Vesa Health & Technology is seeking a Registered Nurse Case Manager/Advocate to support the Primary Care Clinic at the Coast Guard Station in Kodiak, AK!
_\*\*The clinic requires that the RN must have at least 3 years of case management experience in order to be considered.\*\*_
The program seeks to implement a patient and family centered health care delivery for primary care that is team-based, comprehensive, and designed to fully meet the complete primary care health and wellness needs of patients.
RN will provide case management and patient advocate care. Will provide advanced practice clinical, administrative, and organizational skills in managing the continuity, access, and provision of care services for complex and defined patients and address the needs of individuals and families with behavioral health needs. Will work with other providers to support case management services to high risk patients, work within a multidisciplinary team to provide patient advocacy/support, and will occasionally be required to provide tele-case management services throughout the entire U.S. Coast Guard.

*Qualifications:*
* Be a graduate from an accredited nursing program
* Possess a valid RN license in 1 of the 50 states.
* Possess a minimum experience of 3 years as an RN case manager
* BLS
