Responsible for the planning, delivery and management of direct patient care utilizing the nursing process and adhering to the standards of nursing practice.I. Major Responsibilities:
1. Utilizes the Nursing Process for the delivery and management of patient’s care from Admission to Discharge, appropriate to the developmental age of the patient population.
2. Performs patient assessment/reassessment according to standard procedure.
3. Develops and coordinates individual plan of care including discharge planning.
4. Directs, supervises, coordinates and evaluate nursing care within assigned clinical setting.
5. Delegates appropriately to others in accordance with job description and competency level.
6. Documents patient care according to standard procedure.
7. Develops, implements/coordinates and documents Education Plan for patient/family/ significant other.
8. Ensures quality and continuity of care through collaboration with other health care providers.
9. Administers medications, performs treatments, therapies and operates equipment according to policy and procedure. Assists physician with treatment and procedures and special tests as required or directed.
10. Participates in and supports Nursing Governance activities.
11. Assumes responsibility for professional growth. Assists in education and development of staff.
12. Participates in performance improvement initiatives and demonstrate the use of quality improvement in daily operations.
Standard Staffing Level Responsibilities:
1. Complies with established departmental policies, procedures and objectives.
2. Attends variety of meetings, conferences, seminars as required or directed.
3. Demonstrates use of Quality Improvement in daily operations.
4. Complies with all health and safety regulations and requirements.
5. Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.
6. Maintains, regular, reliable, and predictable attendance.
7. Performs other similar and related duties as required or directed.

II. Position Qualifications:
License/Certification/Education:
Required:
1. Current Massachusetts Nursing licensure and registration.
2. BLS is a requirement for every RN unless advance cardiac life support (ACLS) certification is current, in addition to any unit specific certifications and competencies.
Preferred:
1. Bachelor of Science degree in Nursing highly preferred.
Experience/Skills:
Required:
1. Thorough knowledge of the discipline of Nursing.
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.
III. Physical Demands and Environmental Conditions:
Work is considered medium. Position requires work indoors in a patient care environment.

Alternating 2 & 3 -12 hour night shifts per week
7pm-7:30am
Every 3rd weekend and holiday

I. Major Responsibilities:

8. Performs EKG’s, performs Point of Care (POC) tests and bladder scanning according to hospital standards. Collects, labels and prepares specimens for transport to lab.
9. Phlebotomy in certain locations.
10. Performs other selected unit specific procedures such as battery changes for telemetry units, checking status of unit equipment, cleaning equipment between patients and proper storage of supplies.
11. Assists RN/Licensed Independent Practitioner with treatments and procedures by preparing patient; obtaining and setting up equipment and supplies, such as intravenous and hemodynamic lines, and utilize sterile technique when indicated. Help in positioning the patient and providing the patient with emotional support. Clean up equipment and supplies following the treatment and/or procedure.
12. Observes, measures, records oral food and fluid intake. Sets up full strength tube feeding solutions and refills tube feeding bags as directed. Empties and records output, as approved, from a variety of sources, e.g., Foley catheter, ostomy appliances and drainage systems. Performs ostomy care on established ostomate patients as directed.
13. Demonstrates sensitivity and responsiveness to patients’ needs by acting as a source of information and approaches all interactions with the intention of building positive rapport and relationships.
14. Provides respectful, culturally and age-appropriate communication with patients, staff, family, visitors and others.
15. Replenishes patient and unit supplies and equipment.
16. Requests assistance from identified resources when more direction is needed.
17. Keeps current with hospital and unit changes by reading communication boards and/or books, bulletin boards, posted notices and reads and responds to UMMHC e mails on a regular basis.

Position Purpose:
The Registered Nurse Provides quality patient centered care considering age specific, developmental, cultural, and spiritual needs through competent clinical practice and application of the nursing process. They consistently integrate concepts of relationship-based care into practice and serves as a clinical and educational resource to others on the unit.
The Inpatient Rehab units at Chelsea Hospital care for patients requiring comprehensive inpatient rehabilitation after traumatic brain injuries, cerebral vascular accidents, amputations, debility, poly trauma or spinal cord injuries. Inpatient Rehab is a collaborative program between Chelsea Hospital and University of Michigan Health, staffed by a team of award-winning U-M Health rehab physicians and Chelsea Hospital nurses, therapists, and specialists.
Our Inpatient Rehabilitation unit has recently expanded to a 40-bed unit and is now accepting higher acuity patients. As part of our commitment to excellence in patient care, we are proud to be participating in the Spinal Cord Injury (SCI) Model System.
Shift Available: Night shift - 7pm-7:30am
Status Available: Part time, benefit eligible (48 hours per pay period – every two weeks)

PRIMARY DUTIES AND RESPONSIBILITIES:
With supervision, the Financial Clearance Specialist I:
- Reviews and monitors new patient daily appointment record and identifies and prioritizes patients with insurance plans that require prior authorization.
- Verifies insurance benefits, utilizing automated eligibility systems, payer portals, and/or telephone communication.
- Accurately and courteously communicates financial clearance process to patients, including a high-level benefits explanation, network status of DFCI for their plan, and authorization/ referral requirements; assists patients with resolving insurance related issues prior to initiating treatment.
- Reprioritizes work to respond to requests for clearance for clinically urgent patients and produces high quality work under time constraints.
- Monitors established patient work queues to ensure required authorization and/or referrals are obtained prior to service being rendered and within payer guidelines.
- Works with patient and/or Primary Care Physicians to complete payer-specific prior authorization request forms and applies appropriate guidelines to prior authorization requests.
- Responds to health plan-reviewed prior authorization requests that do not meet initial policy criteria. Works with the health plan to resolve issues and/or coordinate appropriate provider-to-health plan interventions (e.g., peer-to-peer discussions, letters of medical necessity, provider-initiated appeals, etc.).
- Effectively communicates with appropriate DFCI staff (e.g., New Patient Representatives, New Patient Coordinators, Established Patient Schedulers, clinical team) regarding financial clearance status.
- Maintains accurate, thorough, and complete records related to financial clearance activities to support post-claim denial workflows.
- Completes assigned work queues, projects, and other duties as directed.

I. Performs a variety of complex administrative duties for patients in need of routine and/or urgent appointments, medical procedures, tests, and associated ancillary services in an ambulatory in/outpatient setting. 1. Greets visitors and/or patients for scheduled and/or urgent care appointments and procedures.

12. Collaborates with insurers to obtain patients’ prior-authorizations for procedures and tests as needed.
13. Follows guidelines established by insurers to ensure that pre-authorization, pre-certification, and clinician referrals for treatment are obtained prior to patient visits.
14. Complies with referral management regulations.
15. Verifies eligibility for procedures or tests from various health care institutions.
16. Follows up to correct discrepancies.
17. May facilitate patient prescription renewal matters, (via telephone, fax, and email), within scope of authority as directed by clinicians.

20. Reviews and audits billing discrepancy reports and researches errors for resolution as directed by office or clinical management.

Collects blood specimens by venipuncture and capillary from patients. Performs EKG, pulse oximetry, and point-of-care testing on inpatients and outpatients as required.Major Responsibilities:
- Performs venipuncture and capillary phlebotomy procedures on patients using appropriate specimen collection equipment and techniques.
- Review requisition and orders from physicians to ensure appropriate tests are drawn and tests meet guidelines for Medical Necessity. Prepares Advance Beneficiary Notice (ABN) for tests that do not meet Medical Necessity guidelines and obtains appropriate signatures from patient and self.

- Follows established Blood Bank protocols perform tests including the most complex for an assigned area and prepares component blood products for transfusion. The role of the Patient Access Scheduling Agent is the front door for patients to secure access to the right care, at the right time, with the right provider. The Patient Access Scheduling agent performs a variety of complex scheduling activities that ensures patients get access to the care they need by providing exceptional patient service.I. Major Responsibilities:
1. Handles incoming phone calls from patients, families and referring providers to schedule patient appointments. Scheduling scope includes multiple providers across multiple clinics/departments, practices across multiple campuses.

3. - Under the direction of the scheduling operations management team, provides appointment scheduling ranging in complexity from one physician appointment to multiple care provider appointments in accordance with department scheduling guidelines for existing patients
- Obtains detailed clinical information face-to-face, by telephone or electronically
- May provide basic information regarding the need for insurance referral(s)
- Provides general disease or program-specific information to callers/patients within the scope of knowledge and authority
- Performs independent patient and outside provider call triaging and assists in directing patient flow
- Serves as a liaison between patient/family/provider
- Resolves issues directly or ensures appropriate management of call by others
- Recognizes emergencies and appropriately responses using standard operating procedures and critical thinking skills
- May be responsible for scheduling numerous complex research protocols, involving extensive interaction with those associated with in-house studies, grant-funded studies and industry-sponsored clinical trials
- Carries out specific processes in the patient scheduling systems so that correct special billing and revenue processes function properly
- Requires clinical, operational and regulatory knowledge of the standard of care or clinical trial protocol for the patient population coordinated
- Participates in training for new hires
- Prepares, open, and close provider schedules and handles daily schedule changes
- Conduct slide submission as needed
- Other duties and responsibilities as assigned

The Patient Records Specialist is responsible for the effective coordination and organization of materials required for a new patient visit. Provides administrative support and coordination for aspects of patient care for new patients and may flex across multiple disease centers or assist in scheduling tasks.

- Uses electronic tracking systems to thoroughly document progress in all tasks, including retrieval of pertinent patient materials
- Uses standard processes and protocols to monitor and follow up with patients, hospitals, and other parties on materials status
- Collaborates with fellow Records Specialists and New Patient Coordinators to ensure seamless coverage and task management in times of full and partial staffing levels; This may include assisting with registering, scheduling, or performing other tasks that are typically the responsibilities of the New Patient Coordinators
- Maintains standard communication with DFCI clinical and support staff with regard to status of patient materials
- Assists in processing paperwork and completing administrative tasks associated with clinical care including managing materials requests, orders, and records uploading
- Responds to emails and manages other requests as appropriate for new patients. This includes the responsibility to relay detailed clinical information provided by patients, internal/external providers, and others such as staff from ancillary services
- Resolves problems independently, ensures continuous communication with New Patient Coordinators, and appropriately escalates issues to management
- Possesses a level of independence requiring knowledge of multiple disease specific programs
- Provides administrative support and coordination for all aspects of patient care for new patients, primarily focused on supporting materials management
- Triages issues and answers general questions, with the goal of resolving requests in real time
- Recognizes emergencies and appropriately responds using standard operating procedures and critical thinking skills
- Serves as a liaison for patients including efficient routing of calls per telephone triage guidelines
- Can comprehend and implement new concepts or modifications to processes requests quickly
- Ensures quality clinical care and adherence to standard operating procedures and compliance requirements
- Schedules appointments following guidelines that support continuity of care and a high standard of timely communication and rescheduling as necessary

- Provides program specific information to callers and refers calls as necessary
- Assists in processing paperwork associated with clinical care including managing materials requests, signing up for EMR portal
- Assists in the training of new staff
