POSITION PURPOSE
The Utilization Review (UR) Coordinator I colleague is responsible for conducting the utilization review process in accordance with federal and state law, licensure/ accreditation and hospital standards and Third-Party payor requirements. The review process includes but is not limited to admission/re-admission review (includes identification of fragmentation of care, combined admissions), pre-certification/re-certification review, data gathering for identified projects, internal audits, retrospective reviews, statistics, coding, billing or verification issues.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES MAY INCLUDE
1. Provides leadership and expertise for utilization management processes.
2. Completes the UR review and obtains authorization on retrospective cases that the insurance has changed after discharge by performing the following:
o Reviews assigned medical records in a timely manner for admission, concurrent or retrospective review using criteria to determine appropriate level of care.
o Exhibits consistent documentation of criteria in the EMR
o Follows the hospital Utilization Review Plan to ensure effective and efficient use of hospital services.
o Demonstrates competency with InterQual application with a passing score on competency test, Inter Rater Reliability when administered. Demonstrates competency using MCG criteria.
o Analyzes and disseminates appropriate clinical information for payer authorization. Extracts pertinent information and communicates in a succinct fashion to the 3rd party payer. Demonstrates sound clinical judgement that supports IP level of care for appropriate reimbursement to the hospital. The UR Coordinator must be knowledgeable of health plan contracts and meet their clinical submission deadlines.
o Demonstrates expertise in providing IS/SI criteria to Third Party Payers to obtain authorization for level of care and/or procedures for initial admission, concurrent or retrospective reimbursement.
o Contacts the internal physician advisor on cases that do not meet established guidelines for admission or continued stay
o In the event of an inpatient denial by the payer, obtains further documentation from the physician to support an IP level of care.

4. Provides leadership and expertise for utilization management processes.
5. Manages the DNFB (discharged but not final billed) list and other UR hold reports to ensure
o Account problems are being resolved and clean claims released timely
o Authorization is secured timely
o Claims can be released for hospital reimbursement
6. Develops and shares reports as requested.
7. Initiates and coordinates Peer-to-Peer discussions with physician and payer as warranted.
8. Obtains authorizations as required for reimbursement from appropriate Third-Party Payor.
9. Mediates and coordinates process between insurance company and hospital clinical staff when a patient requires transfer to another acute care hospital.
10. Processes all technical and administrative denials for all clinical areas of the hospital and facilitates the appeal process as appropriate.
11. Communicates to the payer patient discharge date and discharge plan.
12. Mediates between case manager and payer to ensure a seamless transition in care and appropriate post-acute follow-up.
13. Provides updated third-party payor information to assigned Case Manager for continuity of care.
14. Identifies areas of quality concerns, inappropriate use of resources and any other issues that restrict the implementation of hospital, department objectives and refers findings for appropriate follow-up.
15. Reviews IP denials with the appeals nurse to determine appropriate status and reimbursement from payer. Appeal determinations are made at that time.
16. Assists other departments in the reimbursement process including, but not limited to, changes in inpatient, outpatient and observation status, identification of appropriate surgical status and other interventions needed to reduce patient and hospital liability of financial loss.
17. Demonstrates proficiency in using various computer programs required including EPIC and various payer portals, etc.
18. Responsible for combining admissions on those cases that are appropriate and communicating to Insurance Verification and Health Information Management.
19. Assists with payer-specific audits as assigned, i.e. RAC, HDI
20. Functions as a resource to physician, hospital staff or departments and other ‘customers” of the hospital to assist in complying with the utilization review processes.
21. Participates in UR Committee, department staff meetings and ad hoc committees on an as needed basis.
22. Participates in review and analysis of outcome data related to UR functions and identification of system and process issues that contribute to denials.
23. Discusses patient and hospital information only among appropriate personnel in appropriate private places.
24. Maintains good rapport and cooperative relationships, and approaches conflict in a constructive manner.
25. Helps to identify problems, offer solutions, and participate in their resolution.
26. Maintains the confidentiality of information acquired pertaining to patients, physicians, associates, and visitors to Trinity Health.
27. Behaves in accordance with the Mission, Vision and Values of Trinity Health.
28. Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.
OTHER FUNCTIONS AND RESPONSIBILITIES

REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE
Education: Associate’s degree in health-related field or nursing preferred.
Experience: Three years of recent acute care utilization management experience.
Licensure/Certification: Current RHIT or LPN preferred
REQUIRED SKILLS AND ABILITIES
1. Understanding of computers and software in order to collect and report information for required data sources.
2. Ability to work autonomously with little direction and be accountable for outcomes.
3. Excellent customer service orientation skills necessary to deal effectively with various levels of hospital personnel, outside customers and community groups.
1. Assists with the development of workflow, policies, and procedures to optimize ECT services, ensuring efficiency and effective service delivery.
2. Develop training for staff to provide high-quality service delivery between internal and external resources.
3. Works closely with both internal programs/services and external organizations to coordinate patient referrals, ensuring seamless integration of services.
4. Coordinates all pre-procedure requirements and obtains documentation to ensure clearance of procedure.
5. Manages the ECT scheduling and throughput of services including minimizing unfilled time.
6. Coordinates services with other departments to ensure safe flow of service delivery.
7. Coordinates prior authorization, benefits verification, and ensures reduction of denials.
8. Partner with billing to review coding/claims issues.
9. Assures complete, accurate, and timely documentation of clinical information in the EHR, incorporating current documentation and billing guidelines and requirements.
10. Onboarding new colleagues to ECT workflows; provide training and education to current colleagues as needed.
11. Track and report out all Key Performance Indicators (KPI) timely including time-to-start, cancelations/no-show rates, adverse events, Length of Stay (LOS), recidivism, and utilization.
12. Prepare for surveys/audits, as needed.

Qualifications:

POSITION PURPOSE
Performs phlebotomy on both outpatient and inpatients. Performs specimen processing, computer order entry and other pre-analytic functions, as required. Obtains accurate patient demographic and financial information from patients or patient advocate. May registers outpatients in the hospital electronic medical record system (EPIC) and determines preliminary patient or insurance payment obligation.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
- Performs successful sample collection on outpatient and inpatients to include venipuncture blood draw, capillary puncture, infant heel sticks, and urine collection utilizing proper patient identification and customer service skills. Troubleshoots problems related to missing specimens, discrepancies between specimens and orders.
- Obtains patient demographic and insurance information from a patient or patient advocate. Performs data entry through the electronic medical record system which may include insurance verification, issuing Advance Beneficiary Notice (ABN) and utilize automated systems to obtain and process insurance information.
- Acts as a representative of the department, communicating with patients and physician offices. Responds to inquiries in a courteous and helpful manner following departmental policies and procedures.
- Uses good judgment to decide when to contact a Medical Laboratory Scientist, Manager, or a Pathologist, as necessary.
- Knowledge of reference sources for patient preparation and specimen collection and handling requirements to be able to assist other health care professionals. Able to access on-line websites for assistance, when needed.
- May utilize a glucometer or perform waived urine testing prior to administration of glucola.

- Completes specimen handling and processing including centrifugation of vacutainer and aliquoting/pouring over of specimens. Aliquots and adjusts pH of 24-hour urine specimens as required by location.

Position Purpose:
Performs computerized tomography procedures for the evaluation of pathological entities as prescribed by a physician. Assumes responsibility and maintains the general care and safety of patients during diagnostic procedures.
What you will need as a CT Technologist:
Must be a graduate of a formal program in Radiology Technology in an JRCERT approved school.
Must be registered and in good standings with American Registry of Radiologic Technologist.
Current Advanced certification in Computerized Tomography or must complete Advance registry within one year of the hire date.
ARRT and CT certification
Basic Cardiac Life Support (BCLS)
ARRT certification with two years as a registered technologist.

What you will do as a CT Technologist:
Completes all necessary work sheets, patient records and documentation. Processes scans and related functions as needed.
Assists Radiologist in performing complex sterile diagnostic procedures such as biopies.
Administers intravenous contrast media to include utilization of power injectors. Monitors patient for possible reactions as appropriate to the study and provides immediate care to patient if a reaction should occur.
Perform a wide range of computerized tomography procedures. Operation of CT equipment, power injectors, PACS, RIS system and processing of films.
Assist physician in performing procedures in the procedure room.
Maintain professional and ethical relationships with all customers, as necessary to preserve optimal quality of care.
Provide on call coverage as needed in the Computerized Tomography area.
Perform general nursing duties consistent with meeting the needs of the patient if needed.
Perform clerical duties related to patient chart documentation, radiology files, billing and procession of images into PACS and conventional filming.
Maintains a working knowledge of applicable Federal, State, and Local laws and regulations, the Trinity Health Organizational Integrity Program, including the Standards of Conduct, Code of Ethics, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behaviors.

Position Summary:

- Selects the correct CT Scan factors (mA, kVp, slice width, helical vs. non-helical, etc.) to produce high quality images; adapts technical factors for unusual variations of the patient or other factors (pathology, etc.).
- Provides the appropriate radiation shielding and collimation to adhere to ALARA (as low as reasonably achievable) principles. Provides for the radiation safety of all persons in the area where radiation is employed.
- Performs office/clerical duties in the absence of clerical support staff.
- Places orders and charges in the Hospital Information computer as necessary.
- Gives pertinent preparatory instructions for the test.
- Obtains the patient’s name, date of birth, phone number and MPI number if known.
- Obtains pertinent clinical history that supports the procedure being ordered and consults the ordering physician or nurse when the information is inadequate.
- Obtains any required Laboratory values prior to the procedure.
- Secures all necessary supplies to maintain a prudent inventory level for patient care.

- Graduate of an approved school of Radiographic Technology

REQUIRED EDUCATION / LICENSURE

· Minimum of one year of previous direct care experience (acute or long term) or successful completion of CENA, PCT, MA, or EMT course.
· Student nurses must have completed their nursing fundamentals class of a BSN program.
What you will do:
- Training is provided to prepare our colleagues for success.
- The PCT assists patient in completing ADLs including: Bathing, showering, toileting, positioning, turning, transferring, ambulation using assistive devices and with feeding and menu selection.
- A PCT will collects and send specimens including blood and blood cultures and inserts and removes peripheral IV catheters. They are responsible to record patient information and required data in appropriate areas in order to meet documentation requirements.
- 1 East is a locked 36 bed, secured unit and services patients 18 through older adult.
- Services criteria includes the presence of life threatening and/or severely incapacitating symptoms and Potential of harm to self or others is considered serious.
· Patients are admitted Through Psychiatric Access department in the Emergency Department at THAA. In most cases face to-face assessments will be required and in other cases transfers or direct admissions from other locations may occur.

Our Labor and Delivery department is a 24-hour inpatient unit consisting of 12 Labor-Delivery-Recovery suites, 4 Perinatal suites, 6 Antepartum suites, 3 Operating suites, a 4 bed recovery room and a 7 station triage area. Labor and Delivery provides Inpatient and outpatient low and high-risk perinatal care, prenatal testing and treatment, antepartum, intrapartum and postpartum care, transitional and normal newborn care and pregnancy loss services.
SUMMARY:
Under the direction and supervision of the Nurse Manager provides support for patients, visitors and colleagues on assigned nursing units. Facilitates efficient patient throughput including admissions, discharges and transfers. Supports patient experience and culture of safety goals through equipment maintenance, unit supplies, unit appearance and overall environment of care. Provides reception type services to patients, families, visitors and other unit guests.
EDUCATION AND EXPERIENCE:

GENERAL SUMMARY
Provides clinical support to physicians as it relates to patient care. Maintains examination rooms and drug sample cabinets by ensuring they are clean, well stocked, and up to date. Performs some telephone triage based on patient symptoms, and provides routine information to patients regarding their condition, diagnostic results (non-abnormal), and medications. Completes related paperwork. Provides back-up support to receptionists, office assistants, and cashiers.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Prepares patients for physician by taking vital signs (blood pressure and weight, and temperature when symptoms warrant), obtaining chief complaints and filling out prescription refills. Locates and enters missing information in chart at physician request. Ensures that patients are in rooms and ready to be seen by physicians on schedule. Communicates results of patient preparation to physician (i.e., information requested but not shared by patient, etc.).
2. Assists physicians in exams and procedures as requested. Medical assistant presence is required for all gynecological exams and sigmoidoscopies.
3. Performs tests and procedures, including, but not limited to: EKG's, pulmonary function testing, vision testing, strep tests, urinalysis, urine straight catheterization, suture removals, injections, holter monitors, ear irrigations, and stool for occult blood cards. Reminds physician to include procedures on route slips.
4. Arranges patient admissions, including calling Bed Management for a bed, transporting patients to the hospital, ensuring physician progress notes accompany patient, and precertification occurs when necessary.
5. Assists patients with answers to questions or other requests.
6. Assists in scheduling tests, return visits, and filling out requisition forms as necessary (patients may schedule own tests whenever possible).
7. Ensures exam rooms, procedure rooms, lab area, drug sample cabinets, specialty trays and equipment are in clean condition, stocked and in good working order. Notifies Clinical Office Coordinator or designee of supply shortages and equipment malfunction.
8. Sorts and distributes information from physician "out" boxes. At physician request, make routine (non-abnormal) information calls to patients regarding their condition, diagnostic test results, and medication. Triages incoming calls based on patient symptoms, and contacts physicians with time-sensitive or urgent information. Calls other departments for patient information as needed or requested; calls in prescriptions.
9. Documents phone contacts, medications and other relevant information in patient chart.
10. Completes HMO referrals at physician request.
11. Works as part of a medical team, and provides support to other medical assistants, receptionists, cashiers and office assistants voluntarily or as requested when time permits.
12. Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution.
13. Maintains the confidentiality of information acquired pertaining to patient, physicians, employees, and visitors to St. Joseph Mercy Hospital. Discusses patient and hospital information only among appropriate personnel in appropriately private places.
14. Behaves in accordance with the Mission, Vision and Values of St. Joseph Mercy Health System.
15. Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.
