The Environmental Services Associate is responsible for maintaining a clean, safe, and sanitary environment for residents, visitors, and staff. This role supports the mission of St. Peter's Health Care Services through high-quality housekeeping services and adherence to infection prevention standards.
Key Responsibilities:
Clean resident rooms, common areas, fixtures, and surfaces according to established housekeeping procedures and schedules.
Follow infection prevention and sanitation protocols at all times.
Dust, vacuum, mop, sanitize, and maintain assigned areas to ensure a safe and welcoming environment.
Build positive relationships with residents, families, coworkers, and other departments.
Respond courteously and professionally to resident and staff requests.
Report maintenance concerns, safety hazards, or operational issues to the Housekeeping Supervisor.
Complete additional duties as assigned by department leadership.
Qualifications:
Ability to perform housekeeping and environmental services duties effectively.
Basic knowledge of cleaning methods and equipment.
Ability to lift, push, and pull up to 50 pounds.
Strong attention to detail and commitment to cleanliness and safety.
Excellent customer service and communication skills.
Previous housekeeping, environmental services, hospitality, or healthcare experience is preferred but not required.

At The Terrace at Glen Eddy, we are a unique team, with low turnover and content staff. We are big on teamwork and making The Terrace a place where we have time to provide the care each resident deserves. No lifts, no feeding, no treatment carts! Every nurse and aide is trained to assist with medication and ADL’s, and in the assisted living environment, it’s a great place to hone your caregiving skills. We are a beautiful, small, 42 bed assisted living located in the heart of Niskayuna, convenient to Schenectady, Latham, and surrounding areas. Position Summary: LPNs (Licensed Practical Nurses) are responsible for medications, treatments, and staff oversight under the direction of the RN. In this position you will bring trust, comfort, and empathy to all resident interactions and make a positive difference in the lives of our residents.

Clinical Competencies:
LPNs provide direct and indirect care under the direction of an RN including procedures, treatments, and related duties as assigned and within the NYS Scope of Practice for an LPN.
- Resident support – Deliver compassionate, person-centered care while fostering a sense of dignity and independence for each resident.
- Medication administration – Conduct daily medication pass with accuracy and care in accordance with physician orders and facility guidelines.
- Shift leadership – provide leadership for the care team during your shift, including guidance and support.
- Team collaboration – work closely with resident assistants, other nurses, and management in a cooperative, respectful environment.
- Consult with the RN on any changes in a care plan or schedule and communicate with patients and their families about their care plan.
- Observe, measure, record, and report clinical data relating to each resident's health status within licensure requirements.
- Attends and actively participates in in-servicing, orienting and educational activities.
Education Requirements:
- Must be a high school graduate.
- Must graduate from a nursing education program acceptable to the New York State Education Department (NYSED).
- Must be licensed with current registration in New York.
Must be able to:
- Stand, walk, and lift for long periods of time
- Read and write using medical terminology
- Working knowledge of computers including electronic medication administration records, processing of orders, checking email, and preforming required in-servicing, along with use of other required applications
- Have the visual and auditory acuity adequate to perform observations and patient care
- Maintain composure and positivity in a high stress environment
- Promote physical, cognitive, and psychosocial well-being of each resident

Pay Range: $24.00 - $31.10 per hour

If you are looking for a position as a clinical dietitian, this could be your opportunity. Here at St. Peter's Health Partner's, we care for more people in more places. This position is located in person at 720 Hoosick Road Troy, NY.
The role of the clinical dietitian will include education, goal definition/implementation, and monitoring of patient outcomes (i.e. progress towards goals). They will also be responsible for medical record documentation accordance with defined policy and procedure, provision of patient, family and community education, consultation to medical and nursing staff on clinical nutrition and food service issues, and completion of required in-servicing on patient nutritional management. A successful dietitian will also appropriate practice of infection control measures i.e. response to isolation precautions, direction of food service staff as needed. Electronic Menu system utilization and management, participation in food and nutrition management activities such as test trays and meal rounds as requested.
We are looking for a committed clinical dietitian with good interpersonal skills with a genuine interest in helping individuals. Previous dietitian experience and NYS Dietitian/Nutritionist certification is preferred.
Minimum Requirements:
- Bachelor's Degree in Nutrition/Dietetics
- Completion of Dietetic Internship
- Registered by ADA*

ESSENTIAL FUNCTIONS:
- Performs complex diagnostic and therapeutic procedures in diagnosing emergency medical problems and prescribing necessary treatment and service for quality patient care; records data and tentative conclusions for review by physician.
- Coordinates and schedules laboratory studies and diagnostic procedures; explains the necessity, preparation, nature and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient.

Essential Functions

Leadership and Staff Support
- Provides daily work direction and guidance to Environmental Services team members.

- Cleaning and preparing discharge and transfer rooms and selected patient-care equipment.

- Assists in training staff on cleaning equipment, supplies, and procedures.

POSITION SUMMARY:
This incumbent provides routine patient care to promote patient comfort and contributes to the overall patient plan of care. This position is responsible for on-going patient data collection and documentation in the medical records. The position also performs a variety of support functions which contribute to the overall efficient operation of the nursing unit/department.
CORE FUNCTIONS
1. Assists with daily living activities and completes necessary tasks to promote patient health, including assisting patients with basic hygiene needs, maintaining an optimum level of mobility and providing assistance with nutritional intake as needed.
2. Collects patient specimens and data, including vital signs, input/output and other delegated measurements in accordance with policy and standards of care. Documents objective data and routine aspects of patient care. Assists in the transportation of patient specimens.
3. Recognizes and reports abnormalities and/or changes in the patient’s health status and responses to treatment to a licensed professional. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patients for which care is provided.
4. Performs procedures and uses equipment as delegated by a licensed professional, consistent with demonstrated competencies and according to state scope of practice. Procedures may include blood draws, foley catheter removal, IV removal, EKGs and Health Unit Coordinator duties. Provides necessary supplies, support and assistance to clinical staff and patients for procedures, under the direct supervision of a licensed professional.
5. Performs receptionist/clerical duties to support overall operating efficiency and effective flow of communication and information throughout the department. Participates in the resolution of customer problems or collects necessary data and follows through to appropriate person for problem resolution.
6. Maintains adequate levels of office, dietary, and medical supplies and completes requisition forms when supplies need to be ordered. Reconciles supply order upon delivery. Monitors and tracks the use and whereabouts of electronic equipment such as patient ID scanners and lap top computers. Ensures that all are accounted for and in working order.
7. Based on area of assignment may assist with transporting patients, including pre-operative and immediate post-operative patients to critical care unit. Participates in room turnover.
8. Supports a fast-paced environment with a strong focus on customer service. Interacts with staff at all levels, as well as patients, families, and physicians.
MINIMUM QUALIFICATIONS
High school diploma/GED or equivalent working knowledge.
Current BLS certification is required.
Must possess excellent organizational skills, as well as effective human relations and communication skills.
Computer literacy and keyboarding skills are required. In California: advanced skills are excluded.
BSS team members must have 12 months Banner experience in the role.

As a Banner Staffing Services -BSS (PRN) Behavioral Health Specialist you will focus on safety checks, assisting in ADL's, Milieu management, falls prevention measures, and 1 on 1 observations. Previous nursing assistant or BHS experience is preferred. BHS will be working on floors, completing tasks required and will be working on all three of our units. You will be dedicated to SAGE specifically.

You will have the opportunity to utilize our self-scheduling tool for shift and weekend coverage as well as enjoy an additional $1/hour weekend shift differential. No required holidays or weekends but available to fill our greatest needs. AVADE and BLS certified.
POSITION SUMMARY
This position provides direct care to psychiatric patients working under the direction of a registered nurse.
CORE FUNCTIONS
1. Provides basic nursing care to patients including technical monitoring, taking vital signs and physical care. Orients patients to treatment setting and assists with activities of daily living as needed. Reviews and participates in implementation of the patient treatment plan. Makes rounds and monitors patients activities, physical and mental status and reports to nursing staff. Checks patient belongings for contraband. Conducts a variety of patient safety monitoring activities such as observational checks. Documents observations, interventions, and group activities in the patient medical record while maintaining confidentiality of all health information.
2. Assists in maintaining a safe, orderly & therapeutic environment. Maintains patient safety and protects patient rights through observation and reporting patient behaviors, communications, interactions and physical status. Recognizes patients who are behaving aggressively, acting as a danger to themselves or others, or are in a crisis or medical emergency situation. Provides intervention including, but not limited to, redirecting of patient and/or participating in restraint or seclusion as needed.
3. Assesses and interacts therapeutically with patients and families. Provides treatment that promotes patient dignity, independence, individuality, strengths, privacy, and choice. Demonstrates respect for cultural diversity and patient boundaries. Plans and facilitates social and didactic group activities. May participate in other rehabilitation and treatment programs.
4. Provides quality patient care by maintaining knowledge and skill level related to the symptoms and treatment of behavioral disorders relevant to position responsibilities and in accordance with state agency regulations. Maintains knowledge to identify types of medications commonly prescribed for patient group and the common side effects and adverse reactions of the medications.
5. Assists in the admission, discharge and transfer process of patients and completes required documentation in the patient medical record.
6. Position completes work under moderate supervision. Position interacts with all levels of clinical staff on assigned unit/program and members of the multidisciplinary teams, as well as patients, families, and medical and psychiatric physicians.
MINIMUM QUALIFICATIONS
Requires a Bachelor’s Degree in a behavioral health related field OR High School diploma or equivalent with 1 year of direct patient care in an inpatient behavioral health setting.
This should include but is not limited to experience in an inpatient psychiatric facility; assisting with personal grooming and other activities of daily living; conducting educational, recreational and skills groups appropriate to current patient population; creating, maintaining, and encouraging an inpatient therapeutic milieu; maintaining observation precautions and applying non-violent de-escalation interventions in crisis situations; recognizing the common symptoms of mental illness such as depression, psychosis, mood dysregulation, substance abuse, maladaptive behavior, and anxiety; understanding how symptoms are related the patient’s mental illness diagnosis and providing ordered treatment interventions appropriate to the diagnosis. Such experience is commonly achieved in care of patients in psychiatric acute care hospitals, inpatient psychiatric facilities and State hospital programs. Some outpatient programs, therapeutic group home and residential programs may meet the experience criteria. Concentration on skills and experiences demonstrating competence in building therapeutic rapport, developing collaborative relationships with the multidisciplinary team, de-escalation techniques and limit setting.

This position coordinates a smooth patient flow process by answering phones, scheduling patient appointments, providing registration of patient and insurance information, obtaining required signatures following established processes, procedures and standards. This position also verifies insurance coverage, validates referrals and authorizations, collects patient liability and provides financial guidance to patients to maximize medical services reimbursement efforts. This also includes accurately posting patients at the point of service and releasing information in accordance with organizational and compliance policies and guidelines.
CORE FUNCTIONS
1. Performs registration/check-in processes, including but not limited to performing data entry activities, providing patients with appropriate information and intake forms, obtaining necessary signatures and generating population health summary.
2. Verifies insurance eligibility benefits for services rendered with the payors and documents appropriately. Assists in obtaining or validating pre-certification, referrals, and authorizations

4. Enters payments/charges for services rendered and performs daily payment/charge reconciliation in a timely and accurate manner. Balances cash drawer at the beginning and end of the day and prepares daily bank deposit with necessary paperwork sent to centralized billing for record purposes.
5. Schedules office visits and procedures within the medical practice(s) and external practices as necessary. Maximizes reimbursement by scheduling patients in accordance with payor plan provisions. Confirms patient appointments for the following day as necessary and ensures patients are properly prepared for visits.
6. Demonstrates proactive interpersonal communications skills while dealing with patient concerns through telephone calls, emails and in-person conversations. Optimizes patient flow by using effective customer service/communication skills by communicating to internal and external customers, care team, management, centralized services and HIMS.
7. Assists in responding to requests for patient medical records according to company policies and procedures, and state and federal laws.
8. Provides a variety of patient services to assist in patient flow including but not limited to escorting patients, taking vitals and patient history, assisting in patient treatment, distributing mail and fax information, ordering supplies, etc.
9. Works independently under regular supervision and follows structured work routines. Works in a fast paced, multi-task environment with high volume and immediacy needs requiring independent decision making and sound judgment to prioritize work and ensure appropriateness and timeliness of each patient’s care. This position requires the ability to retain large amounts of changing payor information/knowledge crucial to attaining reimbursement for the services provided. Primary external customers include patients and their families, physician office staff and third party payors.
MINIMUM QUALIFICATIONS
High school diploma/GED or equivalent working knowledge.
Requires knowledge of patient financial services, financial, collecting services or insurance industry experience processes normally acquired over one or more years of work experience. Requires the ability to manage multiple tasks simultaneously with minimal supervision and to work independently.
Requires strong interpersonal, oral, and written communication skills to effectively interact with a wide range of audiences. Strong knowledge in the use of common office software, word processing, spreadsheet, and database software are required.

CORE FUNCTIONS
1. 4. Fulfills requests for images and reports ensuring availability in system, hard copy (film), and/or CD as required and follows proper protocols for releasing confidential patient information.
5. Receives patient in applicable systems, confirms patient demographics and insurance information and performs insurance verification when required. May be required to calculate and collect copays, deductibles, and self-pay. Totals charges on patient’s super-bill at check-out. Enters pending discharges into the system. Coordinates patient appointments for procedures and treatments that may be required prior to timely discharge. Processes patient discharges. Coordinates transportation to care centers and residential homes as requested by patients or physicians.
6. Prints physician schedules, super-bills, labels and other applicable forms in preparation for next business day. Pulls and files patient charts and prepares new patient charts as needed. Prints and files transcription notes. Reconciles charge tickets to each day's schedule, identifies incomplete tickets, missing charge codes or missing diagnosis and notifies clinical staff. Forwards completed tickets and/or billing package to the business office or billing company.
7. Assists technologists and nurses in pre and post procedure room preparation by obtaining necessary supplies and assisting patients as necessary. Ensures procedure rooms are stocked appropriately. Understand sterile procedure and assists with setting up sterile trays, where applicable. Adheres to radiation safety policies. Escorts patients back to the patient exam rooms, takes vitals and patient history; may assist with patient treatment to include peripheral IV insertion, IV discontinuation and Point of Care Testing (POCT) if required by the assigned facility (completion of Banner’s peripheral IV insertion course and validation of skill must be completed prior to any peripheral IV insertion and discontinuation performed. 9. This position assists in the operations of the medical imaging department under direct supervision with technologists and nurses in department. Incumbents work in a fast-paced, sometimes stressful environment with a strong focus on customer service. Must navigate a variety of separate computer programs at any given time and handle multiple demands on time. Internal customers: All levels of medical imaging and nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External Customers: regulatory and health agencies, non-clinic providers and facilities, vendors, patients and family members.

The Community Hospice has a part time opportunity for a Bereavement Service Facilitator to join our hospice care team caring for patients in the Capital Region area as part of our Interdisciplinary Team caring for patients and families receiving hospice care. As a member of the hospice interdisciplinary team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff.
What you will do:
The Bereavement Services Facilitator will assist the bereavement team in the operation of the bereavement program by: Completing intakes with new clients and providing referrals when appropriate. Making follow-up calls to assess bereavement needs of primary care person and families; co-facilitating in-services and support groups; organizing regular bereavement mailings; organizing regular memorial services. Supports bereavement program by attending team meetings, attending site and discipline specific staff meetings.
Responsibilities:
- Completes initial intakes with new clients and provides referrals when appropriate.
- Coordinates school-based grief support with local school districts to organize individual and group programming as requested.
- Makes follow-up calls to assess bereavement needs of primary care persons and families.
- Coordinates and participates in in-services, support groups, and on-site family groups
- Provides individual and group grief support to families, agencies, Hospice staff, volunteers, interns and medical students.
- Provides community education activities and include goals, objectives and evaluation of educational activities.
- Honors family and caregiver’s choices regarding bereavement contact or services.
- Organizes and facilitates memorial services on-site and in the community
- Assists with coordination and participation in crisis responses.

- Participates in collection and tabulation of bereavement program statistics, utilizing queries to invite specific populations to groups and services.

- Maintains and updates electronic bereavement files. Reviews patient charts to ensure all data is complete and accurate.
- Participates in regional and agency-wide QI initiatives related to the Bereavement process/program.
- Participates in orienting staff, volunteers, interns and medical students to bereavement services.
- Attends Hospice team and staff meetings.
- Answers and screens phone calls on a daily basis.
- Schedules in-services and bereavement groups/meetings as needed.
- Maintains confidentiality of patient information except when patient has given permission for disclosure. Maintains confidentiality of personnel information.
- Meets individualized productivity expectation. Promotes and utilizes performance improvement process in daily work routine and contributes positively to the performance improvement process (including participation on committees) within the Agency and St. Peter’s Health Partners.
- Participates in staff development programs, in-services, continuing education opportunities and team meetings to keep current and proficient in job responsibilities. Responsible to attend mandatory in-services and to understand your role in the organization’s corporate compliance plan.
- Maintains professional knowledge and skill level appropriate to services being provided and up-to-date knowledge of Agency policies and procedures. Applies this knowledge in everyday practice.

- Associate Degree in Human Services field is required.
- Bachelor's Degree in Human Services field is preferred.
- 2 Years experience with clients in a human service capacity.
