The Social Worker LCSW will support our Outpatient Heart Failure team located within Saint Francis Hospital within our Hospital Outpatient Department (HOD). Opened in 1977, Torrington Community Hospital is a critical access hospital that provides excellent medical care to residents in southeast Wyoming and bordering Nebraska communities.  We have a strong Therapies team with a lot of longevity along with cohesion to promote a great work/life balance. Evaluations and treatments are provided on a 1:1 basis, with an average of 8 patients per day with 25 beds. This allows for documentation to be completed during the workday.

As the Master Social Worker, Ambulatory Care, you will see patients in the multi-specialty clinic setting. You will assist patients and families to identify resources in the community. The top candidate will have the ability to support whole patient care/integrated care. They will assist patients and family with understanding the disease process, helping patients and families identify supports systems, and assist the patient with resources to make informed decisions about their healthcare. The selected candidate will work alongside a team of nurses, and therapist to help the patient utilize community resources.

This is a Full-time, Monday-Friday 8:00am-5:00pm position.

POSITION SUMMARY
This position develops, coordinates and provides social work services to patients and families who are faced with social, emotional and situational stressors precipitated by illness, injury, and/or disability. The goal is to aid in adaptation and empower the patient and the family to participate to the fullest of their abilities in the discharge planning process. This position provides developmentally appropriate care for the population that it serves which includes planning for the safe discharge and continuity of care, the ability to recognize and plan for the unique needs of all ages as well as the physically disabled, mentally ill, chronically ill and terminally ill patient.

CORE FUNCTIONS
1. Processes and facilitates the assessment with analysis of functional and psychological needs of the patient within the framework of his/her developmental stage, functional abilities, cultural milieu, and support network. Assessment of the relationship of the patient's medical needs to the patient's home situation, financial resources, and availability of community resources. Assessment of the social and emotional factors related to the patient's illness, need for care, response to treatment, and adjustment to care. Assessment includes: initial assessments, behavioral observations, test (MMSE, depression screening, substance abuse screening), resource identification - strength based, collection and analysis of information to determine an individual’s treatment needs.

2. Formulates a plan of intervention acceptable to the patient, family, and health care team. Facilitate adjustments to the plan of care when necessary to promote enhanced outcomes. Collaborates with all members of the healthcare team to develop, manage, and communicate patient needs and discharge plans.

3. Documents all interventions in the patient medical record both timely and accurately including all elements of the transitional care plan to include the discharge plan.

4. Provides advocacy, assistance, support, counseling and crisis intervention to patients and families. Facilitates hospital-sponsored support groups.

5. Functions in a liaison role between the hospital and community in making community resources available to the patient and family.

6. Maintains knowledge of Medicare, Medicaid and other program benefits to assist patients with discharge planning and choices. Knowledge of contemporary behavioral health and system theories relevant to health care, end of life dynamics, and interventions; grief and bereavement counseling, substance abuse identification and interventions, support of victims of abuse, neglect, or violence. Provides professional education to staff and community.

7. Gather / assessing information, apply counseling and developmental theories, utilizing diagnostic frameworks, and engage in collaborative treatment planning.

8. Has freedom to determine how to best accomplish functions within established procedures. Confers with supervisor on any unusual situations. Positions are facility based with no budgetary responsibility. Internal customers: All levels of nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External Customers: Home health agencies, nursing homes, insurance providers, volunteer services, county and governmental agencies and medical supply companies.

MINIMUM QUALIFICATIONS

Requires a Master’s Degree in Social Work, Counseling or related field (requirement is based on business need and regulatory compliance, all positions may not have this requirement).

Requires a Licensed Master Social Worker (LMSW) (equivalent*) or Licensed Clinical Social Worker (LCSW) or have a MSW with the requirement to become licensed within 6 months of hire date. An equivalent license applies to states that do not recognize an LMSW; therefore, the employee must possess a Master’s Degree and be a Licensed Social Worker. For assignments in an acute care setting, Basic Life Support (BLS) certification is also required.

Requires a proficiency level typically achieved with 3 years acute care hospital experience. Performsthe basics of polysomnographic testing and associated interventions under the direct supervision of a Registered Polysomnographic Technologist.

Performs comprehensive polysomnographic testing and analysis, and associated interventions under the general oversight of a Registered Polysomnographic Technologist and/or the Medical Director or designee.

POLYSOM TECH II REQUIREMENTS:

• A license to practice respiratory care from the Idaho Board of Medicine is required or a permit to perform polysomnography related to respiratory care.

• Must be enrolled, or have completed, the ASTEP program and register as an RPSGT within 1 year from hire by the Board for Registered Polysomnographic Technologists (BRPT) Required to be certified or registered as a Polysomnographic Technologist (CPSGT or RPSGT) by the Board for Registered Polysomnographic Technologists (BRPT)

• Minimum two years of direct clinical experience preferred. At hire, Basic Life Support certifications from the American Heart Association or American Red Cross are required.

• Documents routine observations, including sleep stages and clinical events, changes in procedure, and other significant events in order to facilitate scoring and interpretation of polysomnographic results.

• Implements appropriate interventions (including actions necessary for patient safety and therapeutic intervention such as continuous and bi-level positive airway pressure, oxygen administration, etc.)

• Follows “lights on” procedures to verify integrity of collected data and complete the data collection process (e.g., repeats the physiological and instrument calibrations and instruct the patient on completing questionnaires).

• Demonstrates the knowledge and skills necessary to recognize and provide age specific care in the treatment, assessment, and education of neonatal, pediatric, adolescent, adult, and geriatric patients.

• Performs difficult and unusual procedures and therapeutic interventions.

• Scores sleep/wake stages by applying professionally accepted guidelines.

• Scores clinical events (e.g., respiratory events, cardiac events, limb movements, arousals) according to center specific protocols.

• Input orders and task information such as account activation, notify and task completion in the powerchart system.

ESSENTIAL FUNCTIONS

• Demonstrates professional responsibility in the role of Social Worker.
• Complies with personnel policies and hospital safety policies.
• Maintains confidentiality when interacting with patients, families, personnel and the public.
• Demonstrates ability to gather timely assessment information including components of home environment situation, support systems, coping skills/stressors, strengths, grief/loss issues, adjustment to illness, identified needs, and goals. 
• Develops and carries-out intervention plan and make appropriate referrals.

MINIMUM QUALIFICATIONS

• Licensed as a Social Worker or Independent Social Worker through Iowa Dept. of Public Health and the State of Illinois preferred.
• Must possess a master’s degree from a graduate school of social work accredited by the Council on Social Work Education which included specialization in clinical practice preferred.
• Bachelor’s Degree in Social Work or related field required.
• One to two years of social work experience in a health-care related setting preferred.
• Excellent communication, diplomacy, and people skills required.

Trinity Health Of New England Medical Group is looking for a Medical Receptionist / Business Services Rep to join our team

The Business Services Representative is responsible for ensuring patient flow within the practice by performing all business functions such as greeting patients, answering phones, scheduling patient appointments, patient registration, insurance verification, copay and point of service collections, pre-certifications, prior authorizations, referrals, chart prep, manage incoming and outgoing faxes as well as provide clerical support to providers and staff.

What you will do:

• Greets and communicates with patients via phone or in person in a courteous and professional manner identifying themselves and their department.
• Answers telephones in a timely, courteous, and professional manner utilizing appropriate scripting.
• Processes calls to ensure patients are transferred to the appropriate staff member/provider in a timely manner.
• Schedules patient appointments based on department protocols to maximize provider productivity.

Minimum Qualifications:

• High school diploma or equivalent
• Experience within a medical office or billing department preferred.  Knowledge of patient billing and medical terminology preferred
• Excellent keyboard skills.  Requires proficiency in a Windows based computer environment and Microsoft Office products.  Accuracy and dependability required.
