Responsible for the planning, delivery and management of direct patient care utilizing the nursing process and adhering to the standards of nursing practice.I. Major Responsibilities:
1. Utilizes the Nursing Process for the delivery and management of patient’s care from Admission to Discharge, appropriate to the developmental age of the patient population.
2. Performs patient assessment/reassessment according to standard procedure.
3. Develops and coordinates individual plan of care including discharge planning.
4. Directs, supervises, coordinates and evaluate nursing care within assigned clinical setting.
5. Delegates appropriately to others in accordance with job description and competency level.
II. Position Qualifications:
License/Certification/Education:
Required:
1. Current Massachusetts Nursing licensure and registration.
2. BLS is a requirement for every RN unless advance cardiac life support (ACLS) certification is current, in addition to any unit specific certifications and competencies.
Preferred:
1. Bachelor of Science degree in Nursing highly preferred.

1. Thorough knowledge of the discipline of Nursing.

This position provides routine, personal care such as bathing, dressing, or grooming, to convalescent, terminally ill, or disabled persons in their home setting.I. Major Responsibilities:
1. Provides direct and indirect personal care in a safe and timely manner in accordance with departmental policies and procedures.
2. Performs homemaking and other environmental services in a way that facilitates sanitary and safe care in the patient’s home according to policies and procedures.
3. Duties may include:
a. Providing patients with mobility assistance, bathing, dressing, and grooming.
b. Providing patients and families with emotional support and instruction in areas such as caring for patient or self, preparing healthy meals, living independently, or adapting to disability or illness.
c. Changing bed linens, washing patients' laundry, cleaning patients' living area and other homemaking tasks identified within the home health plan of care.
d. Preparing or serving meals to patients according to the home health plan of care.
e. Directing patients in simple prescribed exercises or in the use of braces or artificial limbs.
f. Checking patients' pulse, temperature, and respiration.
g. Changing clean dressings.
h. Performing a variety of duties as outlined in the home health plan of care.
4. Maintains records of patient care, condition, progress, or problems to report and discusses observations with supervisor or professional according to policy.
5. Observes, documents, and reports changes in client status or expectations of services promptly to a supervisor or professional according to policy.
6. Recognizes emergency situations and implements interventions according to appropriate departmental procedures and policies.
7. Renders services in accordance with the written plan of care as documented by the appropriate professional.

1. Current Home Health Aide certification or Certified Nursing Assistant (CNA) license.
2. Must have a valid US-issued driver’s license and a registered, inspected, and insured automobile for work related purposes.

1. Minimum six (6) months of experience providing care as a certified home health aide in a home-based setting.
2. Excellent oral and written communication skills.
3. The ability to read, write and speak clearly in English.
4. Ability to listen and give full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.
5. Ability to develop and maintain constructive and cooperative working relationships with patients, coworkers, and supervisors.
6. Ability to consistently be responsible, compassionate, patient, emotionally stable, and cheerful with patients, caregivers, and other professionals.
7. Ability to be tactful, honest, and respectful of patients and family members in private homes.
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

- Participates in drug therapy management and documents the care provided in patients’ medical records. Participates in interdisciplinary reviews of patients’ progress, as well as communicates pertinent patient information to other health care providers when patients change care settings to ensure continuity of care.
- Prepares medications by reviewing and interpreting physician orders; detecting therapeutic incompatibilities.
- Dispenses medications by compounding, packaging, and labeling pharmaceuticals.
- Contacts and confers with physicians and other practitioners to optimize drug therapy and advising of needed interventions.
- Assists patients in gaining access to medications (e.g., working with payers to resolve issues that may impede access to medication therapies) when applicable.
- Interprets and responds to clinical decision support and drug utilization review from internal and external systems on patients’ drug therapy.
- Provide individualized health promotion and disease prevention, including administration of immunizations where legal and authorized.
- Provides computer order entry, ensuring proper technique and accurate preparation of all pharmaceutical products.
- Supervises technicians as needed.
- Complies with state and federal drug laws as regulated by the state board of pharmacy, the drug enforcement administration, and the food and drug administration.

Position Purpose:

PRIMARY DUTIES AND RESPONSIBILITIES:
- Leads, coordinates, and monitors the review and analysis of practitioner applications and accompanying documents for all credentialing and provider enrollment of physicians and allied health applicants.

Physician Assistant I
The Immunotherapy Toxicity Management Program Consult PA will be responsible for the following duties and responsibilities during their shift:
- Sign in and respond in timely manner to consult pager daily during consult hours.
- Complete requested consults and staff with appropriate Oncology iTox Attending, including but not limited to executing on the following:
- Obtains, compiles, and records patient medical data and presents to the appropriate ambulatory consult attending.

- Makes recommendations to inpatient team regarding work-up of new cancer diagnosis.
- Facilitates patient and family decision making regarding complex treatment

This position administers and supports the facilities management program ensuring compliance with regulatory and The Joint Commission accreditation standards as well as the hospital’s space planning and project and departmental budgeting function for HealthAlliance-Clinton hospital. This position works in collaboration with Facilities Management and Public Safety Management.Administers the department’s purchasing expenditures from quotes through receipt, as well as administers budgets for engineering, grounds, plant operations and public safety. Works independently tracking and supplying data to management, as well as working in a team environment with input to department project plans.

1. Prepares comprehensive capital planning plans for management, including resources, schedules and budgets for construction and maintenance projects, and Public Safety plans. Monitors project plans, schedules, budgets and expenditures bringing concerns to Facility Management.

Monitors and researches variances to budgets and reviews with department management. Supervises daily operation of assigned areas within the Patient Access Center (PAC). The PAC is designed to be a premier health contact center that provides patients, families and referring providers multiple pathways to connect to UMass Memorial Health (UMMH). The Supervisor provides operational oversight of the PAC which includes implementing the PAC’s strategic direction, supporting short and long-term goals, staff oversight and accountability, and acting as liaison with patients, referring providers, UMMH providers, hospitals, and senior leadership.I. Major Responsibilities:
1. Coordinates daily activities for the Patient Access Center, including but not limited to scheduling staff, maintaining work queues, and performing statistical reporting.
a) Evaluates workload across priorities and distribute and monitor the flow of work for an assigned group of employees.
b) Ensures staff coverage and schedule.
c) Serves as the point of contact for escalated phone calls, technical issues, and workflow questions. Resolves or escalates issues where appropriate and provides customer service recovery when necessary.
d) Manages the operations of the PAC to ensure optimal performance, patient, and provider experience. Manages the performance of the scheduling-based team and/or general information functions at his/her assigned PAC location. Oversees and monitors work queues and reports for the Patient Access Center.
e) Examines current resources and identifies opportunities for system support to streamline processes for staff and management, including streamlining workflows.
f) Manages work queues and reports to ensure appropriate productivity and quality standards are achieved.
g) Organizes and maintains clear channels of communication with staff and exhibits appropriate communication with all levels of personnel.
2. Responsible for supervising PAC personnel assigned
a) Conducts onboarding and continuous training to ensure employees receive the appropriate skillset development and are appropriately trained. Ensures staff compliance with mandatory education and training.
b) Ensures staff evaluations are completed according to defined PAC performance standards, account evaluation periods, and conduct disciplinary action process as appropriate.
c) Holds individuals accountable to meet or exceed the performance and customer service standards. Provides support to management during challenging staff disciplinary action and / or termination discussions.
d) Oversees collection of patient liabilities as appropriate.
e) Participates in hiring process such as conducting interviews, recommendations for hiring and coordinate with the Manager and HR.
3) Serves as the PAC liaison with assigned clinics and departments
a) Collaborates and works closely with other clinic and departments to promote a harmonious relationship and atmosphere of teamwork and partake in process improvement initiatives.
b) Assists the Manager to identify areas of opportunity and process improvement to improve communication channels, reduce hand-offs and access with the clinic.
4. Serves within roles of the Scheduling Agent
a) Handles patients, families and referring providers to schedule patient appointments. Scheduling scope includes multiple incoming phone calls from providers across multiple clinics/departments, practices across multiple campuses.
b) Makes outbound phone calls to patients to schedule appointments with specialists and subspecialists.
c) As part of the appointment scheduling process, secures the information needed to facilitate the financial aspects of medical care such as patient demographics (name, date of birth, etc.), insurance information, and account guarantor. Processes updates and corrections to patient insurance and demographic information. Collects patient liabilities prior to or on the date of service, as appropriate.
d) Based upon appointment visit type, secures medical records from outside health care organizations to ensure the provider has the necessary medical information to provide care.
e) Schedules in office procedures (such as biopsies, etc.), which often requires scheduling the appointment and scheduling the room where the procedure will take place.
f) Applies relevant functional knowledge of multiple treatment areas, the priority and sequence of associated procedures, tests, and primary care services.
g) Prioritizes visits and services in a manner that will foster more efficient utilization of physician’s clinical staff, and patient’s time, as well as equipment and facilities.
5) Position is a working managerial position. May perform duties of supporting staff when necessary.

Makes recommendations on employee hires, transfers, promotions, salary changes, discipline, terminations, and similar actions.
