Job duties include the following:

Therapy Works provides indoor treatment space as well as an expansive outdoor treatment area with a variety of therapeutic activities. There is an optional weekly team consultation to support therapists in proving the best care possible for our patients. Therapy Works provides flexibility to meet your and/or your families needs via therapists ability to make their own schedule.
Candidate must have a degree in occupational therapy as well as MN licensure and NBCOT certification.

*Position Purpose*
The purpose of the veterinary technician is to assist the veterinarian team and other support staff to maintain the highest quality of care given to all patients at Cannon Valley Veterinary Clinic. This is done by improving the veterinarian’s efficiency: specifically relieving them of technical work and administrative duties associated with each patient visit.
*Job Responsibilities – *Items marked with an \* are considered essential
* Under the supervision of a Technician 4:
* Collect blood, stool, and urine samples for testing\*
* Perform standard laboratory tests, such as urinalysis, ear cytology, fecal analysis, and blood counts \*
* Take and process x-rays\*
* Send out Idexx samples
* Prepare items and patients for surgery\*
* Assist with recovery of patients\*
* Perform all necessary post-surgery cleaning\*
* Assist in surgery as Doctor or technician feels fit\*
* Ask probing questions about the reason for the visit\*
* Record case history, weight, temperature and other details\*
* Restrain and stabilize animals during examination and treatment\*
* Trim nails, remove sutures and perform other routine procedures\*
* *Draw up and administer medications, vaccines, and treatments prescribed by a veterinarian*\*
* Provide basic education on animal care to clients\*
* *Teach clients how to administer medicines at home*\*
* Be proactive during exam; anticipating procedures or tests that will be done\*
* Be able to triage patients for basic situations depending on skill and comfort level\*

Responsibilities:
Conduct intakes and assessments to gather patient information, evaluate mental health diagnoses, and develop treatment plans with ongoing documentation.
Facilitate individual and group therapy sessions for patients in an outpatient addiction setting, implementing personalized plans with measurable outcomes and providing therapy, case management, and co-occurring mental health and substance use support.
Utilize creativity in interventions to help clients achieve their goals while providing excellent customer service and collaborating with the treatment team to develop and implement individualized care plans that support the community mission.
Active participation and responsibility in building, growing, and maintaining caseload to meet agreed upon weekly billable expectations.
Participate in clinical consultations, staff meetings, and trainings to ensure continuity of care.
Maintain accurate and compliant patient records in accordance with 245G and 1115 Demonstration Waiver requirements.
Maintain accurate and timely electronic health records (EHR) by completing session documentation, billing, prior authorizations, and insurance-related tasks within 24 hours while ensuring compliance with policies and procedures.
Requirements:
Master’s degree in one of the behavioral sciences or related fields from an accredited college or university.
Candidates will have clinical licensure or be on licensure track for:
MN: LMFT, LPCC, LICSW, LP
Applicants with a dual licensure of a LADC are preferred but not required.
If LADC licensure applies, Licensed Drug & Alcohol Counselor (LADC) must have an active and unrestricted license in Minnesota or an ADC-T.
Must have identified substance use and/or addiction competencies
Experience completing diagnostic assessments, treatment plans and clinical case notes with the ability to demonstrate and model stable, appropriate boundaries with clients.
Proficient in the use of Office 365 and EHR systems.
Compensation: This is an exempt position. Total earnings for this position will range from $75,000 - $80,000 annually for fully licensed providers. Total earnings for this position will range from $68,000 - $70,000 for pre-licensed providers.
*Actual compensation may be determined by various factors such as licensure, experience, skillset, internal equity, schedule and/or location. Estimates are factored on a 1.0 FTE.

This Position Description (PD) defines the program, training, and technical support services required for a board-certified therapeutic medical physicist (TMP). This is a locums position. The start date is targeted for September 2025 (a start in October is also a possibility). The work is on-site and in-person, and your presence in clinic is expected during normal business hours: 8: 00 AM - 4:30 PM, Monday though Friday.
The work site is at the Minneapolis VAMC. The clinic address is: 1 Veterans Dr, Minneapolis, MN 55417.
*POSITION DUTIES:*
The medical physicist shall perform weekly chart checks, plan approvals, QA checks, and IMRT. Work is located at the Minneapolis VAMC Clinic which hosts 2 TrueBeam Linacs, 2 physicists, 2 dosimetrists, 6 radiation therapists, 2 physicians, and 15 to 20 patients per day total (fairly low workload). The physicist may perform work after normal business hours to complete calibrations and measurements once patient treatments are finished.
The physicist must be board-certified in therapeutic medical physics by the American Board of Radiology (ABR) and must also have the following minimum years of experience on the following procedures, equipment, and software listed below under the “Relevant Experience” section.
*Relevant Experience:*
* Two (2) years experience in a Varian-based clinic (TrueBeam Linac).
* Two (2) years of experience: Documentation detailing experience in performing QA procedures.
* One (1) year of experience: Documentation detailing Therapeutic Medical Physics in a healthcare system that is ACR or ASTRO/APEX certified.
* One (1) year of experience: Brain Lab Accessories following standards of TG-142.

About the job:
Multi-skilled Technician under the supervision of an RN, performs basic patient care activities. Actively demonstrates the organization’s mission and core values and conducts oneself at all times in a manner consistent with these values. Knows and adheres to all laws and regulations pertaining to patient health, safety and medical information. (For patient care providers) Performs basic patient care activities as delegated and supervised by an RN, including taking blood glucose measurements, recording intake and output, assisting patients with personal care and activities of daily living including bathing, grooming, and eating. Performs ordered EKGs according to policy and per documented training and competency assessment. Performs Phlebotomy duties according to policy and per documented training and competency assessment. Assists with keeping patient rooms stocked, clean, and orderly. Attends required in-services, workshops, retreats, staff meetings. Participates in and fosters a performance improvement approach that includes both intradepartmental and interdepartmental activities. Performs other duties consistent with purpose of job as directed.
Education: The level of knowledge ordinarily acquired through the completion of high school level courses.
Ability to read, write, and spell English and perform simple arithmetic calculations.
Licensure:
• Successful completion of EKG and Phlebotomy course upon hire or transfer.
• Successful completion of the initial clinical competency requirements in order to perform independently.
• CPR certification on hire or able to attain within 60 days.
Experience: Previous patient care experience and/or technical education in health-related field preferred.

- Responsible for the daily management of an assigned patient caseload, ensuring timely care and documentation to meet the patent needs/services as prescribed in the established plan of care and per MD orders
- Collects data pertinent to the healthcare consumer's health or the situation from patient assessments in the home setting, telephonic contacts and/or related EMR information available.
- Analyzes the data to determine the issues and available resources to seek effective resolution. Identifies patients at high risk for re-hospitalization, falls and/or high utilization of resources that require additional intervention and case conferencing to ensure appropriate care and frequency of services mitigates risk and ensure high quality outcomes
- Prioritizes multi-issue, situations appropriate to the criticality of patient health and welfare, patient/staff safety and impact to effective, timely services
- Identifies barriers to situation/complaint resolution (i.e. language, literacy, psychological, etc) and follows-up with the appropriate resources or leadership to address timely and effectively
- Develops and provides oversight of a plan of care that is effectively communicated with the parties involved and supports quality patient outcomes and patient satisfaction.
- Implements the identified plan, and directs the care coordination of a multidisciplinary clinical team approach, supervision of LPNs and/or HHAs involved in the care of the assigned patient caseload to promote health and safe outcomes.
- Performs ordered treatments, therapies, and/or procedures in accordance with State Nurse Practice Act and Home Health Conditions of Participation, JCAHO and agency policy and procedures.
- Evaluates progress towards attainment of patient centered goals.
Physical Requirements
Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling. Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies. Must speak English in good, understandable terms. Intact sense of sight, hearing, smell, touch. Finger dexterity. Critical thinking and ability to concentrate. Must be able to respond quickly to changes in patient conditions. Physical Abilities Testing required.
Education, Experience and Certifications
Graduate from an accredited School of Nursing. BSN preferred. BLS required per policy guidelines. Current RN license or temporary license as a Registered Nurse Petitioner in the state in which you work and reside or; if declaring a National License Compact (NLC) state as your primary state of residency, meet the licensure requirements in your home state; or for Non-National License Compact states, current RN license or temporary license as a Registered Nurse Petitioner required in the state where the RN works. Additional education, training, certifications, or experience may be required within the department by the nurse leader.

- Remains and performs ordered treatments, therapies and/or procedures in accordance with State Physical Therapy Practice Act and Home Health conditions of Participation, JCAHO, Medicare Guidelines, OASIS and agency policy and procedures.
MINIMUM JOB REQUIREMENTS
Education
Doctor of Physical Therapy (DPT) degree required. This aligns with current professional entry standards, as the DPT has been the recognized entry-level credential for all new graduates since 2016; prior bachelor’s or master’s-level degrees may be accepted only where consistent with applicable licensure requirements. Maintains continuing education requirements as mandated by applicable state professional standards.
Certification / Registration / License
Physical Therapy License in the State of applicable state or South Carolina must be licensed in the state upon which you work. Basic Life Support for Healthcare Provider certification from the American Heart Association required.
Current state driver's license and auto insurance at all times. Personal vehicles must be well maintained and in safe driving conditions.

Ability to work flexible schedule to meet patient's needs. Personal Protective Equipment such as gloves, goggles, gowns, and masks are sometimes required due to possible exposure to hazardous chemicals or blood and body fluids.
PREFERRED JOB REQUIREMENTS
Work Experience
1-year experience as a licensed PT in a clinical care setting or home health preferred.
Pediatric setting – 1 year pediatric experience within the past 5 years preferred or education and training may be provided by Advocate Health At Home.

Position Summary:
- A Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served.
