$26.55 - $39.85Major Responsibilities:
- Performs facility and equipment maintenance in accordance with applicable codes and standards. Documents through use of the computerized maintenance management system (CMMS).
- Proficient level of skill to maintain, troubleshoot and repair mechanical equipment such as motors, pumps, conveyors, doors, fans, boilers, heat exchangers, controls, etc. within defined parameters and with a moderate level of guidance/assistance.
- Proficient level of skill to maintain, troubleshoot and repair patient care equipment such as beds, wheelchairs, carts, tables, etc. where not maintained by other departments
- Proficient level of skill to maintain, troubleshoot and repair mechanical/electrical systems such as pneumatic tube, HVAC, refrigerators/freezers, ice machines, commercial kitchen equipment, electrical, plumbing, piped medical gas systems, specialized water systems etc. within defined parameters and with a moderate level of guidance/assistance.
- Performs other routine to moderately complex general maintenance functions in the area’s grounds maintenance, carpentry, painting, and snow removal.
- Respond and make repairs associated with emergencies such as leaks, floods, water outages, fires, power outages, equipment failures, etc. as directed or within defined emergency response plans.
- Respond to building occupant requests in a timely and professional manner. Performs zone maintenance rounding. Keeps requesters apprised of the status of corrective actions and service requests.
- Conducts preventive maintenance and performs equipment safety checks.
- Inspects and repairs fire doors, hardware, firewalls and smoke partitions.
- Reads blueprints and maintenance manuals to gain knowledge needed to complete assignments.
Licensure, Registration, and/or Certification Required:
- Driver's license required.
Education Required:
- High school diploma or GED equivalent
Experience Required:
- 2-4 Years of Experience in equipment maintenance
Knowledge, Skills & Abilities Required:
- Basic computer skills such as CMMS, building automation system, Microsoft Office.
- Good communication and interpersonal skills.
Physical Requirements and Working Conditions:
- Must be able to sit, stand, walk, lift, squat, bend, crawl, reach above shoulders, and twist frequently throughout the workday.
- Must be able to lift up to 15 lbs. from floor to overhead, lift up to 50 lbs. from floor to chest-level, lift and carry up to 50 lbs. at waist height a reasonable distance.
- Must be able to push/pull with 50 lbs. of force.
- Exposure to electrical and mechanical hazards and therefore knowledge of safety precautions.
- Position requires travel. May be exposed to road/weather hazards.
- Operates all equipment necessary to perform the job.

Primary shift 12pm-8:30pm Monday-Thursday, Fridays 9a-5:30pm. Rotating Saturdays every 3rd week. No Sundays and/or Holidays. May also provide coverage as needed for PTO and other staffing needs. Staff provide coverage at 2 locations when needed.
Pay Range:
$35.50 - $53.25Major Responsibilities:
- Accountable for positioning and immobilization of patients and proper selection of technical factors along with computer reformatting of acquired data to obtain magnetic resonance scans and/or computer tomography procedures of optimal quality.
- 1)Selects appropriate technical factors on an individual patient and procedural basis to provide optimal quality examinations.
- 2)Performs post processing of computer data to enhance data and image information.
- 3)Operates support equipment for MRI and/or CT, such as satellite console, additional disc drives, power injector, imaging formatters, and laser cameras.
- 4)Transfers patients to and from MRI and/or CT scanning table and prepares patient for examination by explaining procedure and answering any questions patient may have.
- 5)Initiates IVs for administration of contrast media for MRI and/or CT enhancement in accordance with departmental policy and utilizing universal precautions in the process.
- Accountable for securing pertinent history from patient, patient’s chart or family member to provide the radiologist with adequate information for examination interpretation and to ensure procedural criteria is met.
- 1)Assists in obtaining confidential history of patient, which includes allergy screening to ensure appropriateness of patient for examination as well as ensuring the administration of contrast agents during required procedures.
- Accountable for being compliant with established Accrediting Organization and department safety standards to ensure patients and staff well being at all times.
- 1)Maintains magnetic and radiation safety standards.
- 2)Maintains fire safety standards.
- 3)Maintains knowledge of department Code 10-2 policy.
- 4)Maintains knowledge of departmental disaster, evacuation and quench policies.
- 5)Maintains and constantly implements universal and infectious safety standards.
- 6)Maintains documentation of equipment malfunctions and Preventative Maintenance.

- Accountable for obtaining documentation for department records related to patient statistics, supplies, patient billing information and preservation of patient records
- 1)Documents confidential patient’s history to ensure appropriateness of patient for examination.
- 2)Documents patients charge data, modifying and initiating charges as procedure modifications are made per the radiologist.
- 3)Documents any allergic reaction, incident report regarding staff or patient situations.
- 4)Provides timely information for inventory of supplies.
- 5)Ensures all films are properly identified and numbered for each patient according to current department procedures for accurate interpretation by radiologist.
- 6)Assists in scheduling of examinations and documenting complete.
- Accountable for keeping abreast of current magnetic resonance and/or computerized tomography and new developments in the field
- 1)Responsible for attending inservices and educational seminars when appropriate.
- 2)Responsible for sharing with other technologists new techniques learned, as well as working with students when required.
- Accountable for maintaining good relations when dealing with all persons encountered in daily performance of job.

Education/Experience Required:
- 3 years general radiology experience
- ARRT Registry
Knowledge, Skills & Abilities Required:
- Basic knowledge of cross-sectional anatomy and usual CT procedures.
- ARRT Registry -IDNS License -CPR Certification
Physical Requirements and Working Conditions:
- Ability to work overtime when required. -Ability to be flexible in starting times to meet demands. (weekend rotation and flexible hours.) -Attends additional educational and safety inservices. -Ability to respond to work at short notice. -Exposed to radiation. -Ability to operate portable equipment. -Ability to deal with extremely ill patients. -Ability to communicate with patients. -Ability to adjust to flexible work hours. -Ability to work weekends, holidays, and emergency call-back. -Ability to participate in on-call program and available to department needs with minimal notice. -Ability to do light, medium or heavy lifting. LIGHT: Lifting up to 20-50 pounds occasionally and/or up to 10 pounds frequently. HEAVY: Lifting 50 to 100 pounds occasionally and / or 25 to 50 pounds frequently and / or 10 to 20 pounds constantly. -Ability to make independent professional decisions using good judgment skills. -Ability to practice high quality patient care and technical competence. -Ability to function with minimal assistance in performing job activities. -Ability to demonstrate oral and written communication skills. -May be exposed to hazardous chemicals, such as film processing chemicals. -Ability to attend out-of-town site visits to view equipment. *This position will need to recognize needs and behaviors of a variety of age groups of patients treated, i.e., understand child growth and development, normal patterns of adolescent behavior and look for signs of normal aging. Prior experience with various age groups is preferred.
- If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.

POSITION SUMMARY
This position provides managerial and clinical support to patients and staff. Uses depth and breadth of clinical management knowledge to ensure the provision of high quality, safe, and cost-effective patient care. Demonstrates care coordination expertise and oversees the leadership of clinical, financial, and personnel management of the Care Coordination department. This position supervises employees and participates in selection, orientation, counseling, evaluation and staff scheduling. Maintains clinical, leadership and post-acute care services knowledge and competency to evaluate outcomes related to delivery of Care Coordination services.
CORE FUNCTIONS
1. Oversees Care Coordination personnel as assigned. Provides and/or facilitates Care Coordination and/or other related activities and serves as a resource to other staff members. Promotes utilization management through interdisciplinary patient care and activation of the discharge plan per the care delivery model.
2. Directs personnel actions including recruiting, new hire actions, interviewing and selection of new staff, salary determinations, training, and personnel evaluations. This position also participates in the development of Care Coordination goals and objectives in accordance with company standards. Ensures all goals and objectives are met timely and effectively. Hires, trains, conducts performance evaluations, and directs the workflow for the staff. MINIMUM QUALIFICATIONS
RN: Requires a Bachelor’s of Science in Nursing or in a related field. Requires a current Registered Nurse (R.N.) license in state worked.
Social Worker: Requires a Master’s degree in Social Work. Requires Licensed Master Social Worker (LMSW) (equivalent*) or Licensed Clinical Social Worker (LCSW). An equivalent license applies to states that do not recognize an LMSW; therefore, the employee must possess a Master’s Degree and be a Licensed Social Worker.
For assignments in an acute care setting, Basic Life Support (BLS) certification is also required.
Required minimum of two years’ experience providing care in an acute patient setting. This position provides routine patient care to promote patient comfort and contribute to the overall patient plan of care. This position is responsible for on-going patient data collection and documentation in the medical records and provides a variety of unit support functions which contribute to the overall efficient operation of the nursing unit/department.
CORE FUNCTIONS
1. Provides assistance in activities of daily living and completes necessary tasks to promote patient health, including assisting patients with basic hygiene needs, maintaining an optimum level of mobility and providing assistance with nutritional intake as needed.
2. Collects patient specimens and data, including vital signs, input/output and other delegated measurements in accordance with unit/facility policy and standards of care. Documents objective data and routine aspects of patient care. Assists in the transportation of patient specimens.
3. Recognizes and reports abnormalities and/or changes in the patient’s health status and responses to treatment to a licensed professional. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for which care is provided.
4. Performs procedures and uses equipment as delegated by a licensed professional, consistent with demonstrated competencies and according to state scope of practice; procedures may include blood draws, foley catheters removal, EKGs, and EKG monitoring. Provides necessary supplies, support and assistance to medical staff and patients for unit specific procedures, under the direct supervision of a licensed professional.
5. Performs receptionist/clerical duties to support overall operating efficiency and effective flow of communication and information throughout the department. Participates in the resolution of customer problems or collects necessary data and follows through to appropriate person for problem resolution.
6. Maintains a complete patient record with admission, transfer, and discharge information, progress notes, and consent forms. Ensures that all reports and chart forms are scanned or filed into the patient’s record. Ensures that charting forms are labeled and accessible if they are needed. Maintains adequate supplies of frequently used, patient education materials. Assists in the ordering of patient specimens. May transcribe and process physician orders effectively using the electronic medical record.
7. Maintains adequate levels of office, dietary, and medical supplies and completes requisition forms when supplies need to be ordered. Reconciles supply order upon delivery. Monitors and tracks the use and whereabouts of electronic equipment such as patient ID scanners and lap top computers. Ensures that all are accounted for and in working order.
8. Based on area of assignment may assist with transporting patients, including pre-operative and immediate post-operative patients to critical care unit. Participates in OR room turnover.
9. This position works under supervision and has responsibility for assigned unit only. Incumbents work in a fast-paced, sometimes stressful environment with a strong focus on customer service. Interacts with staff at all levels throughout the facility, as well as patients, families and physicians
MINIMUM QUALIFICATIONS
High school diploma/GED or equivalent working knowledge.

Senior Registered Nurse - RN – Outpatient Vascular Surgery Office – Albany, NY - Full Time

What you will do:
The Senior Registered Professional Nurse has the responsibility and accountability to utilize the nursing process to diagnose and treat human responses to actual or potential problems of individuals or groups. The Registered Professional Nurse works within and contributes to an environment where the St. Peter's Healthcare Services mission is actualized, patient outcomes are achieved, and professional practice is realized.
Responsibilities:
- Brings patient to exam room, takes appropriate vital signs and documents in electronic medical record.
- Review and update medication list to ensure accurate and complete list in electronic medical record (EMR) available for provider review and submission.
- Complete referrals and tracks patients' compliance.
- Review prescriptions electronically and send prescriptions to providers for review and submission.
- Obtains patient consent for procedures as directed by provider.
- Performs pre-visit planning and reviews quality metrics.
- Retrieves telephonic clinical information from patients who call into the office.
- Monitors task list and completes tasks assigned by provider in a timely manner based on urgency.
- Educates patients regarding medication, testing procedures and home care techniques.
- Ensure proper labeling, handling and documentation for patient specimens.
- Follow up with patient regarding test results based on advice given by provider.
- Maintains a clean and safe work environment including disinfecting patient care areas and equipment.
- In conjunction with other nursing colleagues, maintains the medical supply cabinet and drug cabinet.
- Uses the electronic medical record to communicate effectively.
- Performs quality assurance duties as assigned.
- Provides a clinical visit summary (Patient Plan) to patient as requested including educational materials.
- Participates in daily Patient Care huddles as appropriate.
- Works cooperatively with all colleagues to ensure quality patient care at all times.
- Performs other duties as assigned.
What you will need:
- Associates or Bachelor’s degree in Nursing preferred
- HS Diploma/equivalent required
- Current unencumbered NYS RN license
- Basic Life Support certification
- 6 months previous RN experience
- Must be able to lift 20 lbs.
Pay Range: $39.22 - $50.02
Pay is based on experience and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

This individual coordinates the flow of information in a positive and timely manner. The individual would need to possess computer skills and pleasant phone manners. Medical terminology is preferred for this position but not required.
