Deer Creek Dental Clinic is looking for one to two Registered Dental Hygienists to join our growing team in Tomah, WI. We are a technology-forward general dental practice with a strong emphasis on periodontal health, comprehensive dentistry, full-mouth rehabilitation, and cosmetic care.

A Hygiene Schedule Built for Quality Care
Our hygienists typically see *6–8 patients per day*, allowing time to focus on comprehensive patient care rather than rushing through appointments.
* Adult appointments: *60 minutes*
* Children's appointments: *60 minutes*
* Scaling & root planing: *90 minutes*
* Dedicated hygiene assistant *2 days per week*
* Clinic coordinator provides additional support the other 2 days
* Hygienists assist with local anesthetic while doctors complete exams
We are a perio-focused practice that believes strongly in diagnosing and appropriately treating periodontal disease. We're looking for a hygienist who shares our commitment to education, prevention, and providing patients with the care they need for long-term oral health.

About Deer Creek Dental Clinic
We are a general dental practice serving patients of all ages, from young children through older adults, with additional emphasis on high-end full-mouth rehabilitation and cosmetic dentistry.
Our office manager and doctor have worked intentionally to create a practice where team members are supported, appreciated, and enjoy coming to work. We value positivity, professionalism, teamwork, communication, and a drama-free workplace.
Candidates selected to move forward will be invited to spend approximately half a day shadowing our hygiene team. This is not a working interview, it is an opportunity for you to experience a normal day in the practice, meet the doctors and team, ask questions, and make sure the fit feels right on both sides.

The Role
We're hiring a Full-Time Physical Therapist to deliver in-home outpatient orthopedic and post-surgical care to patients across Washington, D.C. You'll manage your own daily route with the autonomy to actually treat your patients the way you trained to — no clinic walls, no double-booked schedules.
What You'll Do
Provide personalized, evidence-based in-home Physical Therapy — orthopedic, post-surgical, and total-joint rehab. Carry a manageable visit-based caseload with a sensible daily route (we don't run double-and-triple booked schedules). Use Prompt EMR and FOTO outcomes — efficient documentation that respects your clinical time. Collaborate with an experienced clinician-owner and small clinical team.
Who You Are
Licensed Physical Therapist in Washington, D.C. (or eligible to license within 60 days). Graduate of an accredited Physical Therapy program (DPT preferred). Skilled in orthopedic evaluation, manual therapy, and post-surgical rehabilitation. Comfortable working independently in patients' homes. Reliable transportation and a valid driver's license. Strong communicator with a patient-first approach.

DEPARTMENTAL SUMMARY: The Rehabilitation Department provides therapy and nursing services to patients, families and patient’s caregivers including: Rehabilitation Nursing Care, Physical Therapy, Occupational Therapy and Speech Language Pathology. Our department operates as a system department and provides services to Acute Care, Inpatient Rehabilitation Facility and Outpatient settings across four campuses. Our Rehabilitation departments offer a variety of specialty services across the various services delivery areas.
POSITION OVERVIEW: The Speech Language Pathologistevaluates, plans, and provides treatment and rehabilitation as prescribed by a referring physician. Works with family and team members regarding patient’s abilities and goals.
This position does not directly manage any other caregivers.
ESSENTIAL FUNCTIONS AND DUTIES
Adheres to Oregon State Speech-Language Licensing Board Standards.
Performs evaluation and treatment of patients with speech, hearing, language, voice, cognitive or swallowing disorders.
Provides community resource, knowledge and referral services.
Provides staff consultation and training.
May be required to drive Rehabilitation van and meet all related SCHS vehicle policy requirements.
Functions as a member of the Interdisciplinary Care Team to assure care is accomplished effectively and efficiently in a cost effective manner.
Participates in clinical care conferencing.
Gathers data and accurately documents information in a timely manner demonstrating compliance with department standards.

This is a Full-time night position working in the Med/Surg department 3 12-hour shifts 7pm – 7:30am, and every third weekend. This position is eligible for a $10,000 sign on bonus.
The Registered Nurse in the Medical/Surgical Department is accountable for the coordination of nursing care, patient education, and discharge planning.
Essential Functions
Use assessment skills and techniques to determine patient needs and condition upon admission and on an ongoing basis, responding appropriately.
Completes and documents the nursing admission assessment in a thorough and accurate manner as defined by unit policy.
Assesses the need for patient and family education.
Hourly Rounding on patients.
Initiate, develop and document patient plan of care based on patient’s assessment and medical diagnosis and setting priorities for nursing action in accordance with the unit’s policy on time-frames.
Review and update the plan of care once every 12 hours or as necessary.
Develop a patient education and discharge plan as necessary.
Collaborates with other health team members in planning and providing care.
Delegates care to team members based on complexity, staff education and competency, state regulations and Knoxville Hospital and Clinics’ policy.
Provides safe and comprehensive nursing care according to state regulations, hospital policy and procedure and DNV standards.
Establish care priorities based on patient problems; communicate with staff and new employees of patient problems, concerns, and changes in condition. Notify physician promptly regarding concerns.
Must be proficient in medication administration, including IV therapy, as evidenced by lack of medication errors, reports medication errors.
Perform procedures accurately, completely, and safely according to established policy and procedure and physician orders.
Provide care with minimal supervision in a timely manner.
Maintains a safe work environment for patients and coworkers. Uses safety precautions and PPE appropriately.
Use an interdisciplinary approach to patient care.
Use critical thinking skills to intervene with appropriate care for urgent or emergent patient care needs.
Job Requirements
Current license as a Registered Nurse in Iowa.
Current BLS with ACLS and PALS preferred within one year.
Mandatory Reporter Training.
One year general Medical / Surgical experience preferred.
Required to complete all annual competency training and maintain license and/or certification.
Care of acute and chronically ill patients.
Able to identify and manage life-sustaining physiologic functions in unstable patients.
IV and Central Line management skills.
