*Company:* Twin Cities Physicians
*Location:* St. Cloud, Minnesota & Surrounding Communities | Additional Willmar Coverage
*Schedule:* *Full-Time or Part-Time | Monday through Friday | No Evenings, Weekends, or Hospital Call*
*Compensation:* *$120,000 - $160,000+ Total Compensation* (Base Salary + Productivity Incentive Program) + *Mileage Reimbursement*
Become the Provider Communities Count On
There's a difference between seeing patients and truly becoming someone's provider.
When you care for the same population over time, people begin to know you. Community nurses call because they trust your clinical judgment. Families recognize your name. You understand your patients' histories, their baseline, their chronic conditions, and the small changes that might mean something bigger is happening.
*That's the kind of primary care we're building at Twin Cities Physicians.*
We're looking for a Nurse Practitioner to provide *on-site primary care to older adults throughout St. Cloud and surrounding communities*, with additional coverage in the Willmar area as needed.
Instead of practicing inside the same clinic every day, *you go to your patients*, caring for seniors where they live and building relationships with the communities surrounding them.
You'll have significant clinical autonomy, a *Monday through Friday schedule with no evenings, weekends, or hospital call*, and *$120,000 - $160,000+ in total compensation* with additional earning potential through our productivity incentive program.
Build Your Practice Across Central Minnesota
This isn't a role where every morning starts with a waiting room full of unfamiliar faces.
You'll develop a consistent patient population across senior living communities and follow those patients over time.
You'll learn their histories. You'll manage their chronic conditions. You'll see them after hospitalizations. You'll recognize changes in their health. And you'll work closely with the nurses and caregivers who interact with them every day.
That continuity gives you something increasingly difficult to find in healthcare: *the ability to understand the whole patient instead of treating a single moment in time.*
Your primary territory will center around *St. Cloud and surrounding communities*, with the flexibility to provide additional coverage in the *Willmar region* when needed.
For the right provider, it's an opportunity to build a meaningful clinical presence across Central Minnesota.
Practice Independently Without Practicing Alone
You'll have the autonomy to *assess, diagnose, treat, prescribe, manage chronic conditions, and develop individualized care plans* for your patients.
But autonomy doesn't mean isolation.
Twin Cities Physicians is a *physician-led medical group*, giving you access to clinical collaboration, organizational resources, and an experienced team while still allowing you to practice with meaningful independence in the field.
*You own the patient relationship. We provide the platform behind you.*
What This Career Gives You
* *$120,000 - $160,000+ total compensation*
* *Productivity incentive program* with additional earning potential
* *Full-Time or Part-Time* opportunities
* *Monday through Friday*
* *No evenings, weekends, or hospital call*
* *Mileage reimbursement*
* Significant *clinical autonomy*
* Build and manage a consistent *patient panel*
* Develop long-term relationships with patients, families, and senior living teams
* Collaboration with a *physician-led clinical organization*
* Comprehensive benefits package
* *Paid Time Off + Paid Holidays*
* CME Allowance
* Malpractice Coverage
* DEA Reimbursement
* Clinical and administrative resources
See More Than You Can See in an Exam Room
Providing care where your patients live changes your perspective.
You'll work within *assisted living, memory care, independent living, and skilled nursing communities*, giving you greater visibility into the factors affecting your patients' health.
You see their environment. You understand their functional status. You communicate directly with the nurses and caregivers who know their daily routines. You see how medications are being managed and what support systems are actually available.
When a patient returns from the hospital, you're there to reconnect the pieces. When a nurse notices a subtle change, they know who to call. When a family has questions, they're talking to a provider who understands their loved one's history.
*You're not just treating today's diagnosis. You're managing what happens next.*
What You'll Be Doing
* Provide comprehensive *on-site primary care* within senior living communities
* Round primarily throughout *St. Cloud and surrounding communities*, with additional Willmar coverage as needed
* Manage your own panel of *older adult and medically complex patients*
* Diagnose and manage acute and chronic medical conditions
* Perform comprehensive assessments, *annual wellness visits*, preventative care, and routine follow-ups
* Develop individualized treatment plans based on patient needs and goals
* Manage medications and *chronic disease treatment plans*
* Monitor changes in patient health over time
* Coordinate care following hospitalizations, rehabilitation stays, and specialty visits
* Collaborate with physicians, nurses, therapists, pharmacists, caregivers, and senior living teams
* Educate patients and families regarding diagnoses, medications, and treatment plans
* Identify concerns early and help *reduce avoidable hospitalizations and readmissions*
* Maintain accurate and timely clinical documentation
Your Background Doesn't Have to Say "Senior Living"
Some of our strongest potential providers may never have worked specifically in senior living.
Experience in *primary care, family practice, internal medicine, geriatrics, long-term care, skilled nursing, post-acute care, hospital medicine, home health, hospice and palliative care*, or other settings serving medically complex adults can translate extremely well.
What matters is whether you have the *clinical judgment, independence, communication skills, and interest in older adults* needed to manage a medically complex patient population.
If you enjoy getting to know your patients instead of constantly starting over with someone new, you'll understand why this model is different.
What We're Looking For
*Required Qualifications*
* Active *Minnesota Nurse Practitioner license*
* Strong assessment, diagnostic, and *clinical decision-making skills*
* Ability to practice independently while collaborating with a physician-led team
* Strong communication and care coordination skills
* Genuine interest in *caring for older adults*
* Reliable transportation and comfort with community-based travel
* Ability to provide care throughout *St. Cloud and surrounding communities*, with flexibility to assist in the *Willmar region*
*Preferred Qualifications*
* Experience managing *medically complex older adults*
* Chronic disease management experience
* Primary care, geriatric, or community-based clinical experience
* Assisted living, memory care, or skilled nursing experience
* Advanced wound care experience
* *Wound Care Certification (WCC)* is a plus
A Different Kind of Provider Career
This role does require travel between communities. In exchange, you're not tied to the traditional clinic model or its schedule.
You get *clinical autonomy without professional isolation, productivity without an assembly-line clinic, and continuity with the patients you're responsible for.*
You get a *Monday through Friday schedule with no evenings, weekends, or hospital call*, mileage reimbursement, strong earning potential, and the resources of an established clinical organization behind you.
And over time, something else happens.
You stop being _a_ provider who visits the building.
*You become their provider.*
About Twin Cities Physicians
Twin Cities Physicians is a growing, *physician-led medical group* providing proactive primary care to seniors throughout Minnesota.
Our providers bring healthcare directly into assisted living, memory care, independent living, and skilled nursing communities, helping improve *access to care, continuity, communication, quality of life, and patient outcomes while reducing avoidable hospitalizations*.
We're expanding our presence throughout Central and Western Minnesota and looking for providers who want to help us build something lasting in the communities they serve.
Make Your Impact Bigger Than a Single Appointment
If you're ready for a primary care career where you can *build relationships, practice with autonomy, earn $120,000 - $160,000+, and become the provider communities know and count on*, we'd like to meet you.
*To stand out, email Tom Lecy at [email protected]* with your resume and tell us about your clinical background, your experience caring for older adults, and what you're looking for in the next stage of your career.
Pay: $120,000.00 - $160,000.00 per year
Benefits:
