The program supports the largest number of deliveries in the Capital region, many of which are high-risk pregnancies and multiple births. The family centered team YOU will work with includes physicians, licensed nurse midwives, dedicated perinatal nurses, social work and spiritual care services, skilled techs, and lactation consultants. What you will do:
Nurses working in Mother/Baby unit have the support of comprehensive orientation developed by a clinical nurse specialist in maternal/child nursing, on-site BLS and neonatal resuscitation classes, as well as on going clinical education. The staff is highly involved in quality improvement activities, program development, recognition activities, and team building. The clinical ladder program provides the opportunity for professional growth through mentoring, opportunities for leadership, and generous educational funding.
Responsibilities:
- The RN is responsible for assessing the needs of each patient to which he or she is assigned
- Planning the nursing care needed to meet those needs, including spiritual, emotional and physiological needs. Once planned, the RN is responsible for seeing that the plan of care is carried out through performance and or leadership of supervised personnel and that the plan of care is evaluated
- Directly accountable to the Nurse Manager for the quality of patient care delivered
- Modifies and delivers care that is specific to the age, growth and development of patients according to the unit scope of services
- When serving in the charge role, he or she is responsible for clinical and leadership functions.
- Responsible for assessing the needs of each patient and planning the nursing care needed to meet those needs

- One year of Med Surg or Maternity
Pay Range:$40.00-$51.10
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.

Position Summary: LPNs (Licensed Practical Nurses) are responsible for medications, treatments, and staff oversight under the direction of the RN. In this position you will bring trust, comfort, and empathy to all resident interactions and make a positive difference in the lives of our residents. Job Specific Competencies
Principal Responsibilities:

LPNs provide direct and indirect care under the direction of an RN including procedures, treatments, and related duties as assigned and within the NYS Scope of Practice for an LPN.
- Resident support — Deliver compassionate, person-centered care while fostering a sense of dignity and independence for each resident.
e Medication administration — Conduct daily medication pass with accuracy and care in accordance with physician orders and facility guidelines.
Shift leadership — provide leadership for the care team during your shift, including guidance and support.
e Team collaboration — work closely with resident assistants, other nurses, and management in a cooperative, respectful environment.
- Documentation — complete required documentation promptly and accurately to support resident care.
- Consult with the RN on any changes in a care plan or schedule and communicate with patients and their families about their care plan.
- Attends and actively participates in in-servicing, orienting and educational activities.
Education Requirements:
- Must be a high school graduate.
- Must graduate from a nursing education program acceptable to the New York State Education Department (NYSED).
- Must be licensed with current registration in New York.
Must be able to:
- Stand, walk, and lift for long periods of time
- Read and write using medical terminology
- Working knowledge of computers including electronic medication administration records, processing of orders, checking email, and preforming required in servicing, along with use of other required applications Have the visual and auditory acuity adequate to perform observations and patient care
- Maintain composure and positivity in a high stress environment
- Promote physical, cognitive, and psychosocial well-being of each resident Work with residents in a highly personal manner during stages of acute and chronic illness

What you will do:
The Registered Professional Nurse has the responsibility and accountability to utilize the nursing process to diagnose and treat human responses to actual or potential problems of individuals or groups. Obtains patient consent for procedures as directed by provider.
Performs pre-visit planning and reviews quality metrics.
Retrieves telephonic clinical information from patients who call into the office.
Monitors task list and completes tasks assigned by provider in a timely manner based on urgency.
Educates patients regarding medication, testing procedures and home care techniques.
Ensure proper labeling, handling and documentation for patient specimens.
Follow up with patient regarding test results based on advice given by provider.
Maintains a clean and safe work environment including disinfecting patient care areas and equipment.
In conjunction with other nursing colleagues, maintains the medical supply cabinet and drug cabinet.

Upon enrollment, a Care Coordinator collects information via a comprehensive assessment that will support developing a comprehensive plan of care with the member. The assessment will include their medical and behavioral health needs, substance abuse, activities of daily living, their socio-economic and housing status, and provides an opportunity to understand their social determinants of health. Additional responsibilities include conducting developing a person-centered care plan that coordinates and integrates a comprehensive array of a member’s needs and services in collaboration with an interdisciplinary care team. The aim is to assist the member in reaching optimal wellness and recovery.
Education & Experience Requirements:
- Bachelor's Degree in Human Services, or related field, required.
- Two years or more working with individuals with chronic medical conditions and behavioral health issues. Five years experienced strongly preferred.

Previous direct patient care experience is desirable.Employees of St. Peter’s Health Partner’s enjoy an excellent benefit program and salaries are based on your education and experience. High School Diploma or GED Required, Patient Care experience a plus!
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
PCT Pay Range:$17.25 - $25.00
