What You Will Do:
- Conduct virtual assessments and monitor patients' conditions remotely.
- Develop and coordinate individualized care plans in collaboration with the interdisciplinary team.
- Provide virtual education and support to patients, families, and caregivers regarding symptom management and end-of-life care.
- Liaise with field nurses, physicians, social workers, and other team members to ensure comprehensive care delivery.
- Respond promptly to urgent virtual consults and triage patient needs effectively.
- Utilize telehealth technology to conduct regular check-ins and follow-ups with patients and families.

Minimum Qualifications
- 5+ years of Home Care, Hospice, ER and/or Triage RN Experience
- Active RN license in a Compact State (eNLC)
- BSN required by an approved Nursing education program.
- Proficiency in Epic or Home Care EMR Experience
- Strong communication and interpersonal skills to build rapport with patients and families virtually
- Ability to work independently and as a part of collaborative remote team
- Able to adapt to rapid changes to meet business/department needs
- Patient/Customer Service Experience

About the job:
In accordance with the Mission and Guiding Behaviors; the RN Administrative Supervisor, in collaboration with the management team, serves as a support person in planning, organizing, directing and coordinating optimal patient care activities on the nursing units; and functions as a resource person to other hospital departments on given tour of duty within the standards, policies, procedures and guidelines of the organization.
What you'll do:
- Managing Performance: In collaboration with the management team provides coaching, direction and supervision of all staff members in house during assigned shift. Facilitates appropriate human, material and other resources to ensure effective patient care. Expedites patient flow to minimize diversion of patient access
- Education and Training: Partners with appropriate persons to educate employees in organizational policy, changes, and new technologies. Recommends, supports, and participates in education opportunities. Demonstrates appropriate clinical competencies.
- Financial Performance: Ensures financial performance and profitability of designated areas through monitoring and evaluating resource utilization and equipment distribution during assigned shift. Collaborates with Unit Directors and Patient Care Staff in maintaining the correct staffing throughout the hospital.
- Performance Improvement: In collaboration with the management team, focuses on optimal patient outcomes, regulatory compliance and quality improvement initiatives.
- Communication: Follows and/or establishes communication channels to ensure appropriate effective internal and external communication between and among stakeholders.
- Service Excellence: Monitors and ensures optimal level of patient, staff and physician satisfaction and patient expectation and service levels.
- Meets population specific and all other competencies according to department requirements.
What we're looking for:
- Education: Graduate from school of nursing. Baccalaureate degree strongly desired.
- Licensure / Certification: Current license to practice as a Registered Nurse in State of Ohio.
- Experience: Minimum of four to five years recent clinical experience in hospital setting with Care Manager/Charge Nurse or other relevant hospital management experience required.
- Effective Communication Skills
- Demonstrates results oriented leadership and management ability.
- Demonstrates facilitation and team skills.
- Telemetry qualified or has attended the basic ECG course.

Position Purpose:
- This Diet Clerk Position will be working in Food & Nutrition Services at Mount Carmel Grove City working five 8-hour shifts per week from 11:30am - 8:00pm with rotating weekends and holidays.
In accordance with the Mission and Guiding Behaviors, the Diet Clerk performs clerical and administrative duties in support of patients, clinical dietitian and patient meal services. The Diet Clerk functions in the spirit of teamwork, is accountable for communications with the standards, policies, procedures and guidelines of the organization.
What you will do:
- Exhibits each of the Mount Carmel Service Excellent Behavior Standards holding self and others accountable and role modeling excellence for all to see. For example: demonstrates friendliness, courtesy and effective communication skills. Creates a professional environment and provides first class service.
- Visits all patients as assigned assisting the patient or family member with menu selections.
- Responds to patient needs, requests in a timely manner.
- Demonstrates knowledge of special modified diets using department policies and procedures to assure patient receives the diet ordered.
- Checks patient trays for accuracy during meal service.
Minimum Qualifications:
- Education: High school graduate or equivalent highly preferred. Additional college training in Dietetics or related field preferred. Must know how to read, write and speak English.
- Experience: Previous health care experience or demonstrated knowledge of dietetics and medical terminology preferred.
- Demonstrates priority setting, organizational skills and the ability to remain calm under stressful situations.
- Possesses a high level of professionalism and the ability to establish good rapport with patients, family members, staff and physicians.

What you'll do:
- Exhibits each of the Mount Carmel Service Excellence Behavior Standards holding self and others accountable and role modeling excellence for all to see. For example: demonstrates friendliness and courtesy, effective communication creates a professional environment and provides first class service.
- Cleans patient rooms and other assigned areas daily, including collecting and removing trash, changing bed linens and sanitizing all surfaces
- Stocks paper items and other supplies in patient rooms as necessary or designated
- Move furniture for repair or cleaning and set up meeting rooms.
- Assist in cleaning of emergency spills/floods that are observed or requested
- Proactively communicates with all staff members in an accurate & timely manner
- All other duties as assigned.
What we're looking for:
- Education: High School Graduate or GED preferred.
- Ability to clearly and respectfully communicate with patients, visitors, and all colleagues and be able to verbalize approved scripting with all the above.
- Reading, basic math and writing skills required.
- Knowledge of the correct use of chemicals, dilution of chemicals, supplies, tools and equipment. When applicable, must also know proper use and procedures to operate the steam sterilizer, grinding/shredding system, waste compactor, recycling compactor and cardboard balers

Position Purpose:
Full Time PAT RN, non-patient facing position. Collaborates with surgeon's offices and medical offices to clear patients for surgery. Assists with delineation of PAT information into EHR, medication and medical history into EHR.
