Performs duties described in the United States Department of Transportation/National Highway Traffic Safety Administration position description for EMT Paramedic including; responding to requests from the public for emergency medical assistance; examining, assessing and stabilizing patients at the point of call and during transport to the appropriate point of entry.I. Major Responsibilities:
1. Administers skilled medical care to individuals in response to calls for emergency treatment in accordance with the Massachusetts EMS Pre-Hospital Treatment protocols. Examines, assesses, and stabilizes patients at emergency scene, i.e., advanced cardiac life support, hemorrhage control, treatment for shock, immobilization of fractures, bandaging, proper treatment for burns, poisoning, or drug overdose, manages gynecological emergencies, delivers newborn, manages mentally disturbed patient, etc.
2. Obtains medical history. Checks vital signs. Advises/informs on-call physician of diagnostic indication. Performs definitive life support. Administers treatment and utilizes advanced life support equipment and procedures as directed by authorized physician, i.e., performs endotracheal intubation, defibrillation, synchronized cardioversion, nasogastric intubation, closed chest intraosseus fluids, etc. May perform external trans thoracic cardiac pacing, medication enhanced intubation including surgical cricothyroidotomy, and other advanced scope of practice procedures.
3. Responds to a wide range of emergency situations as required.
4. Provides control at the scene of medical emergency. Conducts triage, treatment and transportation at mass casualty incidents including incidents involving terrorism and weapons of mass destruction.
5. Documents pre-hospital care received by patients in order to facilitate admission to health care facility and for database purposes.
6. Prepares patients for transport. Consults with radio dispatcher as to appropriate available medical facility in order to ensure immediate treatment of patient upon arrival. Monitors patient in ambulance to maintain stability. Administers additional treatment when necessary.
7. Drives ambulance to and from scene of emergency. Inspects, clears, stocks and may make repairs (in emergency) to Emergency Medical Services vehicle and equipment to ensure and provide continuous availability of appropriately decontaminated units.
8. Maintains control of narcotics Class B and C. Maintain up-to-date records of inventory of both narcotic and non-narcotic medications stored in vehicle and supply source at hospital. Ensures emergency vehicle is properly stocked with necessary supplies, equipment and medications.
9. Reports suspected abuse following standard procedure.

II. Position Qualifications:
License/Certification/Education:
Required:
1. Certification as an Emergency Medical Technician - Paramedic by the Massachusetts Department of Public Health.
2. Certified in CPR - Basic Rescue by either the American Red Cross or the American Heart Association.
3. Certified in Advanced Cardiac Life Support by the American Heart Association.
4. Valid driver license.
Experience/Skills:
Required:
1. Experienced in accident scene management and control.
2. Demonstrated ability to work well under often stressful situations.
3. Strong organizational and interpersonal skills.
4. Effective oral and written communication skills.
Preferred:
1. Minimum three year’s experience in an urban EMS service integrated with a municipal first response system.
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.
III. Physical Demands and Environmental Conditions:
Work is considered heavy. Position requires work outdoors in a patient care environment.
Additional Required and Preferred Education and Experience:
ADDENDUM FOR: Resuscitation and Airway Management Specialist (RAMS)
Position Summary:
Worcester EMS paramedics who have completed the clinical competencies for the DAI/RSI program will serve in the Pavilion (PAV) in capacities consistent with their level or training and scope of practice. All work performed in the hospital will be done under the clinical supervision of the Medical Directors and the attending physician.
Major Responsibilities:
• Establishing and maintaining peripheral IV access
• Administering medications and fluids within hospital defined scope
• Participating in “Overwatch” activities which involve meeting with other clinical staff to identify patients at elevated risk of clinical decompensation and monitoring these patients for any acute changes throughout the course of a shift. Any worsening or significant change to be communicated to the care team.
• Working in a collaborative fashion as part of the care team with physicians, APPs, PCAs, Unit Secretaries, Respiratory Therapists, and RN staff.
• Assist with procedures and patient care activities at the request of the clinical team.
• Assisting with transport of critical patients to and from areas of the hospital while monitoring for any acute changes.
• When summoned in case of critical decompensation or cardiac arrest, at the direction of the physician or APP, will be responsible for management of the patient’s airway according to the Resuscitation and Airway Management Specialist (RAMS) training and scope. The airway procedures will include:
o Administration of medications for induction prior to paralysis to include Ketamine and Etomidate as well as to suggest adjunct medications such as fentanyl if indicated.
o Administration of neuromuscular blockade using either succinylcholine or rocuronium.
o Administration if needed of push dose vasopressors in the case of hemodynamic instability.
o Use of LMAs and other supraglottic airways.
o BVM ventilation.
o Use of PEEP valve during ventilation.
o Direct laryngoscopy.
o Video laryngoscopy.
o Orotracheal and nasotracheal intubation.
o Use of bougie to assist in intubation.
o Cricothyrotmy in Cannot intubate, cannot ventilate (CICO) situations.
o Placement of orogastric or nasograstric tube.
License/Certification/Education:

1. Worcester EMS paramedic who has completed the clinical competencies and remain current for the DAI/RSI program.

Responsible for the planning, delivery and management of direct patient care utilizing the nursing process and adhering to the standards of nursing practice.I. Major Responsibilities:

• Provide direct patient care and daily living support

b. Perform medication reconciliation.
c. Review procedural sedation history.
d. Provide pre procedural instructions, providing the patient with opportunity to ask clinical questions without requiring separate call back.
3. Create patient specific premedication regiment in collaboration with procedure APP.
4. Obtain IV access and preprocedural lab work.
5. Administered preprocedural procedural medications, titrating according to specific patient needs. Also administer chemotherapy and performs procedures safely and efficiently in accordance with Institute policy. May administer blood transfusions.
6. Work as co-sign to Intrathecal Chemotherapy administered by APP.
7. Assists in emergency nursing procedures necessary for prompt control of changes in patient`s physical condition.

Primary Duties and Responsibilities
- Performs pre-analytical specimen processing: Receives patient specimens from within the institute and outside institutions.
- Becomes familiar with patient and specimen requirements; stores and transports specimens according to procedures; ensure specimen integrity is preserved at all times.
- Unloads and loads the pneumatic tube system and accurately distributes specimens; does routine and stat specimen delivery runs to BWH laboratories; does specimen pickup runs from clinic locations at DFCI
- Receives specimens into the Laboratory Information System (LIS) by scanning barcodes or manual entry
- Verifies upon receipt that all samples are labeled according to policy and addresses variances according to policies.
- Documents in the LIS any specimen or test cancellations per policy; contacts the appropriate provider or reference laboratories with questions regarding discrepancies with test orders or specimen containers.
- Processes add on tests according to policy
- Processes tests sent out to reference laboratories; communicates with reference labs as needed
- Processes specimens by loading the pre-analytic automation system or via centrifugation, aliquoting, storing, and shipping per test requirements.
- Performs various quality checks to ensure all requests are accurate before dispensing the specimens to the appropriate performing lab.
- Responds to telephone inquiries to assist providers, nurses and other caregivers with lab test, test requirement and result inquiries.
- Performs all duties with accuracy and efficiency to ensure patient care is not compromised.
- Utilizes electronic system for faxed tests results from reference laboratories.
- Reviews pending lists as required; addresses and corrects discrepancies or pending test results by contacting the reference lab and the provider as necessary.
- Processes bone marrows orders and specimens.

Knowledge, Skills and Abilities
- Develops knowledge to accurately and efficiently ensure all test requests are in the LIS.
- Understands lab safety policy and procedures and Department of Transportation specimen shipping regulations.

Primary Duties and Responsibilities:
1. Participates in day-to-day operations of the program including patient education and call triage

A. Assumes nursing care responsibilities for a defined group of patients within the program, including but not limited to assessment, education, coordination, and documentation.
1. Facilitates individualized care within the context of functional status, cultural consideration, health literacy, psychosocial, reproductive/fertility, and spiritual needs for patients, families and caregivers.
2. Supports a smooth transition of patients from active treatment into survivorship, chronic cancer management, or end-of-life care.
3. Assists patients with cancer with issues related to treatment goals, advanced directives, palliative care, and end-of-life concerns using an ethical framework that is nonjudgmental and nondiscriminatory.
4. Acts as a liaison between the patients, families, and caregivers and the providers to optimize outcomes.
5. Advocates for patients to promote patient centered care that includes shared decision making and patient goals of care with optimal outcomes.
6. Provides psychosocial support to and facilitates appropriate referrals for patients, families and caregivers.
7. Contributes to ONN program and role development, implementation, and evaluation within the healthcare system and community.
8. Participates in the tracking and monitoring of metrics and outcomes, in collaboration with administration, to document and evaluate outcomes of the navigation program.

PRIMARY DUTIES AND RESPONSIBILITIES:
Clinical Responsibilities:
- Performs radiotherapy on site at assigned department.
- Responsible for daily patient care and education. Appropriately involves the patient and their family in care and care decisions. Maintains and demonstrates respect for patient confidentiality and modesty. Explains all treatment and imaging procedures. Arranges daily radiation treatment schedule. Attentive to patient needs and concerns by continuous monitoring of the patient’s physical and physiological response and reactions to treatment and refer patient for appropriate management as needed.
- Administers radiation therapy treatment accurately and safely, following departmental policy and procedure.
- Positions and immobilizes patients according to the treatment plan and effectively evaluates images according to standard guidelines. Mobilizes patients using appropriate mechanical assistive devices, such as ceiling lifts and lateral transfer devices, during the provision of patient care.
- Assesses patient relative data as deemed pertinent to age-specific needs and provides care as described in the department’s policy and procedure manual.
- Participates in the clinical instruction and evaluation of the Radiation Therapy students. Accurately perform and teach all policy, procedure, and QA measures including New Start and Cone Down chart checks, weekly chart checks, scheduling, treatment procedures, image review guidelines, and billing procedures.
Simulation Procedures:
- Explains simulation procedure, including the administration of permanent tattoos for set up purposes.
- Operates equipment and software, including but not limited to CT scanner and software, Treatment planning software (Eclipse), LAP Lasers, IV contrast Power injectors.
- Create immobilization devices, bolus, and accurately documents set up instructions.
- Assists and interacts with all departmental Radiation Therapists, demonstrating the ability to answer all questions and details pertaining to simulation to ensure quality treatment.
- Participates in the IV contrast program, including Patient Screening, and Peripheral IV placement.
- Administers Oral Contrast.
- Assists in scheduling/rescheduling appointments, as needed.
Administrative Responsibilities:
- Assists the Lead Radiation Therapist in the efficient operation of the assigned clinical area. Performs daily QA measures to ensure proficient operation of all imaging and treatment equipment. Performs daily shutdown procedures. Assists in organizing team during equipment failures and notifies patients as well as all functional areas of the department. Assists in resolving clinical problems and difficult set-ups.
- Assumes Lead responsibilities in the absence of the Lead Radiation Therapist, when requested. Assists the Chief and Lead Radiation Therapist to ensure coverage for meeting attendance and administrative duties.
- Strives for continuous improvements and technique standardization.
- Assists with the triage of emergent cases and demonstrates the ability to negotiate the scheduling of emergent cases with other care team members.
- Accurately performs all new start QA measures, weekly chart checks, patient scheduling, treatment delivery, image review, and billing procedures involving the electronic medical record (EMR) system.
- Utilizes the Quality Assurance (QA) program to encompass all aspects of treatment, treatment delivery and quality patient care.
- Accurately documents treatment variances and inconsistencies using the appropriate tools.
- Practices the QA program of each treatment unit and assists with the instruction of new staff members and students as part of orientation.
- Responsible for the safe and accurate utilization of all aspects of the electronic medical record (EMR) and Record and Verify system. Accurately documents all treatment delivery information in the EMR.
- Participates in and appropriately documents the Time Out procedure for every treatment.
- Assists with orientation, teaching, and instructing new staff, students, visitors, and other staff members who are unfamiliar with the department or specific procedures.

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
- Demonstrated competencies for all procedures performed within assigned treatment and/or simulation areas, including but not limited to 3D, IMRT, Align RT and tattoo less workflows, stereotactic related procedures, Adaptive Radiotherapy and contouring, Craniospinal Radiotherapy, Pediatrics, Total Body Irradiation.
- Demonstrated ability to care for patients, including acute inpatients, pediatric patients, and pediatric patients requiring anesthesia.
- Demonstrated ability to perform all aspects of both treatment and simulation.

- Demonstrates an understanding of the patient functional mobility assessment tool and mobilizes patients using appropriate mechanical assistive devices, such as ceiling lifts and lateral transfer devices, during the provision of patient care.
- In-depth knowledge and demonstrated ability to accurately perform all new start QA measures, weekly chart checks, patient scheduling, treatment delivery, image review, and billing procedures involving the electronic medical record (EMR) system.
- Advanced knowledge of and ability to accurately perform and teach all policy, procedure, and QA measures including New Start and Cone Down chart checks, weekly chart checks, scheduling, treatment procedures, image review guidelines, and billing procedures.

Job Profile SummaryUnder general supervision of the Mammography/Ultrasound Supervisor, performs ultrasound scans to produce images to assist physicians in the diagnosis of injury and/or disease for patients of all ages. Performs related clerical and service duties.
I. Major Responsibilities:
1. Obtains and records appropriate patient clinical information and/or history; verifies physician assessment of each patient, making adjustments in the standard imaging procedures and techniques to accommodate patient size, age, or condition using Universal Protocol
2. Assists, transfers, and positions patients as needed, ensuring their modesty and physical safety, utilizing auxiliary equipment and/or devices as needed; selects equipment scan parameters appropriate to examination and patient condition
3. Processes and records scanned images per Radiologist protocol to ensure quality and accuracy. Reviews scans with Radiologists as requested. Accurately documents work in HIS/RIS/PACS systems.
4. Checks operation of equipment, sees that Q.A and maintenance schedules are adhered to; maintains cleanliness of tables and equipment to prevent cross contamination of contagious diseases, keeps rooms well stocked with linen and necessary supplies
5. Keeps up to date with changes in the field of ultrasonography by inservice study, attending courses or seminars as necessary and/or recommended, or participating in professional organizations.
6. Maintains yearly competency in Trophon disinfection processes
7. Assists in other area/modalities as needed

UMass Memorial Medical Center is the largest academic medical center in Central Massachusetts and the region’s only Level I Trauma Center. The Department of Cardiac Surgery is seeking a Nurse Practitioner or Physician Assistant to join a high-performing, collaborative academic team.
Our Cardiac Surgery service includes 16 Advanced Practice Providers, 3 Cardiac Surgeons, and 7 Surgical Intensivists. This is a full-time position with a rotating day/night schedule, including weekends and holidays. The standard schedule consists of three 12-hour shifts per week.
Our team provides comprehensive, state-of-the-art cardiac surgical care to a diverse patient population, including:
Open and endovascular aortic repair
Coronary revascularization
Structural heart procedures
Mechanical circulatory support, including a destination VAD program
The clinical scope spans the full continuum of perioperative cardiac surgery care, from high-acuity tertiary and quaternary cases to routine surgical procedures. The program performs approximately 550 cases annually.
Advanced Practice Providers are integral to the service and serve as the first point of contact for patient care across the ICU, post-operative floor, and outpatient clinic. Surgically trained APPs provide operative first assisting and endoscopic vein harvesting in all cases.
Role Responsibilities
Under the general supervision of a physician, the Advanced Practice Provider will deliver comprehensive medical care, including but not limited to:
Performing and documenting complete histories and physical examinations, with thorough review of the medical record to formulate diagnoses and treatment plans
Ordering, interpreting, and reviewing laboratory tests and imaging studies
Participating in daily inpatient rounds; evaluating patients; reviewing diagnostic data; and documenting daily progress notes
Requesting consultations and communicating effectively with consulting services
Performing routine bedside and clinical procedures in accordance with approved Delineation of Privileges
Assisting in surgical and other invasive procedures, as applicable
Position Qualifications:
Master’s or Doctoral degree from an accredited Nurse Practitioner or Physician Assistant program; board certification required
Adult-Gerontology Acute Care Nurse Practitioner (AGACNP-BC)
Current Massachusetts license
ACLS and BLS Certified
Preferred: 1–3 years of relevant clinical experience
