The Pediatric Hematology/Oncology Clinical Pharmacy Specialist provides advanced, comprehensive pharmaceutical care to children and adolescents with hematologic and oncologic conditions in the ambulatory setting. This pharmacist will practice as an integral member of the multidisciplinary care team, partnering closely with physicians, advanced practice providers, nurses, and the pharmacy staff to ensure safe, effective, and evidence ‑ based medication management.
The specialist will support chemotherapy management, optimize supportive care, participate in clinical research and quality improvement initiatives, and serve as a preceptor for pharmacy students and residents.
Essential Functions:
Review and interpret medication orders; assess for clinical appropriateness, therapeutic goals, adverse effects, contraindications, interactions, and disease ‑ specific considerations.
Evaluate, monitor, and modify drug therapy for pediatric hematology/oncology patients, including chemotherapy and supportive care.
Provide comprehensive medication counseling for patients and families, including oral chemotherapy education and adherence support.
Serve as the clinical liaison between the pediatric oncology practice and the Specialty Pharmacy, collaborating with pharmacy staff and support teams to ensure timely medication access and continuity of care
Advance standardized, evidence‑based care by developing and updating clinical guidelines, pathways, and EMR‑based decision support.
Participate in drug therapy consults and pediatric oncology clinical research protocols as needed.
Support quality assurance and improvement efforts to uphold accreditations and ensure compliance with regulatory and institutional standards.
Provide drug information and clinical recommendations to pediatric oncology providers, nurses, and other healthcare professionals.
Precept pharmacy students and residents; contribute to educational programming within the pharmacy department.
Representing Pharmacy Services on committees and workgroups as assigned.
Qualifications:
PharmD with PGY-1 residency or 5 years pediatric hospital experience required.
PGY-2 specialized residency training in pediatrics and/or Hematology-Oncology preferred.
Experience in ambulatory oncology pharmacy practice preferred.
Hospital pharmacy experience required (pediatric hospital pharmacy experience preferred).
Active Pharmacist License (or legally eligible) in States of Delaware, New Jersey & Pennsylvania required.
Board Certification in Oncology Pharmacy Specialty (BCOP) or BCPPS required within 12 months of eligibility.
American Heart Association BLS and PALS required upon hire. ACLS preferred.
PA Clearances are not required for those associates hired to work in DE.

NCH‑DE is a multi‑specialty, freestanding pediatric hospital and the only ACS‑verified Pediatric Level I Trauma Center in the state of Delaware. Our Emergency Department has roughly 55,000 visits annually and serves a broad catchment area across Delaware, Pennsylvania, New Jersey, and Maryland, offering exposure to a wide breadth of pediatric emergency and trauma care.
In 2023, our 44-bed ED expanded to include a 6‑bed Behavioral Health Unit, staffed in close partnership with psychiatry to better meet the needs of pediatric patients requiring urgent psychiatric care. As a pharmacist on this team, you will play a key role in shaping safe, evidence‑based medication use across high‑acuity and fast‑paced clinical environments.
The Role
The Pediatric ED Clinical Pharmacy Specialist provides advanced, comprehensive pharmaceutical care to children and adolescents presenting with a wide range of acute and complex conditions. This role is deeply embedded within the multidisciplinary care team, working side‑by‑side with physicians, advanced practice providers, nurses, and pharmacy colleagues.
You will serve as a subject matter expert and real‑time resource for medication management in the emergency setting, contributing directly to patient safety, clinical decision‑making, and optimized therapeutic outcomes.
In addition to direct patient care, this specialist leads and participates in policy development, medication safety initiatives, quality improvement efforts, and education of pharmacy learners and healthcare professionals across disciplines.
Key Responsibilities
Clinical Care & Emergency Response
Review, verify, and interpret medication orders for clinical appropriateness, therapeutic goals, safety, contraindications, interactions, and disease‑specific considerations.
Participate actively in medical emergencies, including code responses, providing expert medication preparation, dosing, and recommendations in real time.
Ensure continuity of pharmaceutical care across acute and ambulatory settings by:
Obtaining complete and accurate medication histories
Performing comprehensive medication reconciliation with proper EMR documentation
Provide patient‑ and family‑centered medication counseling tailored to pediatric needs.
Clinical Standards, Safety & Quality
Advance standardized, evidence‑based care through development and maintenance of clinical guidelines, treatment pathways, and EMR‑based decision support tools.
Lead and collaborate on ED‑focused medication safety initiatives, medication use evaluations, and quality improvement projects.
Support accreditation and regulatory compliance through ongoing quality assurance and performance monitoring.
Serve as a trusted drug information resource for providers, nurses, and other healthcare professionals.
Education & Leadership
Precept pharmacy students and residents; contribute to departmental and institutional educational programming.
Develop and deliver pharmacy‑related education and in‑service training for multidisciplinary healthcare teams.
Participate in or lead committees, task forces, and continuous improvement initiatives as assigned.
Ensure pharmacy services align with established standards of care and are responsive to age‑specific patient needs.
Qualifications

Key Responsibilities:

Job Description
The Ulster BOCES Center for Innovative Teaching & Learning (CITL) Student-Based Programs is seeking an Education of Speech & Hearing Handicapped Teacher for a full-time, 10-month position for the 2026-2027 school year. Experience working with special needs children and Certificate of Clinical Competence preferred. Appropriate NYS certification and fingerprinting required.
Ulster BOCES provides programs for students ages five through twenty-one who are identified as needing specialized programs and/or supports according to their Individualized Education Program (IEP). Special Education programs are offered at Ulster BOCES sites in Port Ewen, NY and in leased classrooms in component school districts. They have been developed and offered to meet the specific needs of our students in order to assist them in realizing their potential to achieve both academically and socially.
The following programs are available in Special Class configurations of 12:1:1, 8:1:1, 8:1:2, 6:1:2 and 4:1:2. All classes are staffed by a full-time Special Education teacher and either one or two teaching assistants (additional supplementary 1:1 aides may be requested for individual students as per Committee on Special Education recommendations). Multiple programs are available.

1. Provides academic experiences appropriate to the needs of pupils. Determines speech difficulties of assigned students; prescribes methods of remediation of diagnosed difficulties; and implements methods and materials prescribed to remediate difficulties.
2. Develops and uses instructional materials suitable for instruction of pupils with a wide range of mental, physical and emotional maturities.
3. Develops in all pupils an awareness of their worth as an individual and their role in their family and community. Encourages pupils to express themselves creatively.
4. Provides individual and small group instruction designed to meet the individual needs of pupils.
5. Assists regular education teachers in planning and coordinating appropriate activities for the speech impaired students.
6. Promotes parent participation and involvement in educational activities provided for their children.
7. Communicates regularly with parents by means of written communication and individual parent conferences. Interprets school program to parents in order to strengthen parental understanding of the individual pupil's needs and the school's role in the individual pupil's life.
8. Provides appropriate climate to establish and reinforce acceptable pupil behavior, attitudes and social skills.
9. Cooperates with other professional staff members in assessing and helping pupils solve health, attitude and learning problems.
10. Maintains professional competence through in-service education activities provided by the district and self-selected professional growth activities.
11. Selects and requisitions books, instructional aides and instructional supplies.
12. Participates in curriculum and other developmental programs as required.
13. Participates in faculty meetings and committees.
14. Assumes other duties as assigned by the Supervisor.
