license in the state of practice (if applicable), OR Generalist Trainee permit or Lab Assistant license in the state of practice (if applicable) and must obtain Clinical Lab Scientist Generalist license within 90 days of hire.
- Required - ASCP, AMT or AAB generalist certification as a Medical Laboratory Scientist or Medical Technologist must be obtained within 90 days of hire. ASCP categorical or AAB individual discipline certification as a Medical Laboratory Scientist or Medical Technologist ONLY if working in a designated Academic Medical Center facility. Any incumbents certified by ASCP on or after January 1, 2004, must successfully complete the requirements of the ASCP's Certification Maintenance Program.
Knowledge Skills and Abilities (KSAs)
- Must have computer skills and dexterity required for data entry and retrieval of patient information. Must be proficient with Windows-style applications and keyboard.

Job Duties
- Performs, develops and interprets routine and/or specialized laboratory testing independently without error, within established time frames
- Performs, analyzes and records quality control and instrument maintenance/troubleshooting making judgments concerning results, and takes appropriate action necessary to maintain accuracy and precision. Escalates appropriately when results are out of control and ensures adequate documentation of corrective action
- Adheres to regulatory compliance guidelines and maintains an awareness of accreditation standards for laboratories by reviewing the most current checklist/standards. Follows safety guidelines, OIG compliance requirements and follows protocol for proficiency testing of samples
- Participates in performance improvement opportunities by assisting in the collection and analysis of data and supports business development and cost reduction initiatives by managing supply and reagent inventory
- Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style

This job provides respiratory care treatment, oxygen services, diagnostic testing and ventilation support services to both in-patients and out-patients ranging in age from neonatal to geriatric. The RT-CRT set-ups, monitors and weans Oxygen Therapy and Artificial ventilation. The RT-CRT draws, analyzes and reports Arterial Blood Gases and EKGs. The RT-CRT delivers intermittent therapy and assists with changing of artificial airways. The RT-CRT set-ups, monitors and weans Oxygen Therapy and Artificial ventilation. The RT-CRT draws, analyzes and reports Arterial Blood Gases and EKGs. Education

Work Experience
Required - 1 year of experience in Security, law enforcement, or military
Certifications
Required:
- Basic Life Support (BLS) from the American Heart Association
- Valid driver’s license in the state of practice and clean DMV report
- Crisis Prevention and Intervention (CPI), and firearms training
- International Association for Healthcare Security and Safety (IAHSS) Certification to be obtained within 90 days of promotion/hire
- Employment is contingent upon the successful completion of the FTO Program within the first 60 days of employment
Knowledge Skills and Abilities (KSAs)
Must have computer skills and dexterity required for data entry and retrieval of information.
Must be proficient with Windows-style applications and keyboard.
Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individual.
This position is considered Team A Essential Personnel and must report for duty in the event of a disaster.
Ability to read, write and speak the English language.
Ability to continuously walk the company property including outside areas, interior areas, using stairwells in 11 floor plus structure.
Job Duties
Enforces all company safety and security rules, regulations, and policies on company property.
Conducts routine inspections of on-site and off-site locations of the company.
Investigates accidents and other incidents which are assigned and reports all occurrences in a timely manner.
Performs opening/lockdown of all buildings as approved by Administration and performs other safety and security related duties as assigned.
Participates in emergency response and related preparedness drills.
Maintains required annual certifications and completes training requirements within required time periods.
Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
Other related duties as required.
