What You'll Do
Serve as the attending physician for patients in each assigned SNF.
Collaborate with NPs/PAs to manage daily patient care and oversee clinical supervision.
Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
Participate in Theoria's Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
Your Qualifications
Physicians (MD/DO) must hold an active U.S. medical license and be board-certified in an appropriate specialty. New graduates who are board-eligible and actively pursuing licensing are encouraged to apply. Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.

You’ll work closely with our doctors and clinical team to help patients understand their oral health, manage periodontal disease, and feel confident about their treatment decisions.
What You'll Do
Provide preventive and periodontal hygiene care
Complete periodontal assessments and charting
Perform prophylaxis and scaling and root planing as appropriate
Take diagnostic dental x-rays
Educate patients about home care, periodontal health, and prevention
Help patients understand recommended treatment
Build long-term patient relationships
Collaborate closely with our doctors and clinical team
Maintain accurate and timely clinical documentation
Support a comfortable, positive patient experience
What We're Looking For
Active Colorado Dental Hygienist license, or eligibility for licensure
CPR/BLS certification
Strong patient communication skills
Commitment to high-quality clinical care

Reliable, organized, and detail-oriented
Comfortable learning and using modern dental technology
Experience with Open Dental, Dentrix, or similar software is a plus
