I. Performs a variety of complex administrative duties for patients in need of routine and/or urgent appointments, medical procedures, tests, and associated ancillary services in an ambulatory in/outpatient setting. Assess patients’ needs, including but not limited to, financial counseling, interpreter services, social services and refers to appropriate person or area. Alerts providers to emergent patient care needsII. Major Responsibilities:
1. Greets visitors and/or patients for scheduled and/or urgent care appointments and procedures.
2. Confirms and verifies patient demographic and insurance information.
3. May collect co-payments from patients upon arrival.
4. Obtains signatures of consent from patient/guardian for treatment authorization and insurance/billing information.
5. Assess patients’ needs, including but not limited to, financial counseling, interpreter services, social services and refers to appropriate person or area.
6. Receives and directs phone calls.
7. Connects the patient’s call to the provider or responds to the patient and takes messages as directed.
8. Schedules urgent care appointments as needed and directed by clinicians.
9. Schedules patients for treatment by multiple providers and treatment areas, and arranges a variety of associated tests and procedures according to established guidelines and specific criteria.
10. Prioritizes appointments in a manner that fosters optimum patient care, efficient utilization of clinical staff, as well as resources.
11. Ensures tests and procedural prep information is provided to patients verbally, mail, and/or by the patient portal as necessary.
12. Collaborates with insurers to obtain patients’ prior-authorizations for procedures and tests as needed.
13. Follows guidelines established by insurers to ensure that pre-authorization, pre-certification, and clinician referrals for treatment are obtained prior to patient visits.
14. Complies with referral management regulations.
15. Verifies eligibility for procedures or tests from various health care institutions.
16. Follows up to correct discrepancies.
17. May facilitate patient prescription renewal matters, (via telephone, fax, and email), within scope of authority as directed by clinicians.
18. Preps the patient’s information for clinicians for scheduled patient visits as needed.
19. Scrubs Patient Encounter information and submits electronically.
20. Reviews and audits billing discrepancy reports and researches errors for resolution as directed by office or clinical management.
21. Maintains accurate and timely records, logs, charges, files, and other related information as required.
22. Performs a variety of related administrative and clerical duties, such as retrieving files distributing mail and other records, faxing, collating, data entry, and relaying messages to clinicians, residents and staff.
23. May prepare special reports or spreadsheets for clinicians as requested.
24. Collects co-payments from patients for visits, maintains records or makes daily cash deposits from patient visits.
25. Calculates daily totals of co-payments received for submission to the second tier for co-payment reconciliation.
26. Composes, or selects standard form letters for clinician’s response to routine inquiries and procedures, such as back-to-school authorizations.

Provide nursing care for a vulnerable adult population. Performs nursing assessment, treatment, education and health care service coordination to individuals and families. The LPN collaborates with all components of the team and other health care agencies to ensure coordinated, comprehensive services. Services will be provided at Community Healthlink and outreach locations.
I. Major Responsibilities:
1. Assesses the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, medication administration and documentation.
2. Provides appropriate health care and coordinates referrals as prescribed by providers.
3. Maintains documentation required for all primary care clients receiving in-home nursing services.
4. Promotes education and adherence to prescribed treatment protocols.
5. Communicates with health care facilities when clients are admitted and plans for discharge to ensure continuity of care.
6. Facilitates wellness education to individuals and offer groups with health and wellness focus.
7. Records all client information within electronic medical record system daily, received by outside healthcare agencies.

Performs a variety of complex administrative duties for patients in need of routine and/or urgent appointments, medical procedures, tests, and associated ancillary services in an ambulatory in/outpatient setting. Under the direct supervision of a pharmacist, this position is responsible for the preparation, dispensing, and delivery of medications for patients. Completes other tasks associated with drug distribution processes, automated dispensing systems, packaging, IV systems, training, and scheduling.I. Major Responsibilities:
1. Answers telephone calls and triages requests from customers to the pharmacist staff.
2. Triages window requests from customers.
3. Delivers medications to the patient care areas and picks up copies of physician orders, return of medications, etc. Completes scheduled rounds on patient care areas.
4. Assists with inventory management tasks which includes, but is not limited to, ensuring adequate inventory of supplies and materials, ordering, receiving and storing non-controlled medications from the wholesaler on a daily basis, storing medications in designated areas in the central pharmacy, IV room and anesthesia prep area, ensuring bin labels are visible and clear, segregating medications to appropriate storage bins, etc., and checking for expiration dating on medications on a consistent basis to assure medications do not leave pharmacy with short dating.
5. Accurately packages unit dose medications with appropriate labeling information as outlined in policy and procedures to support Pharmacy distribution process including Pre-packing bulk medications and compounding ointments, creams, oral solutions, and other medications as needed.
6. Under the supervision of a pharmacist, completes all assigned drug distribution tasks assigned.
7. Under the supervision of a pharmacist, completes all necessary tasks associated with day- to-day operations of Pyxis distribution system including refilling medications, loading new medications, inventory maintenance, expiration dating, returning bin procedures, troubleshooting for nursing and pharmacy, customer service resolution for nursing and pharmacy, and controlled substance discrepancy resolution assistance for the pharmacist staff as needed.
8. Assists the management team with inventory optimization of Pyxis on an ongoing basis.
9. Identifies quality improvements and customer service improvements with Pyxis automation.
10. Completes other distribution tasks as assigned by pharmacist.
11. Trains colleagues and technical staff as needed.
12. Under the supervision of a pharmacist, accurately completes all daily assignments for IV room distribution following USP 797 practice standards and pharmacy policy and procedures.
13. Under the supervision of a pharmacist, completes all necessary tasks associated with day- to-day operations of IV Room distribution systems including patient specific IV preparation – daily batch preparation, batch IV production for pharmacy prepared products, small volume parenterals, large volume parenterals, IV syringes, IV infusions, TPNs, and other patient specific IV preparation as needed.
14. Completes quality control tasks, e.g., hood cleaning documentation, temperature logs, daily cleaning of IV room, etc., to comply with USP 797 practice standards.
15. Complies with all internal pharmacy documentation requirements as outlined in policy and procedures.
16. Completes TPN production following internal policy and procedures with automated pumps and complies with all internal documentation requirements for preparing and dispensing.
17. Completes chemotherapy production following internal policy and procedures.
18. Completes IV distribution tasks as assigned by the pharmacist.
19. Complete monthly nursing unit inspections as needed following pharmacy policy and procedures.
20. Identifies safety, quality and workflow improvements as noted during the monthly nursing unit inspections.
21. Acts as a liaison for pharmacy while delivering medications to the patient care areas.
22. Troubleshoots drug distribution issues for nursing to ensure optimal patient care and minimal delays in care.

The role of the Patient Access Scheduling Agent is the front door for patients to secure access to the right care, at the right time, with the right provider. The Patient Access Scheduling agent performs a variety of complex scheduling activities that ensures patients get access to the care they need by providing exceptional patient service.I. 1. Handles incoming phone calls from patients, families and referring providers to schedule patient appointments. Scheduling scope includes multiple providers across multiple clinics/departments, practices across multiple campuses.

Performs secretarial and administrative support services for assigned physicians/faculty/ medical service line chiefs where the work requires a thorough knowledge of medical office procedures and UMMHC business and academic administrative practices. Duties involve interfaces with faculty; medical staff, patients, and various UMMHC administrative management and staff, which require considerable tact and diplomacy. 1. May perform duties to coordinate medical office activities and staff as assigned, to include training, and scheduling the work of assigned employees, recommending processing improvements and implementing new procedures, and monitoring employee performance.
2. Transcribes medical reports for physicians from dictation. Produces correspondence, reports, memos, etc. from dictation drafts, models, etc. Proof reads/edits manuscripts, grant applications, academic and professional papers, and other material for appropriate grammar, spelling, format and style.
3. May attend meetings in capacity of recording secretary. Distributes meeting agenda and minutes.
4. May serve as coordinator of divisional residency, fellowship, clerkship, and/or graduate or undergraduate education programs.
5. Composes, or selects standard form letters for physicians’ response to routine inquiries and procedures, such as back-to-work authorizations.
6. Arranges meetings and activities for faculty for the most efficient use of available time. Makes travel arrangements for trips, conferences, etc.
7. Schedules and coordinates patient visits, medical procedures for both inpatients and outpatients with admitting, the OR, testing centers, pre-admission, etc.
8. Coordinates internal and external meetings.
9. Greets visitors and/or patients. Ascertains their needs, and provides standard information as required. Fosters quality customer service.
10. Receives and addresses patient-care related phone calls. Provides routine information on procedures and standard policies, refers matters to appropriate person within department.
11. Identifies problem related priorities, and responds to emergency need to interrupt or contact physician and/or clinicians, within physician’s guidelines.
12. Issues and verifies patients’ managed care referrals.
13. Registers patients for each patient visit, utilizing computer-based patient registration/ scheduling system.
14. Verifies patient’s insurance coverage and eligibility.
15. Escorts patients to examination rooms and chaperones patients as required.
16. Collects co-payments from patients for visits, maintains records, and makes daily cash deposit.
17. May perform on-site charge entry processes, utilizing computer-based systems.
18. Batches daily encounter forms and relays/submits forms/information to the billing office.
19. May processes standard and non-standard business office administrative paperwork, such as purchase requisitions, employment requisitions, employee time sheets, expense vouchers, etc. Follows-up with personnel outside the medical office and UMMHC to expedite timely action and alleviate, or report delays as appropriate.
20. Provides guidance to departmental personnel in medical office and UMMHC administrative policies and procedures.
21. May provide work guidance to office clerical staff.
22. Sets up and maintains confidential, sensitive files and records for multiple functions, academic materials, medical records, etc. ensuring efficient retrieval of information. Coordinates retrieval of medical records.
23. Ensures adequate inventory of office supplies and basic maintenance of office equipment.
24. May coordinate basic set up and maintenance of office personal computers and associated equipment. Ensures timely service.
25. Participates in performance improvement initiatives and demonstrates the use of quality improvement in daily operations.
26. Ensures compliance with health and satiety requirements and with regulatory agencies such as DPH, etc.

Position Purpose:
The PCA, under the direction of a registered nurse, is responsible for direct patient care considering age specific, developmental, cultural and spiritual needs. PRIMARY DUTIES AND RESPONSIBILITIES:
- Responsible for managing the patient onboarding, initial activities and refill activities associated with assigned patients.
- Provides daily status updates to the appropriate provider offices and teams for all new orders related to their assigned disease or satellite.
- Evaluates new Epic orders from assigned groups and triages to OAT (oral anticancer teaching) team and releases to appropriate pharmacies as appropriate.

- Responsible to coordinate with patient and prescriber office, as necessary for filling, refilling and/or shipment of medication, and signature delivery requirements.
- PCC will communicate with the patient, or prescriber, drug storage and handling guidelines.
- Promotes drug compliance/persistency and compliance to REMS requirements with a strict adherence of the use of pharmacy management assessments, templates, data collection, reporting, recording and call guides.
- Maintains current notes and documentation related to patient's drug therapy and pharmacy care plan; flags issues that need to be addressed accurately in TherigySTM.
- Avoids the use of slang, and abbreviations with system entries, unless on approved abbreviation listing for the company.
- Records the REMS authorizations accurately and timely.
- Reports all adverse events utilizing the appropriate internal or external form and escalates any active clinical issue to either a telephonic nurse or pharmacist.
- Establishes and maintains strong customer relationships with physician practices to streamline communication to the patient, payers, pharmacist, nurse, lab, and other healthcare related parties.
- Participates in administration of patient's pharmacy care plan as developed by the pharmacist or nurse.
- Troubleshoots and resolves patient and payer issues in a timely fashion, seeking clarification from either Benefit Verification or Reimbursement team, in order to provide accurate and reliable information to a patient, patient representative or prescriber.
- Embraces a continuous quality improvement customer service approach by pro-actively identifying areas of improvement and communicating those ideas to the healthcare team.

UMass Memorial Medical Group seeks diagnostic Cardiothoracic Chest Imager to join our growing team. Our competitive compensation and benefits package places us as your top choice in the region.
Our cardiovascular imaging program performs approximately 1,500 cardiac MR examinations and approximately 3,000 coronary and cardiac CT examinations per year. The Home Health & Hospice RN (HHHRN) provides care to patients who are in need of care in their homes. Home health & hospice nursing practice considers the needs of the patient, their family and/or the patient’s caregivers. The HHHRN provides care to patients across the lifespan, from prenatal to post-death periods. Home health nursing takes a holistic approach to personal health practices when treating diseases or disabilities. I. Major Responsibilities:
1. Conducts diagnostic evaluations of various communication disorders in both children and adults. This may include, but is not limited to, language disorders, articulation deficits, auditory and auditory-perceptual deficits, voice disorders, stuttering and neurological disorders such as congential, developmental, or acquired. Assessment may include consideration of the physical status of the oral and vocal mechanisms, breathing patterns, vocabulary, language structure, articulation, voice quality, fluency, auditory processing, language integration, written language and such other parameters as may be deemed appropriate. Analyzes client’s data and interprets test results to physicians.
2. Provides remedial treatment in both individual and group settings for children and adults with the above communication and swallowing disorders, utilizing clinical techniques appropriate to each type of disorder. Uses complex decision making to analyze each patient’s problems and develop treatment interventions to maximize function in daily living activities.
3. Recommends, designs, and/or implements augmentative methods of communication such as communication boards and mechanical devices when oral and/or aural communication is not possible. This is done in conjunction with the patient, family and/or others.
4. Provides information about the communication disorders to patients, families and others.
5. Designs and establishes home programs and other activities which may be used for carryover or reinforcement of the therapy program.
6. Maintains records and reports necessary to the conduct of treatment. This may include, but is not limited to diagnostic evaluation reports, daily treatment notes, monthly physician renewals as appropriate, progress reports as appropriate or requested and losing summaries of treatment.
7. Confers with other individuals and/or agencies involved with the patient’s program. This may include physicians and other related professions, school personnel, social service agencies and the like.

9. Phlebotomy in certain locations.
10. Performs other selected unit specific procedures such as battery changes for telemetry units, checking status of unit equipment, cleaning equipment between patients and proper storage of supplies.
11. Assists RN/Licensed Independent Practitioner with treatments and procedures by preparing patient; obtaining and setting up equipment and supplies, such as intravenous and hemodynamic lines, and utilize sterile technique when indicated. Help in positioning the patient and providing the patient with emotional support. Clean up equipment and supplies following the treatment and/or procedure.
12. Observes, measures, records oral food and fluid intake. Sets up full strength tube feeding solutions and refills tube feeding bags as directed. Empties and records output, as approved, from a variety of sources, e.g., Foley catheter, ostomy appliances and drainage systems. Performs ostomy care on established ostomate patients as directed.
13. Demonstrates sensitivity and responsiveness to patients’ needs by acting as a source of information and approaches all interactions with the intention of building positive rapport and relationships.
14. Provides respectful, culturally and age-appropriate communication with patients, staff, family, visitors and others.
15. Replenishes patient and unit supplies and equipment.
16. Requests assistance from identified resources when more direction is needed.
