What you will do:
Performs linen handling functions including cart preparation, distribution of clean linen to user departments, pack making and soiled linen pick-up.
Responsibilities:
- This Candidate will be responsible for folding linen.
- Filling linen carts to pre-determined par levels and delivering linen carts to various departments throughout the hospital.
- Restocking linen shelves, collect soiled linen from different departments, folding scrubs, loading the scrub machine, building Operating Room packs and keeping the linen department clean and organized.
What you will need:
- Must be able to respond appropriately to written and verbal instructions.
- Computer knowledge desired.
Pay Range:
$16.20 - $20.75

POSITION SUMMARY:
Colleague is responsible for the overall patient safety and clinical risk management functions related to review and response to reported events. Responds to crisis situations that have patient safety and risk management implications and assists staff with problem solving.
EDUCATION, TRAINING, EXPERIENCE, CERTIFICATION AND LICENSURE:
Experience in health care setting of 3-5 years is required.
Bachelor’s degree required (nursing, healthcare related degree is strongly preferred).
Master’s degree preferred.
Registered Nurse preferred.
Ideally, the candidate will have 3-5 years in risk management / patient safety experience.
Certification (CPHRM) is preferred.

OPERATIONS/COMPLIANCE:
Is responsible for data management, investigation & reporting events. Supports the education components of the facility’s risk management program. Promotes organizational patient safety initiatives.
LOSS PREVENTION/PATIENT SAFETY:
Navigates facility-wide systems for risk identification, investigation, and reduction. Organizes and manages facility-wide educational programs on health care risk management and related subjects for health care practitioners. Presents such programs in conjunction with the facility’s education department or other organizations. Maintains a network of informational sources and experts; performs risk surveys and inspects patient care areas; reviews facility to assess loss potential. Works with leadership to develop risk mitigation plans. Ensures that patient care-related incidents are reported to Trinity System Office, CMS, and/or the Department of Health as directed or required by law.
SPECIFIC ACTIVITIES:
- Conducts case finding by daily review of reported events and other information retrieved from other sources (i.e., verbal report, phone report, electronic submissions from MIDAS, and other referrals) and initiates appropriate follow-up.
- Communicates with regulatory agencies, as needed, including, but not limited to: the NYS Department of Health (DOH), FDA, and others.
- Coordinates overall functions including but not limited to: NYPORTS, STARS/ClearSight, Centers for Medicare and Medicaid Services (CMS) death reporting in restraints; ensuring that reporting criteria are met.
- Demonstrates a strong ability to identify, analyze and solve problems.
- Uses appropriate tools when conducting root cause analysis, failure mode and effect analysis, gap analysis, and other risk assessments.
- Provides educational programs on Clinical Risk Management topics as requested.
- Promotes an environment of learning and safety.
- Is readily available to all staff as a resource.
- Competent with data display and analysis.

What you will do:
The Senior Registered Professional Nurse has the responsibility and accountability to utilize the nursing process to diagnose and treat human responses to actual or potential problems of individuals or groups. - Brings patient to exam room, takes appropriate vital signs and documents in electronic medical record.
- Review and update medication list to ensure accurate and complete list in electronic medical record (EMR) available for provider review and submission.
- Complete referrals and tracks patients' compliance.
- Review prescriptions electronically and send prescriptions to providers for review and submission.
- Obtains patient consent for procedures as directed by provider.
- Performs pre-visit planning and reviews quality metrics.
- Retrieves telephonic clinical information from patients who call into the office.
- Monitors task list and completes tasks assigned by provider in a timely manner based on urgency.
- Educates patients regarding medication, testing procedures and home care techniques.
- Ensure proper labeling, handling and documentation for patient specimens.
- Follow up with patient regarding test results based on advice given by provider.

Eddy Visiting Nurse & Rehab Association has a rewarding opportunity for a full-time RN to join our home care team in Columbia/Greene County. This is a dynamic position working directly with an interdisciplinary team in the homecare setting. Must have 1 year prior experience in acute care, rehab, or homecare. Candidates must be organized, able to teach, and be willing to travel from patient to patient.
This RN role must have strong communication with other interdisciplinary team members and family members to be successful. Must have strong critical thinking skills. Excellent orientation provided with support from supervisory team!

Requirements:
- 1 years RN acute care experience required;

Pay Range: $40.00 - $52.81 per hour
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

GENERAL SUMMARY
Registered Respiratory Therapist who is responsible for performing various therapeutic procedures including advanced practice procedures such as, but not restricted to operating balloon pumps, working in NICU, performing Pulmonary Function testing, “PFT”, etc.
Essential duties and Responsibilities
Behaves in accordance with the Mission, Vision and guiding behaviors of Trinity Health.
Provides professional leadership in the delivery of patient care.
Uses "AIDET" principles when interfacing/treating every patient throughout the organization. (Acknowledge, Introduce, Duration, Explanation, Thank You).
Where appropriate, with each patient interaction, uses the concept of "Managing Up" – a form of communication that positions information, a person, or team in a positive manner.
Provides a consistent experience to patients, families, visitors and internal customers by using "Key Words at Key Times." Specifically, uses key words to help patients to understand what is happening with their care, help internal customers understand their service better, and build relationships.
Serves as a consultant, resource person, and role model representing respiratory care in the health system committees or work groups as needed.
Maintains the confidentiality of information according to HIPPA guidelines.
Facilitates good rapport and cooperative relationships approaching conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution.
Maintains safe respiratory therapy practices; alerts management of any safety issues or concerns.
Performs all duties and responsibilities of a Respiratory Therapist per AARC guidelines obtains and maintains competencies per RT II orientation. Some examples below:
Cardiopulmonary arrest situations Other emergency situations Participates in patient rounds Ensure treatments are provided in accordance with physician orders and approved medical protocols. Ensures that equipment is in safe operating condition and contacts repair personnel or Charge therapists as appropriate Based on clinical assessment of patient situations, suggests changes in therapies, etc., to medical staff to benefit cardiopulmonary patient care. Demonstrates knowledge of procedure(s) for oxygen and medical air shutdown. Maintains knowledge of and reviews pertinent hospital and all departmental policies and procedures. Prioritizes and addresses conflicting requests for respiratory care services.
Provides high quality patient care considering age specific, developmental and cultural needs through competent practice and application of the respiratory care practice process.
Demonstrates annual unit/area competencies
Participates in new educational programs and equipment/ procedure implementations.
Facilitates instruction of staff or students during clinical training, providing feedback when applicable.
Other duties as assigned.
Supplemental Duties and Responsibilities
Completes Health Stream yearly on a timely basis per Trinity Health standards
Responsible for own education for professional growth and maintains log of activities.
EDUCATION, EXPERIENCE AND LICENSURE
Education:
Graduate of a two year advanced practitioner -Accredited Respiratory Therapist program: Registry eligible.
Maintain current RRT credential and must hold valid BLS and ACLS certifications.

Position Summary: The Resident Assistant, or RA, is a caregiver providing assistance to residents under the supervision of the LPN and/or RN. Resident Assistants provide compassionate care that is respectful of each resident's needs, values and wishes.

Principal Responsibilities:

Clinical Competencies:
Resident Assistants provide the following nursing services under the supervision and direction of an LPN or RN across the continuum of care at SPHP.
- Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting and mobility in a respectful, compassionate manner.
- Assist residents in taking medications as needed.
- Encourage and support resident independence and dignity.
- Monitor resident safety and well- being, reporting any change to the LPN. Consult with the LPN/RN on any changes of health statuses of residents.
- Build meaningful relationships with residents, understanding their personal histories, preferences and routines and anticipate needs of residents.
- Help with light housekeeping and meal service as needed to maintain a safe and clean living space.
- Participate in team meetings and communicate/collaborate with coworkers and supervisors.
- Promote a warm, friendly, and respectful environment.
Education Requirements:
- Must possess patience, empathy and a genuine desire to help others
- High school diploma or GED preferred.
- Previous experience in senior care, health care or assisted living preferred, but not required
Must be able to:
- Ability to follow care plan and safety protocols
- Working knowledge of computers including electronic health record, checking email, and preforming required in-servicing, along with use of other required applications
- Have the physical, visual and auditory ability adequate to perform resident care
- Maintain composure and positivity in a high stress environment
- Promote physical, cognitive, and psychosocial well-being of each resident
- Work with residents in a highly personal manner during stages of acute and chronic illness
Pay Range: $18.10 - $22.15 per hour
