*Responsibilities and Duties*
AFC Urgent Care providers evaluate and treat patients in an efficient and effective manner when immediate care is needed. AFC providers must be comfortable assessing pediatrics as young as 6 months old. They also must be comfortable in performing procedures in which a local anesthetic may be used. AFC providers conduct annual, sports, school, and pre-employment physicals, vaccinations, and interpret results from diagnostic services such as labs and x-rays.
*Qualifications and Skills*
AFC providers must be Board Certified or Board Eligible with AANPCP, ANCC or NCCPA; have an active Tennessee State License, DEA number, be (or willing to become) DOT Certified, current ACLS, and registered with CAQH.

What You’ll Do
Provide evidence-based pediatric occupational therapy in the home and community
Perform evaluations, assessments, and ongoing progress monitoring (OTs)
Implement treatment plans under supervision (COTAs)
Develop and/or execute individualized treatment plans and goals
Deliver interventions to improve ADLs, fine motor skills, sensory processing, feeding, cognition, and social/emotional development
Educate and empower families and caregivers for carryover in daily routines
Collaborate with physicians and interdisciplinary care teams
Document care in an electronic medical record system
Maintain compliance with all clinical and regulatory standards
Qualifications
Required:
Active, unrestricted OT or COTA license (Colorado)
Graduate of an accredited Occupational Therapy or OTA program
Current CPR certification
Strong communication and interpersonal skills
Preferred:
1+ year of pediatric experience
Home health experience

Responsibilities:
Essential Duties and Responsibilities of the Physical Therapist:
Conduct comprehensive evaluations of patients' physical conditions, functional limitations, and mobility.
Develop individualized treatment plans based on clinical assessment and patient goals.
Utilize various therapeutic techniques, including exercises, manual therapy, and modalities such as ultrasound, electrical stimulation, and heat/cold therapy.
Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.
Communicate effectively with other healthcare professionals, patients, and families regarding treatment plans, progress, and discharge recommendations.
Educate patients and their families on the therapeutic process, home exercise programs, and injury prevention.
Provide support and encouragement to patients to enhance their motivation and compliance with therapy.
Work collaboratively with interdisciplinary teams, including physicians, nurses, occupational therapists, and speech therapists, to ensure comprehensive patient care.
Participate in team meetings, case conferences, and discharge planning to coordinate patient care.
Adhere to regulatory and facility policies, including documentation standards, safety protocols, and infection control procedures.
Participate in quality improvement initiatives and contribute to maintaining high standards of patient care.

Qualifications:
Qualifications of the Physical Therapist:
Graduate of a physical therapy curriculum accredited by the American Physical Therapy Association (APTA) or the Commission on Accreditation in Physical Therapy (CAPT).
Hold a valid State Physical Therapy License.

Our Level II Maternity Center features a 10-bed Labor, Delivery, Recovery, and Postpartum (LDRP) unit with a dedicated operating room for labor and delivery. The practice averages approximately 925 deliveries annually and offers significant surgical volume, with an estimated 50/50 split between gynecology and gynecologic surgery. Physicians have access to advanced technology, including the da Vinci Xi robotic surgical system, allowing for minimally invasive procedures such as hysterectomies, endometriosis treatment, and fibroid removal.
Position Highlights
Employed physician model with competitive salary plus RVU-based incentive bonus
Comprehensive OB and GYN practice with strong surgical opportunities
Diverse patient population representing a wide range of backgrounds and cultures
Integrated clinic schedule caring for both OB and GYN patients
Responsibilities include labor and delivery coverage, antepartum/postpartum care, triage, gynecologic consultations, and surgery
Average clinic volume of 20–25 patients per day
Approximately 90 deliveries per month (average of 3 deliveries per day)
Call & Practice Support
Shared 24-hour call with a current 1:3 rotation, expected to improve to 1:4 or 1:5
Call is home call with a 20-minute response time; no in-house call requirement
Residents manage emergency department consultations and triage
Collaborative support from certified nurse-midwives and Family Medicine physicians with OB training, who independently manage their deliveries while on call
High-risk maternal-fetal medicine, infertility, urogynecology, and other subspecialty cases are referred to specialty centers

Compensation & Benefits
Competitive annual salary with RVU productivity incentives

This job is a gem of an opportunity! Colonial Pharmacy is a busy, cutting edge, independent pharmacy providing classic pharmacy services as well pharmacist provided clinical services. It is well staffed with pharmacist over-lap, has technologies such as two filling robots, Icon, PioneerRx, and a FlavoRx FillMaster. The pharmacy manager has been an owner/operator for over 7 years. He will be on hand to oversee the management but is looking for a staff pharmacist to focus on expanding the business into clinical services with collaborative practice agreements and a medical at home program.
An ideal pharmacist will have an entrepreneurial spirit, be hard working, efficient, self-motivated, have excellent attention to detail, great customer service and people skills. The primary tasks of a new pharmacist will be to work with the owner/PIC and staff pharmacists verifying scripts, providing consultations, interfacing with providers and problem solving as needed. Additional duties will include vaccines, point of care testing and management of MTM, Outcomes and Equip and Med sync programs.
