- SUMMARY: The Nurse Practitioner or Physician Assistant is accountable to facilitate a quality hospital experience and outcome for patients within a cost-effective time frame. The Nurse Practitioner or Physician Assistant will be responsible to the Medical Director of Cardiology and responsible for overseeing the progress of a specific caseload of patients across the care continuum, primarily for acute, in hospital care. Acts as an educator, consultant, and liaison to healthcare providers caring for the patients across the care continuum. RESPONSIBILITIES:
- Demonstrates a fundamental medical knowledge base with mastery of knowledge within a defined content area (cardiology).
- Able to manage a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care.
- Provides leadership in the development, validation, implementation, and evaluation of standards, policies, procedures, and critical paths to support the patient care delivery model.
- Oversee care coordination in the acute care setting, including identifying the need for admission via triage from the Emergency Department, inpatient care, and discharge planning.
- Partner with the healthcare team to achieve mutually agreed upon healthcare goals by providing necessary support, education and encouragement.
- Demonstrates advanced physical and psychosocial assessment skills and knowledge of family dynamics.
- Provides direct care to patients in the acute care setting in collaboration with the Cardiologist.Performs advanced practice skills under the supervision of the Medical Director.
- Participates in the education, clinical research and mentoring of healthcare providers.
- Facilitates patient/family education in area of expertise.
- Position may include rotation to the Cardiology Procedural Care Unit and/or Advanced Heart Failure service.Annually completes mandatory safety infection control, BLS, and ACLS.
- Prescriptive privileges per licensure. MINIMUM QUALIFICATIONS: Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.Baccalaureate in nursing, or equivalent. At least a Master of Science Degree in the field of nursing. Graduate of accredited Nurse Practitioner program. Specialty certification required.Current registration as a Nurse Practitioner with prescriptive privileges in the State of Rhode Island.OR Bachelor’s Degree and two years of graduate study in an accredited Physician Assistant program.Current registration as Physician Assistant in the State of Rhode Island.Certified in Basic Cardiac Life Support (BLS), and Advanced Cardiac Life Support (ACLS).Demonstrates knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages and cultural patterns in each step of the care process.
EXPERIENCE:
Two years related experience as RN in acute care hospital setting and preferred 1-2 years experience as a nurse practitioner or 1-2 years experience as a Physician Assistant. Experience in cardiology preferred. Experience with EPIC a plus.
WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:
General hospital environment with occasional stressful conditions associated with patient care.

SUMMARY:

RESPONSIBILITIES:
- In conjunction with Surgical Services Manager and Service Line Coordinators for specialty areas, communicates surgeon availability, patient acuity, and provides planning methods to achieve desired result for area of responsibility.

- Manage the resources, both human capital and operational capital in an efficient and fiscally responsible manner throughout tour of duty.

- In cooperation with the departmental service line coordinators and frontline staff, assures availability of all specialty items (instruments, equipment, special order supplies, etc.) to provide care to patient during surgery.
- In cooperation with in-service Education Coordinator and departmental supervisory staff, arranges for daily huddles, updates huddle and staffing boards, on-going in-service education programs for new practices and procedures within specialty area. Identifies need, plans and provides formal and informal programs for subordinates and other departmental personnel as appropriate.
- Escalates physician issues to the OR medical director, or his/her designee, when conflict resolution is needed in planning, or prioritizing of surgical cases.
- Maintains smooth coordination of effort and effective communications with other divisions and departments, i.e. surgeons, anesthesiologist, recovery room, etc. to provide for optional patient care in the Operating Room setting.
- May occasionally perform duties of professional nurse as necessary to cover for absence or staffing shortage.
- Maintains and enhances professional expertise through participation in appropriate educational activities.
MINIMUM QUALIFICATIONS:
Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing strongor/strong licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.
BASIC KNOWLEDGE:
Completion of formal education in an accredited school of nursing.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Clinical Care:
Conduct comprehensive health assessments in patients' homes and community settings, if needed.
Evaluate, diagnose, and treat acute and chronic medical conditions within the APP scope of practice.
Perform urgent and episodic visits to support Mobile Integrated Health program.
Communicate with paramedics involved with clinical care in MIH program.

Collaborate with physicians, nursing staff, social workers, pharmacists, behavioral health providers, paramedics, and care managers to ensure coordinated, holistic care.
Participate in multidisciplinary case reviews and care planning discussions.
Assist with transitions of care following hospitalization, emergency department visits, or skilled nursing facility stays.
Coordinate referrals to specialty care, home health services, community resources, and supportive services.
Address social determinants of health and barriers to care through collaboration with community partners and care coordination teams.
Engage family members and caregivers as integral members of the care team.
Program Operations and Quality:
Maintain accurate, timely, and complete documentation in the electronic medical record.
Participate in quality improvement initiatives designed to improve patient outcomes, safety, and operational efficiency.
Monitor program quality metrics, patient outcomes, and care management goals.

EDUCATION:
Current registration as a Physician Assistant or Nurse Practitioner in the State of Rhode Island.
Nurse Practitioner (NP):
Master's or Doctoral degree in Nursing from an accredited program.
Current Nurse Practitioner certification in Family Practice (FNP), Adult-Gerontology (AGNP), Adult-Gerontology Acute Care (AGACNP), or other relevant specialty.
Physician Assistant (PA):
Graduate of an accredited Physician Assistant program.
Current NCCPA certification.
Licensure and Certification:

SUMMARY:
Under the supervision of the Pathology Manager, Phlebotomy Supervisor or guidance of the Lead Phlebotomist, performs responsibilities associated with obtaining blood and various other patient specimens. Transports specimens and completes basic records. Performs related clerical support activities to interview and instruct patients, enters data into computerized database, and maintains related records and reports.

RESPONSIBILITIES:
Schedules venipunctures or capillary blood punctures according to priority and type of test, such as time studies.
Utilizes computer generated collection labels and references Pathology Handbook as necessary to ensure collection of specimens is in accordance with established procedures, i.e. specimens are collected in proper containers and meet special collection requirements as appropriate. Duties are preformed according to hospital policy and observance of universal precautions.
Verifies and identifies patient following established procedures and proper patient preparation prior to specimen collection, such as fasting and special diet.
Obtain blood samples from patient according to venipuncture or capillary puncture techniques. May collect other types of specimens (throat cultures, etc. May require splitting or centrifuging samples. Alerts nursing staff or supervisor to any deviations from puncture or adverse patient condition.
Performs coagulation tests on patients such as bleeding times.Labels, wraps (including appropriate documentation) and prepares samples/specimens for transportation.
Maintains records on sample/specimen procurement.Performs related clerical activities such as, sorts and files records, receiving patient into clinic/treatment area and as necessary interviews patient to obtain insurance, biographical and medical information to complete registration records.
Provides patient with forms (e.g. hospital or regulatory required forms) for tests, gives instructions regarding any special preparation necessary prior to test or appointment. May contact patient to return and repeat tests. May contact outside agencies/physician’s office to obtain additional patient information to complete patient records.
Enters patient’s data into the department computer system. May identify billing errors and make adjustments to reconcile error(s).
MINIMUM QUALIFICATIONS:
BASIC KNOWLEDGE:
High school diploma or GED required.
Phlebotomy certificate through National Accrediting Agency for Clinical Laboratory Sciences and/or completion of a Phlebotomy certificate program to include formal internship demonstrating 100 successful venipunctures.
In lieu of internship, 6 months of phlebotomy experience may be considered.
In lieu of phlebotomy certification, 5 plus years direct phlebotomy experience may be considered.
Ability to perform venipuncture techniques and capillary puncture on adults, children and infants.
Interpersonal skills to obtain and provide information to patients and to explain procedures effectively in order to allay fears and anxiety.
Demonstrated knowledge and skills necessary to provide care patients with consideration of aging processes, human development stages and cultural patterns in each step of the care process.
Clerical skills to prepare and maintain records and reports, including keyboard skills to enter access information.
Familiarity with blood collector equipment, anticoagulants, special precautions and interference.
Ability to distinguish chromatic colors.
A Phlebotomy certificate required.
WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:
Works with in-patient care environment with considerable walking between units and where some physical effort may be required to deal with patients who are uncooperative.
Some exposure to odors when handling specimens.
Exposure to communicable diseases while administering to patients, however chance of injury is minimized by the adherence to established infection control procedures.
Manual dexterity to perform venipunctures.

Assists with oxygen therapy as directed by the Registered Nurse. Changes E cylinders, attaches regulators and adjusts breathing apparatus. Monitors rate of flow. Reports findings to Registered Nurse.
Sets up cardiac monitoring devices according to procedure. Assists with cardiac monitoring, runs 12 lead electrocardiogram (EKG). Reports findings to Registered Nurse.
Utilizes side rails, restraints and assistive devices when available and necessary.
Records selected treatments and procedures for which she/he is responsible.
Maintains patient and family confidentiality according to established hospital standards.
Serves as an active patient/family advocate.
Supports and reinforces teaching done by the Registered Nurse.

Identifies cardiopulmonary arrest and initiates CPR.

Answers phones and intercom, using phone and intercom etiquette, and notifies appropriate staff.
Utilizes hospital computer systems accurately, to execute orders, retrieve data and generate reports. Reports computer and equipment malfunctions to the help desk.
Accurately uses all existing computer systems (Invision, LifeLinks, MedHost, Bed Board, MAK MAR, Hospital Capacity System (HCS) and/or any new systems necessary to carry out job duties.
Arranges for necessary equipment delivery as needed.
Follows up on diagnostic tests ordered and documents booking in the computer under Date/Time Scheduled Tests including any required preparations and holds trays or makes NPO accordingly.
Orders stationary and office supplies as appropriate. Maintains and stocks the A-Z file.
Faxes completed Authorization for Use of Protected Health Information form to other health care providers; receives faxed information and files in the patient's record. Faxes other forms as needed.
Takes responsibility for educational updates by attending education sessions and accessing the Net Learning website. Meets and maintains competencies measured by annual mandatory competency check and mandatory/safety education.
Reports unsafe situations to the Registered Nurse or to the Clinical Manager.

MINIMUM QUALIFICATIONS:

Entry level credentialing of secretarial and nursing assistant skills.
Current Rhode Island registration as a Certified Nursing Assistant.
Certified in Basic Cardiac Life Support (BLS).

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Receives and accessions specimens for processing, paying attention to the demographic information on the requisition slip and specimen label.
Also checks specimens prior to testing using established procedures for specimen integrity and suitability for testing.
Processes a variety of laboratory samples such as blood, urine, stool, body fluids, etc.
Interpret orders and requisitions to determine aliquots needed for distribution to the laboratory.
Processing can include splitting, pouring, relabeling, disposal or delivery to designated laboratory sections. Responds to pneumatic tube, load automation line as well as receive and sort courier deliveries.
Triage STAT specimens.
Logs specimens into computer system, takes temperatures, records receipt of supplies, rotates stock, loads and unloads samples from instruments and performs limited function (waived) testing.
Enters necessary information on log sheets and/or into the laboratory computer system. Performs a variety of clerical duties including duties associated with coding and billing. Prepares and maintains detailed records of laboratory procedure requests and results; coding outpatient billing forms and generates appropriate billing data.
Answers telephone and responds to general and specific information requests.
Responds to and assists in tracking and resolving lab related problems and concerns.
Ensures all actions are in compliance with confidentiality policies and in accordance with the Standard Operating procedures.
Employs precautionary measures in handing and disposal of blood specimens and materials following all general safety regulations and procedures.
Carries out and documents quality control and maintenance procedures.
Notifies supervisor of any equipment malfunctions or unacceptable quality control results as established for each quality control procedure. Keeps all areas clean while working and performs regularly scheduled cleaning duties as defined in the lab area.

MINIMUM QUALIFICATIONS:
The level of knowledge gained from the completion of High School with some course work in the fields of biology and chemistry.

Transports patients to and from the surgical suite. Reviews preoperative patient charts in order to ensure that all forms are signed and that all tests are completed; informs appropriate nursing personnel of discrepancy.

Assists in general cleaning and maintenance of the Operating Room such as washing equipment, sorting and stocking supplies, cleaning room, washing stretchers and changing light bulbs and the like.
Cleans, dries, packages and seals a variety of supplies and material to be sterilized, such as washing, assembling and wrapping basin sets for sterilization. Occasionally operates steam/water pressure sterilizers in order to clean and sterilize items.
Assembles patient record packs of charge slips, appropriate for specific surgical procedure, to be completed by others.
MINIMUM QUALIFICATIONS:
