Position Title
Position Summary / Career Interest:
The Advanced Practice Provider (APP) in the Emergency Department (ED) manages health problems and coordinates health care in the ED setting. These APPs deliver patient care and medical services to individuals, families and groups by utilizing standards of care in accordance with State and Federal rules and regulations to assess health status, diagnose medical conditions, develop medical plan of care, implement treatments and evaluate patient responses in collaboration with collaborative/sponsoring physician as described in a Collaborative Practice Agreement.Responsibilities and Essential Job Functions- Able to evaluate medical problems of patients who present to the ED and appropriately requests consultation of other specialties when indicated.
- Provides education to patients and/or families about preventive care, medical issues and use of prescribed medical treatments and/or medications.
- Creates diagnostically appropriate treatment plans
- Documents all medical evaluation, diagnoses, procedures, treatments, outcomes, education, referrals and consultations consistent with NCQA, The Joint Commission, state regulatory standards and evidenced-based standards of care.
- Maintains documentation compliance for appropriate coding and billing.
- Facilitates evaluation of records by physician(s), peers and quality standards according to protocols and receives and implements constructive directives.
- May refer complex and high priority cases to collaborating physician with regards to complicated diagnostic problems, serious illness, complicated therapeutic problems and re-evaluation of chronic conditions.
- Provides medical and emergency interventions appropriate to the client’s needs; prescribes and monitors medications appropriate to the diagnosis; orders and monitors the appropriateness of emergency interventions.
- Maintains collegiality with all members of the inter-disciplinary team
- Utilizes appropriate chain-of-command and communicates with ED Program Director, APP Director and Medical Director of any clinical issue and contributes to quality improvement or process improvement as necessary.
- Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

Required Education and Experience
- Master Degree Nursing in Nurse Practitioner Studies OR
- Doctor of Nursing Practice in Nurse Practitioner Studies OR
- Master's Degree in Physician Assistant OR
Preferred Education and Experience
- Previous APRN or PA experience.
Required Licensure and Certification
- Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)
- Advanced Cardiac Life Support (ACLS) - American Heart Association (AHA)
- PALS - Pediatric Advanced Life Support - American Heart Association (AHA) within 60 Days
- Advanced Practice Registered Nurse (APRN) - State Board of Nursing for State of Kansas and Missouri (depending on practice locations). OR
- Licensed Physician Assistant (PA) - State Board of Healing Arts for State of Kansas and Missouri (depending on practice locations). OR
- Advanced practice certification required. APRNs may obtain within one year of date of hire; Specific certification may be required depending on specialty.

Job Duties:
The Clinic LPN provides primary clinical support to the provider, patient and their family to assist in the diagnosis and treatment of the patient's condition. Job Duties:
- Obtains and documents patient medical history, chief complaint, vital signs, and provides basic medical data base for provider, and prepares patient for examinations.
- Provides direct clinical care as directed by provider.
- Assists provider in performance of minor surgical diagnostic procedures and ancillary diagnostic examinations as directed by provider to facilitate accurate diagnosis.
- Collects and prepares laboratory specimens and complete associated paperwork to expedite laboratory studies with maximum accuracy and efficiency.
- Executes and documents ongoing patient communication, as directed by provider, prioritizes communication to the provider, responds to patient questions and requests to ensure efficient use of provider time and timely response to patients needs Administers medications and vaccines including intradermal and skin testing under the direction of a provider and per scope of practice.
- Cleans and sets up examination and treatment rooms between cases, ensures required levels of asepsis, availability of appropriate instruments and equipment, and preparation of patient.

POSITION SUMMARY
This position is responsible for providing personalized coordination, clarification and communication of all financial aspects of care continuum, including insurance and authorization verification, registration, financial counseling and claims research for Oncology. CORE FUNCTIONS
1. Performs pre-registration/registration processes. Partners with the clinical care team to determine initial authorizations needed based on the predicted care treatment plan. Obtains patient insurance benefit information for all aspects of the treatment, including, but not limited to, inpatient and outpatient services, prescription drugs, and travel and housing, if necessary. Assesses need for alternative coverage sources.
2. Verifies insurance coverage and obtains authorizations and notifications throughout the patient’s treatment. Obtains all necessary signatures and documentation required by the patient’s insurance plan. Accurately and completely documents all information into the patient records system to ensure maximum reimbursement. Monitors and updates information regarding insurance data, authorizations, preferred providers and changes in patient’s treatment plan. Partners with the clinical care team and insurance provider to ensure continued coverage of patient’s care and maximum reimbursement and minimized financial impact to the patient.
3. Provides financial counseling to patients and their families and serves as the primary resource throughout the patient’s treatment. Discusses benefits and other financial issues with patients and/or family members during initial referral and during continuation of care. Advises patients on insurance and billing issues and options. Explains company financial policies and provides information as to available resources and avenues for alternative payment arrangements. Assists patients, families and team members in addressing insurance coverage gaps via alternative funding options.
4. Provides financial advocacy, assistance and support to patients and families, as needed. Assists patients who are un-insured to access other funding resources and completes required documents. Maintains current working knowledge of Medicare, Medicaid and other program benefits and criteria, particularly as they pertain to long-term care and low-income patients. May serve as a liaison between the facility and community in making community resources available to the patient and family.
5. Acts as a liaison between patient/PFS department/payer to enhance account receivables performance, resolve outstanding issues and/or patient concerns, and to maximize service excellence.
6. Calculates patient liability according to verification of insurance benefits, collects deposits and co-payments.
7. May provide leadership and training to other members of the financial team and serves as a resource for internal and external customers.
8. Works independently under general supervision, leads and follows structured work routines. Works in a fast paced, multi task environment with high volume and immediacy needs requiring independent decision making and sound judgment to prioritize work and ensure appropriateness and timeliness of each patient’s care. This position requires the ability to retain large amounts of changing payor information/knowledge crucial to attaining reimbursement for the services provided. This position is an integral part of the care team, as they serve as the primary contact for all financial aspects of the patient’s care, both for internal and external customers. MINIMUM QUALIFICATIONS
Requires knowledge as typically obtained through an Associate’s Degree, with a focus in social work, healthcare administration or finance.
Requires knowledge of medical terminology and an understanding of all common insurance and payor types, authorization requirements and alternative financial resources as typically obtained through a minimum of three years of diversified experience in a hospital Patient Registration/Financial Services setting. POSITION SUMMARY

As a Medical Assistant, you’ll play a vital role in the care team – supporting providers, connecting with patients, and helping ensure a seamless, compassionate experience every day.Â
As a key member of the ambulatory care team, you will:
- Welcome patients and support their transition into care.
- Take and record vital signs, medical histories, and health screenings.
- Prepare patients for exams and assist providers during procedures.
- Administer medications and vaccines as directed.
- Perform lab-related tasks including specimen collection and point-of-care testing.
- Document care accurately in the electronic health record.

About This Location:
Our main location is in the Medical Office Center at 17850 S Kedzie Ave., Suite 3100, in Hazel Crest, IL, with additional locations in Orland Park, South Holland, and Frankfort, IL. Our practice includes seven providers, four OB/GYN physicians, one gynecologist, two Advanced Practice Nurses and a dedicated social worker. We provide comprehensive obstetric and gynecologic care to women of all ages. In office procedures include colposcopy, endometrial biopsy, LEEP and hysteroscopy.

Physical Requirements and Working Conditions
- Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.
- Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies.
- Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.
- Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions.
- Maneuver foot pedals on carts or machines.
- Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed.
- May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.
- Must be able to respond quickly to changes in patient and/or unit conditions.

- Cleans and disinfects patient rooms, surgical suites, clinical areas, office spaces, corridors and stairwells as assigned by adhering to current industry best practice and established policy.
- Handles various waste streams including municipal waste, recyclable waste, pharmaceutical waste, bio hazardous waste, chemotherapeutic waste and sharps.
- Wears standard personal protective equipment and adheres to posted isolation precautions when entering an isolation room.

This position performs prescribed procedures in accordance with department/facility policies, procedures and protocols. This position ensures optimal care/treatment to patient population.
CORE FUNCTIONS
1. Performs procedures by following and verifying referring providers’ orders. Uses independent judgment and applies learned methodologies according to established policy and procedures.

Certificate or diploma from an approved/accredited Radiologic Technology program.
National Certification required from the American Registry of Radiologic Technologists (ARRT(R) or ARRT(NM)) or Nuclear Medicine Technology Certification Board (NMTCB (CNMT)).
State Licensure required for AZ, WY, NV; includes Radiography or Nuclear Medicine and CT license prior to hire (not required in Colorado).
Advanced Certification in Computed Tomography required within 18 months of hire from the American Registry of Radiologic Technologists - ARRT(CT) or Nuclear Medicine Technology Certification Board – NMTCB (CT).
Certification for BLS is required.

This position will assist the specialty pharmacist with preparation, compounding and dispensing of medications, non-clinical specialty assessments, order processing, and delivery setup for patients receiving prescription services from the Specialty Pharmacy. Assisting the specialty pharmacists with claims processing, inventory management, and all necessary reporting requirements to maintain URAC Specialty Pharmacy Accreditations and access to a network of limited distribution drugs.
CORE FUNCTIONS
1. Enroll selected patients in specialty pharmacy services. Communicate with patient and utilize electronic medical record to gather all relevant medical, demographic, and insurance information and input patient into Specialty Pharmacy software platform.
2. Work in a call center environment and communicate effectively with patients over the phone to overcome financial barriers to treatment. Communicate complex billing issues to patients in a manner easily understood.
3. Provide adherence management and adherence improvement services to patients of the Specialty Pharmacy.
4. Assist, provide, and maintain records for accreditation, licensure, manufacturer limited distribution agreements, payer contracts, and REMS programs as required.
5. Assist with expanding scope and continuous quality improvement initiatives across specialty pharmacy services.
6. Work collaboratively with specialty pharmacists and Fulfillment Center team to ensure an excellent patient experience.
7. Prepare, dispense, and compound appropriate amount of medication under the direct supervision of a pharmacist, and accurately deliver medication to the patient.
8. Follow policies, procedures, and regulatory requirements and perform other work assignments.
MINIMUM QUALIFICATIONS
Requires Pharmacy Technician licensure and active Pharmacy Technician certification OR Pharmacy Intern licensure in the state of practice. Incumbents in Arizona are also required to provide the AZ Board of Pharmacy Wallet Card at time of hire.Â
Minimum of one year of pharmacy technician experience required. Ability to attend and successfully complete 160 hours of on the job training required. Strong customer service focus, superior problem-solving abilities required. Ability to complete moderate to difficult mathematical calculations using a hand-held calculator required.

Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient’s status.
3. Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician orders and the nursing plan of care, in compliance with policies and procedures, standards of care, and regulatory agency requirements. Delegates appropriately, and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for whom care is provided.
4. Evaluates the patient's and family's response to care and teaching, and revises the plan of care as needed. Evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members. Evaluates patient's response and the effectiveness of patient teaching.
5. Documents assessment, planning, implementation and evaluation in the patient record. Documentation is legible, timely and in accordance with policy. Must possess a current, valid RN license in state of practice, temporary/interim RN license in state of practice, or compact RN licensure for current state of practice. BLS certification is required. Additional certification or continuing education may be required based on area of practice.
Relates throughout the interview process the experience, training and education needed to perform the job. Experience in the clinical area for which he or she is applying is desired. Must maintain clinical performance competencies appropriate to the area in which they work as demonstrated through annual validations. Banner Registry and Travel acute care positions require a minimum of one year experience in an acute care hospital setting. Experience must include working in an acute care setting within the past 12 months as a Registered Nurse in the specialty area. In a Home Health setting, team members must be able to operate personal vehicle and possess a valid driver's license. Auto insurance coverage is also required to be active and maintained.
