Major Responsibilities:
Interprets physician referrals and conducts a speech and language evaluation of patients with various speech, language, voice, cognitive and swallowing disorders.
Establishes a written plan of care and implements individual treatment based on evaluation results, functional goals and by utilizing appropriate techniques. Monitors and evaluates outcomes and appropriately adjusts the individualized treatment plan based on the patient's response to treatment and provides status updates to physicians and other teammates. Identifies factors affecting patient's physical and psychosocial comfort and attends to these needs.
Plans for and completes patient discharge including providing written home programs, patient/family teaching, referral for community services, transition planning within the continuum of care and ordering equipment as indicated/appropriate. Communicates and collaborates with appropriate stakeholders to assist in achieving optimum care for the patient.
Completes all required documentation following department policy and accreditation standards with appropriate co- signatures, including patient evaluations, progress notes, team conference reports, and discharge summaries as appropriate. Record patient care and progress of each patient assigned. Completes daily statistics, billing, and scheduling. Applies insurance guidelines regarding authorization and number of visits. Responsible for maintaining site/department established productivity standards. Responsible for understanding and adhering to all billing and documentation policies, procedures, and timelines.
Responsible for cleaning up after patient treatment, putting away equipment, and keeping area neat and orderly. Performs various manual techniques, patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique. Demonstrates skill in use of modalities, exercise equipment and other devices that are available in area worked per department orientation checklist. Demonstrates knowledge of indications and contraindications of modalities. Promotes patient safety by participating in safety initiatives as requested and appropriately follows all safety and infection control measures pertaining to the general working environment.
Participates in performance improvement activities designed to identify and resolve problems affecting patient care outcomes and to improve quality of care. Manages assigned caseload, informing site leadership of coverage needs and ability to cover; assists with coverage, based on department needs and regional needs.
Pursues professional growth through participation in continuing education courses, department in-services, and reading professional literature. Orients new staff to clinical and department policies as requested. Participates in the evaluation process of co-workers by providing verbal or written feedback as requested.
May supervise non-licensed personnel following all applicable state and local compliance requirements. Includes delegating patients to assistive personnel while providing direction, recommendations regarding treatment and ongoing supervision while maintaining responsibility for patient plan of care and goals. Supports the student clinical education program by monitoring, observing, and providing work direction to a minimum of one student per year unless students are unavailable.
Minimum Job Requirements
Education
Master’s degree in Speech Pathology from an accredited school or educational requirements commensurate with the date of graduation; maintains continuing education requirements as mandated by applicable state professional standards.
Certification / Registration / License
Speech Language Pathologist license issued by the state in which the teammate practices. If the teammate supports multiple states, licensure in at least one of those states is required prior to hire.
Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless department leader has determined is not required) American Heart Association. Must Have CCC SLP.

$38.20 - $57.30Major Responsibilities:- Performs advanced cardiac ultrasound procedures such as M-mode, 2-D, Doppler and Color Flow Doppler utilizing a variety of equipment and techniques and makes the necessary measurements required for the interpretation to obtain accurate, high quality results (for example, muscle wall thickness, diameter of chambers, volume of chambers, valve areas, blood flow velocity and direction). May assist physicians in the Catheterization and Electrophysiology Labs by performing echocardiograph studies to aid them in proceeding with complex procedures to decrease risk factors.
- Identifies and defines cardiac abnormalities and extends the scope of the examination appropriately, deviating from standard techniques when necessary and correlating invasive and noninvasive cardiac diagnostic test results with cardiac anatomy and function.
- Prepares patients for procedures and obtains clinical history from the patient including cardiac related physical findings and pertinent laboratory data in order to adapt the imaging techniques to obtain comprehensive and diagnostic echocardiograms to alleviate the probability of erroneous clinical interpretation of the echocardiogram.
- Assists in performing transesophageal studies with cardiologist, calculating appropriate measurements and sterilizes probe.
- Communicates clinical findings to the appropriate staff.
- Records statistics and performs daily maintenance of the Echocardiography Lab and its equipment, reporting all malfunctions as appropriate.
- Acts as a resource to other staff, physicians and students regarding equipment, techniques, procedures and studies to optimize patient care.
- In conjunction with supervisor, implements protocols as new technology evolves, and ensures system echocardiography standards are implemented and adhered to.
- Works closely with medical director to develop and ensure quality processes and protocols are adhered to; works with physicians to perform quarterly reviews, and assists with ICAEL accreditation ensuring all policies, quality processes, reporting, etc. meet standards.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Licensure, Registration, and/or Certification Required:

Major Responsibilities:
- Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation which may include use of therapeutic assistive devices, proper positioning, range of motion and special transferring as indicated.
- Measures and documents blood pressures, TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. Documents accurately in Electronic Medical Record (EMR).
- Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately.
- Participates in admission, transfer and discharge process under supervision of RN including room set up, orientation to room, removing linen, and proper management of patient belongings. Ensures patient call light is always in reach of patient and answers call-lights promptly.
- Collects urine, stool, and sputum specimens and performs point-of-care testing as indicated using approved process for accurate specimen labeling and identification. Maintains competency as required.
- Completes various errands as directed including pick-up and delivery of trays, charts, specimens, and reports. Maintains and orders stock for nursing area. Cleans equipment and work areas.
- Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices. Performs tasks related to nutritional needs of patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection.
- Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. Ensures special patient supplies and appliances are provided and maintained such as TED hose and sequential compression devices.
- Under the direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care, and bladder scans.
- Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. This role assists with on-call schedules, fosters collaboration with internal and external stakeholders, coordinates hospital and trauma clinic schedules, ensures data quality, monitors productivity, and fulfills other key responsibilities under the direction of the leadership team.
Major Responsibilities:
Team Navigation: Assists the leadership with daily operations and team performance by guiding the team to meet individual and team goals, high standards of productivity, quality documentation, interdisciplinary collaboration, and fostering a supportive work environment.
Patient Support: Provides immediate emotional support and resources to hospitalized patients following a trauma, including connecting them with the clinical team when appropriate, assisting with discharge planning, leading support and psychoeducation groups, and safety planning. Resources Navigation: Responds to trauma alerts, safety calls, pre-escalation needs and collaborates with street outreach teams and facilitates mediation to mitigate retaliation risks.
Outreach: Guides the team, offers patient support, works alongside other groups, and actively engages in intensive community outreach. This also involves making follow-up telephone calls and sending letters to discharged patients to connect them with outpatient resources.
Continuity of Care: Schedules patient appointments with the TRC, other internal Advocate Health resources, or community partners before discharge and provides brief follow-up calls after discharge when necessary, to ensure ongoing care which requires strong collaboration with other providers.
Assessments: Conducts intakes and needs assessments, linking patients to appropriate resources and outpatient services. It may also require notifying other hospital providers of unmet needs to be addressed while in the hospital.
Record Keeping: Maintains accurate records of patient interactions and action plans.

- Provides on-going assessment of patient response to treatments and teaches and applies interventions as appropriate. Updates and revises plan of care as indicated and provides physician with status report on a regular basis.
- Communicates problems to physicians and management and clearly states plans, actions and goals so changes are reflected as they occur. Completes a plan of care as indicated and provides physician with status report. Maintains and updates clinical and patient records according to state and federal guidelines.
- Manages assigned caseload including scheduling and informing site leadership of coverage needs and ability to provide coverage. Manages patient's medical condition including medications. Acts as a patient advocate to insure optimal outcomes. Manages assigned caseload for optimal functional and financial outcomes; reviews caseload for Low Utilization Payment Adjustments (LUPA), optimal number of visits based on progress, Prospective Payment Plan (PPS), and Outcome Based Quality Initiatives (OBQI) outcomes.
- May manage a caseload of Physical Therapist Assistants (PTA) including evaluating their patients, insuring compliance with established Care Plan, and adhering to all State of Wisconsin regulations regarding PTA supervision.
- Performs Home Health Aide joint visits as appropriate to maximize patient's functional abilities in the home. May assist with evaluating patients, turning patient over to PTA and informing PTA of Plan of Care (POC) per State of Wisconsin regulations. Teaches appropriate treatment procedures and supportive activities to patients and caregivers and develops plan for obtaining adaptive devices as needed.
- Discusses caseload issues and changes in case census with management. Communicates problems to physicians and management, and clearly states plans, actions and goals so changes are reflected as they occur.
- Participates in chart audits and continuous quality improvement activities. Facilitates discharge planning process and utilizes interagency/system and community resources to assure continuity of care after discharge.

Conveniently located in Milwaukee County, MHEC serves as a central hub for crisis intervention. Our mission is to deliver 24/7/365 mental health assessment, stabilization, treatment, and transition care for children, adolescents, and adults. We provide a therapeutic environment that supports both voluntary and involuntary patients, ensuring safety and dignity for all. MHEC offers immediate crisis assessment and stabilization, specialized care with separate treatment areas for children/adolescents and adults, and comprehensive care coordination to support ongoing recovery and transition planning.
We’re Looking For:
- Completion of an accredited or approved program in nursing
- Registered Nurse license issued by the state in which the teammate practices
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association
What You’ll Do:
As a skilled and compassionate RN, you will play a pivotal role in providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures.
Using evidence-based practice, clinical decision making, compassion, and skills communication while leading efforts to create the safest patient environment and the best patient experience across the continuum.

Physical Requirements & Working Conditions
Requires frequent sitting, standing, walking, bending, squatting, reaching, twisting, and lifting throughout the workday. Must safely lift up to 35 pounds and perform patient handling tasks, including pushing/pulling up to 30 pounds and assisting with 150-pound sliding transfers. Requires functional speech, vision, hearing, touch, and fine motor skills for clinical procedures, as well as the ability to operate equipment foot controls. Must be able to respond quickly to changing patient conditions and perform patient safety interventions, including verbal de-escalation and restraints when necessary. Exposure to blood, body fluids, communicable diseases, chemicals, and hazardous waste may occur; appropriate protective equipment must be used. Physical Abilities Testing and additional department-specific requirements may apply.
