Requirements:
Perform comprehensive patient assessments, including medical histories and physical examinations.
Diagnose and treat orthopedic conditions such as fractures, joint disorders, sports injuries, and degenerative diseases.
Order, interpret, and review diagnostic tests including X-rays, MRIs, and lab work.
Develop and implement treatment plans, including prescribing medications and therapies.
Assist with in-office and surgical procedures, including pre-operative and post-operative care.
Occasionally round on patients in the hospital.
Provide patient education regarding diagnoses, treatment plans, rehabilitation, and preventive care.
Perform in-office procedures such as injections, splinting, casting, and wound care.
Document patient encounters accurately and timely in the electronic medical record (EMR).
Collaborate with physicians and care teams to ensure continuity of care.
Ensure compliance with all regulatory, safety, and confidentiality standards.
Make appropriate patient referrals to practice physicians and other health care providers.
Lends assistance and provides solutions to scheduling personnel for patient triage problems as requested.
Performs other clinical duties as directed by supervising physician(s).
Participates in clinical meetings held by practice physician(s).
Performs call and hospital responsibilities as scheduled and/or requested by physician(s).
Coordinates with Clinic Manager regarding operational issues such as patient scheduling, medical assistance provided by certified medical assistants and nurses, and other patient-flow matters.
Complete the providers section of managed care referral forms for referral to specialty care and/or diagnostic testing as required by each individual patient's health insurance plan.
Complete the clinical portion of third-party payer inquiries and disabilities form as required/requested by billing personnel and/or the practice administrator or practice physicians.
Complete patient sick certificates and other associated patient care forms.
Knowledge, Skills, and Abilities:
Strong clinical knowledge of musculoskeletal anatomy and orthopedic conditions
Excellent diagnostic and clinical decision-making skills
Ability to assist in surgery and perform minor procedures
Strong interpersonal and communication skills with patients and team members
Ability to work efficiently in a fast-paced clinical environment
Attention to detail and strong organizational skills
Education:
Master’s degree from an accredited Physician Assistant program
Current certification by the National Commission on Certification of Physician Assistants (NCCPA)
Active state licensure as a Physician Assistant
DEA registration (or eligibility)
Basic Life Support (BLS) certification; Advanced Cardiac Life Support (ACLS) preferred
Prior orthopedic experience preferred, but not required
Expected Hours of Work
Full-time, typically Monday–Friday with occasional call or weekend responsibilities depending on clinic needs

The physical demands described herein are representative of those that must be met by an employee to successfully perform the essential functions of this job.
Must be able to sit or stand for long periods of time.

Position Summary:
The LPN is responsible for providing residents with the highest quality of direct routine care, under the supervision of nursing leadership. The LPN participates in the development and modification of the tenant Plan of Care, implementing interventions that are delegated, ordered, or prescribed by the RN or physician. The LPN collects resident health data and compares to normal findings and the established current health status, reporting changes and/or responses to interventions in an ongoing manner to the RN. The LPN assigns and monitors nursing tasks or activities of unlicensed nursing staff through, assessment of their knowledge, skill, and capabilities, clarifying expectations, addressing behaviors, or escalating. POSITION SUMMARY
The RN manages and supervises resident care ensuring the highest quality of care is received in accordance with the Minnesota BON standards, is guided by the Residents’ Bill of Rights and abides by the facility Quality Assurance and Performance Improvement (QAPI) program. Through the collection and analysis of data, the RN is accountable for performing a comprehensive assessment to establish the baseline health status of residents as well as to create a detailed Plan of Care that includes individual needs and preferences. Implements and Monitors Nursing Care: Oversees the delivery of nursing interventions, evaluates outcomes, and continuously updates care plans to enhance resident well-being and quality of life.
Ensures Compliance with Standards and Resident Rights: Delivers care aligned with Minnesota Board of Nursing standards, HIPAA, and the Residents’ Bill of Rights, promoting dignity, privacy, and equality in treatment.
Provides Direct and Specialized Nursing Care: Performs RN-specific tasks such as IV administration, PICC line care, and resident education, while recognizing and addressing scope-of-practice limitations.
Maintains Safety, Documentation, and Infection Control: Ensures a clean and safe environment, follows safety protocols, accurately documents care and medications, and upholds HIPAA and infection control standards.
Supervises and Manages Nursing Staff: Leads, delegates, and supports nursing staff; ensures proper staffing levels, provides shift reports, manages incidents, and participates in emergency and disaster procedures.

As MCCN continues to expand specialty care services, we are seeking a compassionate, mission-driven *Cardiologist (MD/DO) *to join our growing team. This is an excellent opportunity to build and grow a cardiology program within a well-established community health organization while making a meaningful impact on the health of underserved populations.
This position will support our North Hills and Sepulveda clinic locations and may be structured as either Part-Time or Full-Time to expand MCCN's specialty care services.
*RESPONSIBILITIES*, include but are not limited to:
* Provide comprehensive outpatient cardiovascular care in accordance with evidence-based cardiology guidelines and standards of practice.
* Evaluate, diagnose, and manage patients with cardiovascular conditions including coronary artery disease, heart failure, hypertension, arrhythmias, valvular heart disease, peripheral vascular disease, and hyperlipidemia.
* Obtain medical histories, perform cardiovascular examinations, and develop individualized treatment plans.
* Interpret and review diagnostic studies including EKGs, stress tests, echocardiograms, Holter monitors, and other cardiovascular testing.
* Manage and optimize cardiovascular medications and treatment regimens.
* Collaborate closely with Primary Care Providers and multidisciplinary care teams to ensure coordinated patient care.
* Identify patients requiring advanced cardiac diagnostics, interventional procedures, electrophysiology services, or surgical consultation and facilitate appropriate referrals.
* Educate patients regarding cardiovascular risk factors, disease prevention, smoking cessation, medication adherence, nutrition, exercise, and lifestyle modifications.
* Maintain complete and timely documentation within the electronic medical record (EMR) system.
* Adhere to Quality Improvement guidelines and participate in MCCN Quality Improvement programs and initiatives.
* Provide clinical consultation and support to Nurse Practitioners, Physician Assistants, and clinical staff as assigned.
* Participate in Provider Meetings, Quality Improvement Meetings, and specialty care development initiatives.
* Assist with the development and implementation of cardiology clinical protocols and workflows as needed.
* Serve as a preceptor instructor to Nurse Practitioner, Physician Assistant, or medical students assigned.
* Attend conferences and seminars when requested and comply with continuing medical education requirements.
* Provide educational trainings to providers and clinic staff on cardiovascular disease prevention, diagnosis, and management.
* Meet productivity expectations of approximately 20–22 patients per day while maintaining exceptional quality of care and patient satisfaction.
*QUALIFICATIONS*
* MD or DO from an accredited medical school.
* Completion of an accredited Internal Medicine Residency Program.
* Completion of an accredited Cardiovascular Disease Fellowship.
* Board Certified or Board Eligible in Cardiovascular Disease.
* Current California Medical License and DEA registration.
* Current BLS certification.
* Minimum one year of outpatient cardiology experience preferred.
* Experience working in community health centers, FQHCs, or medically underserved settings preferred.
* Bilingual (English/Spanish) preferred but not required.
* Ability to work across MCCN clinic locations as needed.

Overview:
All Ways Caring Homecare is seeking a Registered Nurse to provide direct and indirect care to clients in the comfort of their home environment.
The nurse supervises and directs the efforts of the Caregiver when involved in the client’s plan of care. Based on state regulations, nurses may perform administrative and/or field/clinical responsibilities.
External Job Description:
Provides skilled nursing care/treatment to clients per Physicians Orders and Plan of Care
Documents nursing actions of all care given to individuals and informs physicians and family accordingly
Administers and monitors medication and management as prescribed by physician
Conducts nursing assessment visit as needed
Follows policies and procedures according to operation, state and regulatory standards
Completes supervisory visits based on compliance and regulations to include review of plan of care, caregiver competency assessment, and completion of required documentation
Implements nursing support for individuals in a home setting
Works with Director or Nursing/Nurse Supervisor and/or Branch Manager to address the health needs of individuals
Participates in staff meetings, in-services and other meetings as needed
Ensures compliance with physician’s verbal orders and medical reports
Qualifications:
Degree in Nursing from an accredited school
One or more years of nursing experience preferred
Current and unrestricted license in the state of the operation
CPR professional certification
Excellent communication skills with all levels of personnel, internal and external to the company
Capable of working independently with highly confidential information
