- Follows an individualized plan of care that promotes positive immediate and long-term outcomes for the patient/family and reflects the plan of the entire health care team.
- Carries out interventions safely and in compliance with policy to facilitate achievement of expected outcomes as delegated.
- Accurately documents patient response to intervention /therapy based upon expected outcomes.
- Carries out delegated patient care activities and communicates with other members of the healthcare team as evidenced by positive patient outcomes.
- For safety reasons, employees under the age of 18 are not allowed to operate or assist with the operation of power-driven hoisting apparatuses including but not limited to: floor-based vertical powered patient/resident lift devices, ceiling-mounted vertical powered patient/resident lift devices, and powered sit-to-stand patient/resident lift devices (lifting devices).
- For safety reasons, employees under the age of 18 are not allowed to provide care to patients where this is potential exposure to radioactive substances and/or to ionizing radiations.
- Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.
- These statements are intended to describe the essential functions of the job and are not intended to be an exhaustive list of all responsibilities. Skills and duties may vary dependent upon your department or unit. Other duties may be assigned as required.
Required Education and Experience
- Knowledge of general nursing assistant functions
Preferred Education and Experience
- High School Graduate OR
- General Educational Development
- 6 months medical center experience
- Courses in medical terminology
Required Licensure and Certification
- Certified Nurse Aide / Assistant (CNA) - State Department for Aging and Disability Services (KS), Department of Health & Senior Services (MO) - Includes licenses for all other states
- Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA) or must complete BLS certification with in the first two weeks of employment
- Additional certification may be required for certain patient care settings or populations.

- Nonviolent Crisis Intervention - Crisis Prevention Institute (CPI)

Creates the initial electronic health record that serves as the foundation of the patient medical record that is utilized by all members of the healthcare team. Prevents creation of duplicate medical records that can cause treatment safety issues and billing problems. Follows and ensures compliance with the mandate of the organization’s accrediting bodies to use identifiers to positively identify a patient prior to the delivery of patient care to ensure patient safety.
Checks in and registers patients; obtains and verifies complete demographic, guarantor, and insurance information; discusses and collects co-pays and other out-of-pocket patient responsibilities. Obtaining accurate information at the point of registration helps ensure timely payment to the organization and prevents billing issues and patient complaints. PRINCIPAL ACCOUNTABILITIES

- Collaborates in the development and evaluation of patient care. Updates clinical guidelines/protocols/pathways; works autonomously and accepts accountability for outcomes; signs orders in accordance with institution and state board requirements; collaborates in research and teaching projects and applies research to practice.
- Uses interpersonal and verbal communication skills to educate patient/family on care resources; participates in process improvement and multidisciplinary clinical outcome improvement activities utilizing advanced nursing expertise.
- Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
