You will evaluate, develop, and implement personalized care plans to promote independence with activities of daily living, cognitive function, and safety, helping our residents achieve their highest potential within a supportive community setting. Your passion for Occupational Therapy and commitment to resident-centered care will make a meaningful difference every day. We are building an In House Therapy Team and cannot wait to see the positive impact it will bring to the residents of The Victorian.
Responsibilities
● Conduct comprehensive assessments of residents' functional abilities, including activities of daily living (ADLs), cognitive status, upper extremity function, and safety awareness, within The Victorian.
● Develop and execute tailored care plans that incorporate evidence-based occupational therapy techniques, such as ADL retraining, therapeutic exercise, cognitive-perceptual retraining, and adaptive equipment training, specifically adapted for geriatric populations and those with various age-related conditions.
● Provide hands-on therapy sessions to improve resident outcomes in areas like geriatrics, orthopedic rehabilitation, neurological rehabilitation, low vision, and post-surgical recovery.
● Collaborate with an interdisciplinary team including nurses, physical therapists, speech therapists, and facility staff to coordinate care, optimize resident well-being, and facilitate seamless transitions on and off therapy.
● Educate residents and their families on treatment strategies, fall prevention, adaptive equipment and assistive device use, energy conservation techniques, and home exercise programs to support ongoing recovery and promote independence within their living environment.
● Maintain accurate and timely documentation within electronic medical record (EMR) systems, including initial evaluations, progress notes, treatment plans, and discharge summaries, adhering to all regulatory guidelines.
● Stay current with best practices in geriatric occupational therapy, medical terminology, and anatomy knowledge to deliver precise and effective interventions relevant to the resident population.
Skills
● Experience working with geriatric populations, preferably within skilled nursing, assisted living, or long-term care facilities, with a strong understanding of age-related physical and cognitive needs.
● Expertise in care planning for diverse populations, with a focus on patient-centered and resident-centered care, emphasizing functional independence and quality of life.
● Proficiency in a range of occupational therapy techniques, including ADL retraining, therapeutic exercise, cognitive-perceptual retraining, and adaptive equipment training appropriate for geriatric rehabilitation.
● Familiarity with therapy discharge planning processes and facilitating safe transitions for residents coming off therapy.
● Experienced with EMR systems for efficient documentation and resident record management - Specifically Net Health Optima and Point Click Care.
● Comprehensive knowledge of physiology, anatomy, medical terminology, and the principles of occupational therapy practice, especially as they apply to the older adult.
● Strong ability to communicate effectively and compassionately with residents, families, and team members, fostering a positive and encouraging therapeutic environment.
● Ability to adapt therapy approaches to meet the unique needs and cognitive levels of residents varying levels of physical and cognitive function.
● Willingness to supervise 1-2 COTAs and assist with supervisory documentation needs.

ESSENTIAL FUNCTIONS
1. Provides primary care in accordance with established health center protocols within the physician’s scope of practice and Ohio Scope of Practice guidelines
2. Completes assessments, medication regime, and monitors progress of clients in Medication Assisted Treatment Program
3. Routinely screens for cognitive impairment, common mental health problems and behaviors that compromise health, using evidence-based and developmentally appropriate tools

This is a Full-time night position working in the Med/Surg department 3 12-hour shifts 7pm – 7:30am, and every third weekend. This position is eligible for a $10,000 sign on bonus.
The Registered Nurse in the Medical/Surgical Department is accountable for the coordination of nursing care, patient education, and discharge planning.
Essential Functions
Use assessment skills and techniques to determine patient needs and condition upon admission and on an ongoing basis, responding appropriately.
Completes and documents the nursing admission assessment in a thorough and accurate manner as defined by unit policy.
Assesses the need for patient and family education.
Hourly Rounding on patients.
Initiate, develop and document patient plan of care based on patient’s assessment and medical diagnosis and setting priorities for nursing action in accordance with the unit’s policy on time-frames.
Review and update the plan of care once every 12 hours or as necessary.
Develop a patient education and discharge plan as necessary.
Collaborates with other health team members in planning and providing care.
Delegates care to team members based on complexity, staff education and competency, state regulations and Knoxville Hospital and Clinics’ policy.
Provides safe and comprehensive nursing care according to state regulations, hospital policy and procedure and DNV standards.
Establish care priorities based on patient problems; communicate with staff and new employees of patient problems, concerns, and changes in condition. Notify physician promptly regarding concerns.
Must be proficient in medication administration, including IV therapy, as evidenced by lack of medication errors, reports medication errors.
Perform procedures accurately, completely, and safely according to established policy and procedure and physician orders.
Provide care with minimal supervision in a timely manner.
Maintains a safe work environment for patients and coworkers. Uses safety precautions and PPE appropriately.
Use an interdisciplinary approach to patient care.
Use critical thinking skills to intervene with appropriate care for urgent or emergent patient care needs.
Job Requirements
Current license as a Registered Nurse in Iowa.
Current BLS with ACLS and PALS preferred within one year.
Mandatory Reporter Training.
One year general Medical / Surgical experience preferred.
Required to complete all annual competency training and maintain license and/or certification.
Care of acute and chronically ill patients.
Able to identify and manage life-sustaining physiologic functions in unstable patients.
IV and Central Line management skills.
Additional Essential Functions and certifications as needed for area.

SPECIFIC DUTIES/ ESSENTIAL FUNCTIONS:
ESSENTIAL FUNCTIONS: Dispensing and Overseeing Operations of the Outpatient Pharmacy
1. Accurately and timely fill all prescriptions
2. Review prescriptions for Drug-Drug Interactions, Food-Drug Interactions, Drug Allergies, Drug Misuse, etc
3. Maintain patient profiles and pharmacy records
4. Counsel patients on their medications
5. Supervise pharmacy technicians, pharmacy students, and pharmacy residents
6. Participate in 3rd party billing and inventory management operations
7. Oversee patient assistance program
8. Follow current and new Federal and State Laws, including 340b integrity regulations
ESSENTIAL FUNCTIONS: Provide Clinical Pharmacy Services
1. Work in collaboration with all members of the care team to assess, facilitate, plan and advocate appropriate services to meet the patient’s health requirements on an individual basis
2. Provide Medication Therapy Management (MTM)
3. Provide immunizations
4. Precept pharmacy students
5. Maintain good working rapport with physicians and other health care professionals
6. Answer drug information questions posed by providers
7. Provide educational in-services to medical and dental providers
8. Participate in clinical service development/improvement
9. Demonstrate ongoing professionalism including a personal commitment to advancing self and the profession
OTHER DUTIES
1. Assist with housekeeping and maintenance of pharmacy

REQUIRED JOB QUALIFICATIONS:
PharmD Degree
Active State of Ohio Pharmacist License
ADDITIONAL REQUIREMENTS AND CONDITIONS OF EMPLOYMENT:
Three years of clinical experience or post-graduate residency training in a community or clinic pharmacy
Effective interpersonal communication skills
Current BLS certification
180-day probationary period
Background check including Ohio Bureau of Criminal Investigation, Medicare exclusion list, verification of valid driver’s license, employment medical exam, including a TB test, adherence to all CAA policies/procedures and requirements of all funding sources. CAA is a drug/alcohol-free and smoke-free workplace and post-offer drug and alcohol testing is required.
All Health, Behavioral Health, Dental and Administrative employees are required by the Centers for Medicare and Medicaid (CMS) to have a COVID-19 vaccination(s) as a term and condition of employment at Community Action Agency of Columbiana County (CAACC).
