- Perform thorough patient assessments, identifying critical health issues and implement targeted interventions and patient-family centered care plans to achieve optimal health outcomes. Collaborate and negotiate effectively with clinically complex patients, family and the clinical team while striving to achieve patient and organizational goals regarding care needs, choices, and satisfaction during discharge planning and care transitions.
- Aggregate, analyze, interpret and report data on patient outcomes and resource utilization. Facilitate reporting of utilization monitoring and review activities to relevant committees and stakeholders
- Provide continuity of care and discharge planning services for clinically complex patients compliant with regulatory standards. Offers coordinated, relevant options and services based on assessed needs to ensure patient, families, and the healthcare team are informed and prepared to proceed with accountabilities in a timely manner. Participate in the communication processes to facilitate smooth transitions for patients, families, and staff during patient transfers.
- Provide advanced clinical guidance and mentorship to frontline care management team members fostering a culture of excellence and continuous improvement. Lead initiatives aimed at enhancing care quality, patient safety, and overall healthcare delivery efficiency.
- Advocate for patients and their families to ensure their voices are heard and their needs are met within the healthcare system while optimizing the utilization of hospital resources ensuring cost-effective care delivery and adherence to regulatory guidelines.
- Initiate internal and external referrals to assure timely progression of care and transitions for clinically complex patients. Document discharge planning interventions and utilization review activity according to department and organization standards in a timely manner. Perform and document accurate and timely concurrent and retrospective reviews for clinically complex patients based on approved criteria by department standards.
- Communicate effectively with the healthcare team regarding clinically complex patients. Partner with Social Work and unlicensed support personnel to effectively establish and implement a safe plan of care. Serves as a leader of the multidisciplinary rounds and work closely with clinical team members, hospital departments and ancillary services to identify and resolve barriers to discharge, expedite care delivery to avoid delays in timely service provision, and implement and report on care coordination and discharge planning.
- As an expert in care management of clinically complex patients, collaborate and lead discussions with managers, physicians, medical directors, advisory groups, and treatment teams for issues related to physician practices and best practices for the patient care plans. Refer cases to physician advisors as needed to ensure efficient progression of care, accurate status, and compliance with regulatory guidelines. Maintain knowledge of healthcare regulations, reimbursement issues, impact on length of stay and community-based resources. Deliver CMS regulatory notices within CMS established timeframes, as appropriate based on-site guidelines.
- Develop and maintain productive relationships with community-based agencies, particularly those serving clinically complex patients. Represent Advocate Health in a positive manner, working collaboratively, internally and externally to meet patient and family needs. Collaborate with Advocate Health Ambulatory Care Management and Continuing Health to achieve mutual goals and outcomes.
- Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Understand principles of growth and development over the lifespan and possess the ability to assess data reflective of the patient status and interpret the appropriate information needed to identify each patient’s age-specific needs, and provide the care needed as described in departmental policies and procedures.
- Age-specific information is further developed in the departmental job standards.
Licensure, Registration, and/or Certification Required:
- Registered Nurse License issued by the State in which the team member practices.
- Accredited Care Manager (ACM) certification issued by the American Case Management Association (ACMA) needs to be obtained within 2 years, or
- Care Manager Certified (CMC) certification issued by the National Academy of Certified Care Managers (NACCM) needs to be obtained within 2 years, or
- Nurse Case Management (RN-BC) certification issued by the American Nurses Credentialing Center (ANCC) needs to be obtained within two (2) years.
Education Required:
- Bachelor’s degree in nursing - required to be considered for complex inpatient Care Manager role
Experience Required:
- 3 years of care management experience
Knowledge, Skills & Abilities Required:
- Ability to prioritize and organize work.
- Effective communication skills.
- Utilization of critical thinking and timely decision making.
- Ability to navigate the electronic health record.
- Basic utilization of MS Office products.
- Knowledge of Medicare A and B guidelines.
- Knowledge of managed care program requirements/implications.
- Ability to apply elements of utilization management programs.
Physical Requirements and Working Conditions:
- Must be able to sit up to approximately 50 percent of the workday; stand and walk for the equivalent of several blocks at a time.
- Must lift up to 10 lbs. continuously, up to 20 lbs. frequently, and up to 50 lbs. occasionally.
- Manual dexterity required for operation computer and calculator.
- Visual acuity required for facilitating review of written documents/computer screens, medical records, and to record information accurately.
- Clear verbal communications and hearing acuity required for receiving instructions and converse on standard telephone.
- Functional speech and hearing to allow for effective communication of instructions and conversation over the telephone.
- Exposed to normal office environment; including usual hazards related to operating electrical equipment.
- Operates all equipment necessary to perform the job.

3CD is a 23 bed post-op surgical unit with 15 beds for Tele Patients. 75% of patients are admitted from PACU or as stepdown from Surgical ICU. This is a fast-paced, high acuity surgical unit specializing in Gastroenterology, Urology, Gynecology, and Plastic Surgery populations. This is a great unit for experienced Med-Surg nurses seeking to advance their skills and grow in their practice. It is also a great foundational unit for new graduate nurses who seek to eventually develop into more advanced clinical settings, such as ICU, ED, OR, and NP. We are a strong performing inpatient unit as measured by their Nurse Sensitive Indicators and Patient Experience scores. 3CD team members are committed to excellent clinical practice and patient outcomes. 3CD is a 23 bed post-op surgical unit with 15 beds for Tele Patients. 75% of patients are admitted from PACU or as stepdown from Surgical ICU. This is a fast-paced, high acuity surgical unit specializing in Gastroenterology, Urology, Gynecology, and Plastic Surgery populations. This is a great unit for experienced Med-Surg nurses seeking to advance their skills and grow in their practice. It is also a great foundational unit for new graduate nurses who seek to eventually develop into more advanced clinical settings, such as ICU, ED, OR, and NP. FAST PACED post op surgical unit, caring for patients with Gastroenterology, Urology, Gynecology, and Plastic Surgery populations, abdominal drains, wound VACS’s, PCA’s, epidurals, tube feedings etc. 75% of patients are admitted from PACU or as stepdown from Surgical ICU.
Manages the care of 4-5 patients on day and PM shift and 5-6 patients on the night. 3EF can manage patients on the following drips: insulin, ketamine, dobutamine, heparin, angiomax, etc. This is a great unit for experienced Med-Surg nurses seeking to advance their skills and grow in their practice. - Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
- Coordinates tests and treatments with and between various departments for patients on nursing units. Maintains a working knowledge of functions and services offered by all diagnostic and treatment departments to facilitate the scheduling of tests and treatments. Maintains working knowledge of EHR system and other computer systems for ordering patient and unit supplies and equipment. Processes daily patient charges and/or related procedures through the system.
- Facilitates communications required to support patient care, including the multiple-line telephone, unit/site paging and the hospital information system. Screens incoming telephone calls. Monitors department office equipment and troubleshoots minor problems. Arranges repair of major malfunctions.
- Compiles necessary chart forms for admissions, transfers, discharges and expirations. Obtains appropriate copies of medical records for transfers to nursing homes and other health care facilities. Verifies medical records contain complete and accurate patient demographic information, patient identification and registration in the medical record, including appropriate level of care.
- Establishes and maintains positive customer relations and contributes to a harmonious work environment. Quickly and courteously acknowledges requests, responds to call lights, demonstrates proper phone etiquette and fosters communication between the health care team.

Major Responsibilities:
- Evaluates, diagnoses, treats, and manages acute, critical and chronic patients illness and diseases within both an outpatient clinic and inpatient hospital setting.

- Prescribes, recommends and regulates pharmacologic and non-pharmacologic therapies across multiple care settings, including controlled medications that are warranted by patient's diagnosis or changing medical condition.
- Prepares, dictates and signs patient care plans and summaries of patient care. Maintains records according to established standards and guidelines.
- Provides clinical direction to nursing staff regarding the continued care of patients. Follows through with nursing staff to ensure patient receives recommended care.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. $20.80 - $31.20- Processes and maintains proper care of all surgical instruments and equipment, scopes and powered surgical instruments which includes inspection, evaluation of proper function, cleaning disinfecting and sterilization. Maintains quality control and documentation records of the sterilization process. Distributes designated supplies and equipment to all areas of the medical center.
- Receives soiled and contaminated items in decontamination area. Sorts equipment, rinses as needed, places equipment through sonic tanks and washers, reassembles trays, performs safety checks, hangs and sorts instruments, wraps sets according to set policy, and wraps towels and basins appropriately.
- Sterilizes instruments and equipment using the aerator, gas sterilizer or steam sterilizer and operates glass dryer. Packages items in steri/peel as required. Performs quality assurance testing for all methods of sterilization to ensure sterility of supplies. Hand cleans miscellaneous instruments (Baxter pump, hypothermo units, etc.) and operates the cart cleaner.
- Ensures proper care and handling of high cost instrumentation and equipment to include items requiring special handling, laser instrumentation, orthopedic implants, cardiac assist devices, endoscopic and microscopic instrumentation.
- May build case carts according to procedure and physician preference as specified. File case cart sheets and requisitions, responds to Code 4s with defibrillator and back up supplies, maintains CPR annies, tests autoclaves with the tests according to procedure, and changes graphs as necessary.
- Checks incoming trays and follows up with departments as to missing instruments or parts.
- Takes immediate and appropriate action if a question of sterilizer failure occurs.
- Receives and records orders for supplies as requested by departments. Records accurate product number and assures all forms are correctly completed for proper billing. Marks, dates and stocks equipment, which may include returning to appropriate department on a timely basis.
- Opens and unpacks supply boxes, checking for damaged goods and number of items received. Reports any discrepancies to the supervisor. Performs credits for unused products returned, checks stock levels, copies supply sheets, checks carts for outdated supplies, and performs general cleaning of shelves.
- May performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.

- Participates in the preparatory steps of surgery; assists operating team in gathering supplies, opening room for cases, assists in lifting, transferring, positioning, preparing and draping the patient.
- Anticipates and responds to the needs of the Surgeon, by ensuring all required supplies are available for procedures prior to incision, recognizing individual preferences.
- Provides for a clean, safe and efficient area conducive to optimal patient care. Utilizes aseptic surgical techniques in the preparation of materials, supplies and equipment for surgical procedures (e.g., assistance with gowning and gloving; preparation and draping of the surgical area; provision of a sterile field, instruments, suture and related materials; anticipation of the Surgeons' needs; holding of retractors, observing and reporting possible breaks in aseptic technique; and assisting in maintaining a sterile, dry, unobstructed field).
- Ensures the environmental disinfection, safety and efficiency of the operating room. This includes setting up the room for each case including moving equipment (e.g., tables, cameras, microscopes, fracture table, and instrument pans), operating the various types of equipment they have been trained to use during operating room procedures, and returning equipment post procedure. Checks the working condition of equipment and facilitates having damaged items sent out for repair. Notifies appropriate people of equipment and supply problems.
- Acts as a resource and assists in the training of Surgical Technologists and new Surgical Assistants. Participates in department and facility work teams and assists with special projects as needed.
- May perform specific surgical activities that are validated competencies under the direct supervision of the surgeon (i.e. the surgeon is in the same room working on the same patient). A record of the validated competencies must be recorded in the caregiver’s education file.
- May perform specific validated competencies and surgical activities such as wound closure of deep capsules, muscle, subcutaneous tissue and skin under general supervision of the surgeon (i.e. validated competencies may be delegated by the surgeon who is immediately available and able to furnish immediate assistance and direction but not necessarily present in the room when the activity is performed).
- May perform specific validated competencies and surgical activities such as position and prep of patient on the surgical table (excluding complex fracture table, spinal frame, or head stabilizers/pins/tongs), application of the tourniquet as indicated in preparation of a procedure, securing of drains, application of the dressing, cast or immobilizer at the end of a case and safe transfer from the surgical table to transport cart/bed without supervision of the surgeon (i.e. validated competencies may be delegated by the surgeon and be performed without supervision of the surgeon and with the surgeon not present).
- Provides competency confirmed by a sponsoring surgeon and submits documentation to the medical executive committee a minimum of every two years for approval and/or renewal.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA), and

Major Responsibilities:
- Evaluates, diagnoses, treats, and manages acute, critical and chronic patients illness and diseases within both an outpatient clinic and inpatient hospital setting.

- Assists RN with daily care of patients including bathing, oral hygiene, feeding, activity orders and ambulation which may include use of therapeutic assistive devices, proper positioning, range of motion and special transferring as indicated.
- Measures and documents blood pressures, TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. Documents accurately in Electronic Medical Record (EMR).
- Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately.
- Participates in admission, transfer and discharge process under supervision of RN including room set up, orientation to room, removing linen, and proper management of patient belongings. Ensures patient call light is always in reach of patient and answers call-lights promptly.
- Collects urine, stool, and sputum specimens and performs point-of-care testing as indicated using approved process for accurate specimen labeling and identification. Maintains competency as required.
- Completes various errands as directed including pick-up and delivery of trays, charts, specimens, and reports. Maintains and orders stock for nursing area. Cleans equipment and work areas.
- Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices. Performs tasks related to nutritional needs of patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection.
- Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. Ensures special patient supplies and appliances are provided and maintained such as TED hose and sequential compression devices.
- Under the direction of RN and as determined by site/unit requirements, may be required to remove IVs, provide post mortem care, and bladder scans.
- Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation.
- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Licensure, Registration, and/or Certification Required:
- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 45 days, and

Operational Integration & Performance
- Directs clinical and administrative operations across assigned care locations to ensure consistent execution of service line, divisional, and enterprise priorities.
