We are a *small, home-like skilled nursing facility in New Philadelphia, Ohio*, and we're looking for a compassionate and experienced *Physical Therapist to join our team on a PRN basis.*
Our size allows us to provide a more personal approach to rehabilitation. You won't just be treating a diagnosis—you'll have the opportunity to *know your residents, understand their goals, work directly with our nursing team, and see the progress you help create.*
This is a great opportunity for a PT who wants *flexibility without sacrificing quality of care.*
WHAT YOU'LL DO
As a Physical Therapist, you will be responsible for providing skilled physical therapy services to residents in accordance with professional standards, applicable regulations, and individual resident needs.
Responsibilities include:
* Complete physical therapy evaluations and re-evaluations.
* Develop individualized plans of care based on resident needs and clinical findings.
* Provide skilled physical therapy interventions.
* Establish appropriate functional and rehabilitation goals.
* Provide therapeutic exercise, gait training, balance training, transfer training, mobility training, and other skilled interventions as appropriate.
* Monitor resident response to treatment and modify the plan of care as clinically appropriate.
* Assess changes in resident condition and communicate concerns to appropriate members of the interdisciplinary team.
* Collaborate with nursing, therapy, physicians, residents, families, and other healthcare professionals.
* Participate in care planning and interdisciplinary meetings as appropriate.
* Provide education to residents, families, caregivers, and facility staff.
* Complete accurate and timely documentation.
* Assist with discharge planning and transition-of-care recommendations.
* Promote resident safety, independence, and highest practicable level of functioning.
* Follow all applicable federal and Ohio requirements, professional standards, payer requirements, and facility policies.
WHY WORK WITH US?A SMALLER FACILITY. A BIGGER IMPACT.
We believe rehabilitation works best when you *know the person you're treating.*
At our facility, you'll have the opportunity to:
*Build relationships.*
Get to know residents and their individual goals rather than treating them as a diagnosis or room number.
*Work with a supportive team.*
Our therapy and nursing teams communicate closely to provide consistent resident-centered care.
*Have flexibility.*
PRN scheduling allows you to work when you are available while still making a meaningful contribution.
*Use your clinical expertise.*
We value the professional judgment and experience our therapists bring to the team.
*See the results.*
From a resident taking their first steps after hospitalization to someone regaining the strength to return home, your work matters.
WE'RE LOOKING FOR A PT WHO IS:
* Compassionate and resident-focused.
* Clinically confident and professional.
* Dependable and responsive.
* A strong communicator.
* Comfortable working independently.
* Collaborative with nursing and the interdisciplinary team.
* Organized and detail-oriented.
* Passionate about helping older adults maintain or regain independence.
* Committed to quality documentation and clinical outcomes.
* Someone who enjoys working in a *small, personal healthcare environment.*
QUALIFICATIONSRequired:
* Graduate of an accredited Physical Therapy program.
* Current, active Ohio Physical Therapist license.
* Current CPR certification or ability to obtain according to facility requirements.
* Ability to meet facility employment and health requirements.
Preferred:
* Skilled nursing or long-term care experience.
* Geriatric rehabilitation experience.
* Experience with Medicare/Medicaid rehabilitation services.
* Experience working with medically complex residents.
PRN FLEXIBILITY
We understand that flexibility matters.
This PRN position is ideal for:
* A PT looking for additional hours.
* A therapist who wants to maintain a flexible schedule.
* A PT transitioning between positions.
* A therapist looking for occasional skilled nursing experience.
* A clinician who enjoys working with a close-knit team.
*We'll work with you to determine a schedule that fits your availability and our residents' needs.*
JOIN OUR TEAM
If you want to work somewhere that *knows your name, values your expertise, and puts residents first*, we'd love to hear from you.
