Coordinates the referral and transfer of patients in an efficient and cost effective manner. Provides quality patient care considering age specific, developmental and cultural needs. Demonstrates unit/area designated competencies.General Summary
Responsible for facilitating patient transfer and/or transportation to alternative care settings i.e., Extended Care Facilities (ECFs), home, home with ongoing therapy, based on referral from Social Worker or Case Manager. Coordinates the referral and transfer of patients in an efficient and cost effective manner. Provides quality patient care considering age specific, developmental and cultural needs. Demonstrates unit/area designated competencies.
Essential Functions and Responsibilities
- Completes screening of patients' medical records and documentation of relevant clinical information via continuity of care tools i.e., ECF forms, and maintains accurate record of status of activities related to discharge plan.
- Facilitates referrals to community agencies as directed by Social Worker/Case Manager arranging transfer and/or transportation of patients to ECFs or home care settings with external agencies i.e., ECF, ambulance, wheelchair van.
- Collaborates with Social Worker/Case Manager to facilitate appropriate placement of patients to ECFs based on data obtained via Social Worker, patient medical record, PCIS, i.e., medical needs, reimbursement sources, patient/family choices.
- Communicates with ECFs to facilitate appropriate patient placement taking into consideration:
a. patient/family desires
b. patient clinical condition and needs
c. patient insurance coverage
- Investigates patient's insurance coverage for long term care and/or outpatient care, as needed to facilitate resolution of insurance coverage issues related to ECF transfer.
- Develops strong working relationships with various community agencies in order to promote expedient transfers.
- Maintains awareness of State and Federal Regulations and reimbursement issues as they pertain to continuity of care needs, ECF planning, and/or transportation processes.
- Provides information to Social Work and Case Management Department staff regarding changes and new developments in reimbursement and agency policies, and community resources
- Participates in orientation of new Social Work and Case Management Department staff regarding ECF and/or transportation work processes.
- Compiles departmental statistics regarding patient transfers for the purpose of preparing monthly and annual departmental activity reports as well as special studies related to length of stay.
- Develops, reviews and modifies referral tools and systems to enhance continuity of care planning processes.
- Independently organizes and sets priorities.
- Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offers solutions and participates in their resolution.
- Maintains the confidentiality of information acquired pertaining to patients, physicians, employees, and visitors to St. Joseph Mercy Hospital. Discusses patient and hospital information only among appropriate personnel in appropriately private places.
- Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.

Required Education, Expereience, and Certification/Licensure
Education: High school diploma required or equivalent and one year of specialized training including medical terminology.
Experience: One to two years or Healthcare Experience.
Required Skills and Abilities
- Interpersonal skills necessary to communicate effectively with Social Work & Case Management Department staff, and external agencies when gathering and exchanging appropriate patient information.
- Analytical skills necessary to gather and document patient clinical status and to facilitate transfer of patient.
- Organizational skills in order to assess urgency and complexity of referral and to facilitate appropriate and timely patient transfer.
- Basic computer skills.

Position Summary:
The Orderly will assist surgery colleagues and physicians in providing professional, quality, and service oriented care to the patient and family.
What the Orderly/ Patient Transporter will do:
- Performs patient transports.
- Responds to surgical room cleaning turnover needs including damp disinfectant dusting,
- mopping, removal of contaminated biohazard waste and linens.
- Assists in preparation of surgical procedures.
- Opens and assists with delivery of sterile supplies to the sterile field.
- Breaks down the sterile field and return contaminated carts to the decontamination area.
- Terminally cleans transport stretchers and returns them to point of care.
- Picks up assigned floor beds for the surgical patient post procedure.
- Transports lab and pathology specimens to various receiving departments.
- Assists with surgical shave preps and scrub preps as directed by the perioperative registered nurse.
- Assists with patient positioning as directed by the perioperative registered nurse.
- Assists with collection and transport of X-rays too the surgical suites.
- Assists with pick up and collection of blood products.
- Assists in transportation and patient care needs for PACU as directed.
- Performs surgical light maintenance demonstrated by changing light bulbs.
- Prepares operating room admission packets.
- Keeps the hallways and exits clear and clean.
- Monitors outdates on sterile supplies.
- Monitors medical gases for correct storage and inventory levels.
What the Patient Transport | Orderly will need:
- High school graduate.
- Preferred: Completion of a Medical Assistant Program or equivalent education and /or experience.
- Current cardiopulmonary resuscitation certification or completion of certification within probationary period.
- Preferred: prior patient care experience.
- Preferred: knowledge of medical terminology and sterile technique.
- Ability to push/pull and/or lift 50 lbs.

Required Qualifications:

What you will do:
Records and/or enters laboratory computer results of tests performed and advises supervisor spurious, unusual or highly abnormal results before release of results for reporting.
Reports results of tests ordered stat and highly abnormal or unexpected results in a timely manner and according to established policies and procedures.
Compares test results with previous patient test results when indicated, calling attention of supervisor or pathologist to unusual fluctuations.
Performs required quality control and equipment maintenance and calibrates and operates laboratory equipment used in performance of the tests pertinent to the section) to which assigned.
Notifies appropriate personnel of problems such as unacceptable specimens, equipment malfunctions, and quality control aberrations, in accordance with department policy on procedure.
Prepares standards, reagents, and other materials used in testing.
Contacts nursing unit or client office personnel regarding results and for clarification of requests and other routine problems which may arise.

Position Summary:
Performs professional duties related to a variety of radiographic technical procedures and CT Scan procedures using ionizing radiation for radiological diagnosis and therapy. Has the latitude for independent judgment, ingenuity, and initiative to complete the requested procedure within the scope of his/her license. Assists in mentoring student technologists in their clinical rotation of Trinity Health Grand Haven.
What you will do:
- Performs CT Scans of all types including head, body and extremity scans.
- Performs interventional CT Scans for biopsies or various drainage procedures.
- Positions the patient carefully and accurately to obtain the requested examination; adapts positioning due to variations such as trauma, patient age, patient body habitus, suspected or known pathology, etc. to produce diagnostic images according to developed protocols and accepted medical practice. Applies supportive and/or immobilization devices to obtain the requested examination as necessary.
- Selects the correct CT Scan factors (mA, kVp, slice width, helical vs. non-helical, etc.) to produce high quality images; adapts technical factors for unusual variations of the patient or other factors (pathology, etc.).
- Provides the appropriate radiation shielding and collimation to adhere to ALARA (as low as reasonably achievable) principles. Provides for the radiation safety of all persons in the area where radiation is employed.
- Performs office/clerical duties in the absence of clerical support staff.
- Places orders and charges in the Hospital Information computer as necessary.
- Gives pertinent preparatory instructions for the test.
- Obtains the patient’s name, date of birth, phone number and MPI number if known.
- Obtains pertinent clinical history that supports the procedure being ordered and consults the ordering physician or nurse when the information is inadequate.
- Obtains any required Laboratory values prior to the procedure.
- Secures all necessary supplies to maintain a prudent inventory level for patient care.

POSITION PURPOSE
Performs phlebotomy on both outpatient and inpatients. Performs specimen processing, computer order entry and other pre-analytic functions, as required. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Performs successful sample collection on outpatient and inpatients to include venipuncture blood draw, capillary puncture, infant heel sticks, and urine collection utilizing proper patient identification and customer service skills. Troubleshoots problems related to missing specimens, discrepancies between specimens and orders.
Obtains patient demographic and insurance information from a patient or patient advocate. Performs data entry through the electronic medical record system which may include insurance verification, issuing Advance Beneficiary Notice (ABN) and utilize automated systems to obtain and process insurance information.
Acts as a representative of the department, communicating with patients and physician offices. Responds to inquiries in a courteous and helpful manner following departmental policies and procedures.
Uses good judgment to decide when to contact a Medical Laboratory Scientist, Manager, or a Pathologist, as necessary.
Knowledge of reference sources for patient preparation and specimen collection and handling requirements to be able to assist other health care professionals. Able to access on-line websites for assistance, when needed.
May utilize a glucometer or perform waived urine testing prior to administration of glucola.
Must be able to work with minimal supervision. May be requested to travel to other phlebotomy service centers (PSC).
Completes specimen handling and processing including centrifugation of vacutainer and aliquoting/pouring over of specimens. Aliquots and adjusts pH of 24-hour urine specimens as required by location.
Maintains minimum supply levels and maintains cleanliness of workstation and of the section. Provides input on policies and procedures. Assists in training new employees and students.
Reviews and completes assigned continuing education documents. This includes annual mandatory education and laboratory procedures review and acknowledgment.
Performs and documents routine quality control of equipment (centrifuge, refrigerator temperatures) as required by location.
Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution.

Education:

Position Details:
This is a fully remote position. Work hours will be 8 to 430 or 7 to 330 Eastern.
Training will take place in person in Livonia, MI for one week (expenses paid). Onsite training is required for position. Onsite training would take place during the 2nd week of employment.
What you will do:
Evaluate medical records to identify diagnoses and procedures and accurately assigns and sequences ICD and CPT codes. Abstracts and validates information. Seeks out validating information as needed.
Conduct documentation spot checks and respond to audit feedback.
Ensure timely, accurate client care documentation for billing.
Monitors and informs manager of records that are not completed timely. Monitors, investigates and takes appropriate action for records that are not coded, billed, or rejected.
Stay updated on coding guidelines and reimbursement requirements.
Maintains participant confidentiality and abides by HIPAA guidelines.

Minimum Qualifications:

Virtual Registered Nurse (VRN) Role: The virtual registered nurse leverages technology to deliver patient care remotely, collaborating with bedside staff and the broader care team to ensure quality outcomes and timely discharge. Core responsibilities include, but not limited to:
- Conducting admission screening, patient/family education, discharge planning, and medication dual acknowledgments via secure digital platforms.

OB Licensed Practical Nurse ( LPN ) - Bronson Battle Creek Birthplace - OB / Labor Delivery / Mother BabyThe Bronson Birthplace in Battle Creek has 15 beds – 11 for labor/delivery/recovery/postpartum (LDRP) care, 4 for antepartum/post-op care, 2 operating rooms and 3 triage bays. Additionally, the unit includes a newborn nursery. Here, pregnant, laboring and postpartum patients, as well as healthy newborns, receive comprehensive care. LPNs on this unit have the opportunity to care for postpartum couplets, assist in operating room procedures and provide support during deliveries.
Under the supervision of the Registered Nurse, provides prescribed medical treatment and personal care services which are within the scope of the practice of practical nursing to laboring, ill, injured, convalescent, and disabled persons as delegated, appropriate to the patient’s level of acuity and in accordance with established policies and procedures of the department and organization. Under the supervision of the Registered Nurse, and according to established policies and procedures, the LPN assists with surgical procedures as a scrub member of the obstetrical/surgical team. Prepares instruments and maintains a clean and safe obstetrical/surgical environment. The LPN will assist the Registered Nurse in collecting data, accompanying patients and responding to patient/family requests for services. The LPN may pass medications and administer IV therapy, per medication administration policy. Employees providing direct patient care must demonstrate competencies specific to the population served.
High school diploma or general education degree (GED) and successful completion of an accredited program for Licensed Practical Nursing required.
Experience in health care delivery setting preferred.

- Maintains a clean, orderly environment.
- Performs the duties of a scrub nurse and surgical assist for assigned obstetrical cases. Reviews assigned procedures and ensures that all instruments and equipment are available and properly prepared according to established standards and individual provider’s preference. Assembles surgical trays and ensures that all equipment is properly assembled and in good working order. Scrubs and sets up sterile table; assists in accurately counting all instruments, needles, sharps and sponges; assists with gowning and gloving, assisting instruments, sutures, and other equipment as requested and anticipation of needs.
- Prepares operating room and Labor/delivery room for deliveries and procedures.
- Assists the Physicians/CNMs/PNPs and nursing personnel before during and after deliveries.
- Tears down the OR, LDRP room following procedures and deliveries, and assists house-keeping with cleaning as needed. Cleans equipment and returns to appropriate places. Prepares and returns instruments for cleaning and sterilization. Returns unused supplies to appropriate places.
- Checks, orders and restocks supplies inventories, instruments and equipment in appropriate areas. Rotates stock, stocks patient rooms, supply rooms, procedure carts, medication room, and utility rooms.
- Picks up and delivers specimens, equipment, meal trays, blood products and stat pharmacy orders.
- Cleans, cares for, and is able to utilize the computers and Central Fetal Monitoring systems in the obstetrical area.

- Answers telephones, takes and relays messages, serves a unit communication source.

Health Unit Coordinator-PT 11a-11pThe ED Unit Clerks serve as the department facilitators, aiding in all processes outside of direct patient care to ensure patient care can happen correctly and efficiently. This is achieved by working very closely with the ED Charge Nurse and the ED Flow Coordinator to help manage the department and patient flow within the department. The ED Unit Clerk is responsible for all clerical duties within the department; including answering a 13-line phone system, all encounter scanning, copying, and faxing. The ED Unit Clerk works directly with the physicians within their assigned team to facilitate and assist in various tasks to aid in patient care, such as, paging and contacting other physicians, obtaining medical records, and arranging transportation to various radiology departments. • Solely responsible for the paging and contacting of any and all physicians on call for every consulting and emergent medical service that BMH offers 24 hours per day. Utilizes SmartWeb and Emergin paging systems, as well as ITSM and Conveyant phone databases.
• ED Unit Clerks are responsible for calling and initiating hospital call downs for the ED. This includes tier 1 and tier 2 trauma calls, AMIs, and Strokes, as well as calling down Code Black Level 1 and 2 for the hospital.
• Monitors and operates the 13 bed camera Patient Safety Surveillance program for patients who require extra visual and auditory monitoring.
• Responsible for scanning all encounter level documents into patient charts for emergency room physicians, nurses, and ED Techs. Also provide encounter scanning for Case Management/MSW, for all law enforcement documentation and subpoena information, and for the ED Pharmacy Department. This ensures the proper documentation is compiled for patients, creating the most accurate and complete medical record.
• Arrange and facilitate all patient EMS transfers out of the ED including transfers to nursing facilities, home to patients’ current residence, to KNI for imaging not offered at Bronson, and to KPEP for those patients who qualify. ED Unit Clerks are the only ED employees trained to use the Bronson Transfer Tracking Application.
• Facilitates and monitors the ED’s admission process. Responsibilities range from ensuring the ED Physician pages the correct admitting provider, to broadcasting to the team when the patient has a clean and ready inpatient hospital bed assignment. This helps ED staff expedite moving patients to inpatient status in a timely manner in order to reach SBAR and GEMBA goals, as well as provide the best possible care to patients.
• Facilitates ordering priority 1 blood and initiates massive transfusion protocols with the blood bank. ED Unit Clerks also do the “running” for massive transfusions, which entails retrieving the products from the blood bank and delivering back to the ED.
• Responsible for obtaining medical records from any and all outside hospitals for continuation of patient care for ED Physicians. Collects and encounter scans all requested records into the patients chart.
• Create a daily Physician call schedule for all consulting and emergent services offered by BMH. This involves maintaining and collecting all on-call schedules for all relevant offices and medical groups in the Kalamazoo/BMH area.

Position Purpose
Join Trinity Health at Home and make a meaningful impact by providing compassionate, high-quality care to patients in the comfort of their homes. As a Homecare RN, you will deliver primary nursing care as ordered by the physician, develop individualized care plans in collaboration with the Population Health Case Manager, and ensure exceptional patient outcomes and satisfaction.
What You Will Do
Conduct comprehensive patient assessments at admission and ongoing intervals.
Develop and implement individualized, assessment-based care plans in partnership with physicians and case managers.
Deliver therapeutic interventions and education to promote independence and improve health outcomes.
Monitor and report changes in patient condition, adjusting care plans as needed.
Utilize motivational interviewing and health coaching techniques to support patient goals.
Document care accurately and timely in compliance with agency policies and regulatory standards.
Mentor and support team members to ensure quality care delivery.
Minimum Qualifications
Graduate of an accredited nursing program; BSN preferred.
Current RN licensure in the state of practice.
Minimum one (1) year of professional nursing experience; home health experience preferred.
Strong computer and technology skills.
Current driver’s license and reliable transportation.

Position Summary:
Patient Care Assistant, PCA works in a collaborative environment to deliver excellent patient care under the direction of a RN. A Patient Care Assistant, PCA is expected to assist the patient in maintaining good hygiene, activities of daily living and to support quality of patient care.
What Patient Care Assistant, PCA will do:
- Respond to patient call lights for assistance or by rounding.
- Provide or assist patients with personal hygiene including showers, oral hygiene and dressing/undressing.
- Transports patients as needed for procedures, transfers or discharge.
- Maintains a clean and uncluttered environment in patient rooms.

Minimum Qualifications:
- High School diploma or equivalent
- Certified Nursing Assistant or 6 months of experience direct Patient Care Assistant or Nurse Tech, or completion of at least 1 clinical rotation in Nursing School, or enrollment in the Trinity Health Muskegon PCA program.
- Well-developed communication skills, both written and oral
- Able to communicate effectively with individuals and groups representing divers perspectives.
- Articulates and demonstrates commitment to the philosophy, values, mission and vision of the organization.
