SUMMARY:
The Social Worker is responsible for providing basic evaluative, preventive, therapeutic, restorative, and personal care services to meet the psychosocial needs of Mercy patients and their families. The goal is to enable patients to better utilize medical care resources and achieve their optimal level of health. This role applies fundamental psychosocial theories and methods to individuals, couples, families, and groups, addressing the complexity of each situation. The Social Worker will provide services across the continuum of care, from pre-admission to post-discharge follow-up, interacting with individuals of various ages and diverse backgrounds.
Essential Functions:
- Social History & Needs Assessment: Obtain social history information and identify patient needs through interviews with the patient and family, reviewing medical records, consulting with physicians and medical staff, and contacting other relevant sources as needed.
- Psychosocial Evaluation: Analyze and evaluate the patient’s psychosocial situation, ensuring a thorough understanding of the patient's needs.
- Case Planning & Implementation: Develop individualized psychosocial case plans based on the assessment, in collaboration with the healthcare team. Implement the plan and monitor the patient’s progress.
- Discharge Planning: Coordinate a wide range of discharge planning services. Consult with physicians, medical staff, and patients/families to determine post-hospital needs and create a plan for post-discharge care.
- Resource Education & Referral: Provide patients and families with information on available resources and strategies for utilizing these resources. Assist with necessary referrals to appropriate services.
- Crisis Intervention: Demonstrate the ability to prioritize emergency and non-emergency tasks, providing immediate supportive counseling and necessary assistance to patients and families in crisis situations. Refer patients to community resources when appropriate.
- Psychosocial Counseling & Treatment: Provide psychosocial counseling and treatment services aimed at restoring optimal social and physical health adjustments for patients and families, within their capacity.
- Community Resource Knowledge: Possess knowledge of public and private welfare and health agencies available to serve the community. Make prompt referrals to appropriate agencies to meet patient needs.
- Financial Assistance & Insurance Support: Screen patients for financial assistance and help clarify insurance benefits. Support patients and families in coping with economic stresses related to illness or injury.
- Clinical Consultation: Provide clinical social work consultation to physicians and professional staff through patient evaluations, case conferences, in-service programs, and educational presentations.
- Community Liaison: Maintain relationships with community health and social service agencies, establishing effective professional connections through personal/telephone contact and correspondence.
- Documentation: Document all social work activity in the patient's medical record. Maintain records and reports in a timely, accurate, and qualitative manner.
- Interdepartmental Collaboration: Recognize the role of the social worker in the department and its relationship to the overall function of the hospital in patient care.
Minimum Knowledge, Skills, and Abilities Required:
- Education: Bachelor’s degree in Social Work (required).
- Medical Knowledge: Understanding of medical terminology.
- Clinical Skills: Proficient in clinical interviewing techniques.
- Social Work Knowledge: Familiarity with social work principles, standards, and ethics.
- Regulatory Knowledge: Basic understanding of Medicare and Medicaid regulations, as well as child/dependent adult laws and regulations (desired).
- Training: Proof of completion of Mandatory Reporter abuse training specific to the population served, within three (3) months of hire.

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $92,705.60 - $142,064.00 annually. The RN Supervisor for Periop Services is a licensed registered nurse (RN) accountable for leading, managing and advancing people, processes and / or programs that support direct or indirect patient care. ESSENTIAL FUNCTIONS
A. Follows menus and production guides for type and quantity of items to be prepared for all service points. Utilizes principles for batch cooking. Records actual number of servings prepared.
B. Prepares and serves all food items according to therapeutic diets, standardized recipes, menus and service guidelines including quality and appearance.
C. Assembles meal trays to patient care units, including late trays, room service meal trays, and nourishments for general and therapeutic diets on an as needed basis.
D. Follows HACCP techniques and other food safety guidelines in preparation, storage, handling, and holding of all food items including leftovers.
E. Covers, dates, and labels all food items in storage. Rotates food products and other items on a “first-in, first-out” basis.

The Infection Prevention Coordinator leads the Infection Prevention Program in collaboration with the infectious disease physician to ensure an effective infrastructure, inclusive decision-making, evidence-based approach, and effective leadership style in achieving excellence in infection prevention and control.
Works collaboratively with key stakeholders for disease surveillance, prevention, and containment including the collection, correlation, evaluation, and validation of data to determine patterns and trends of infection. - Educates on evidence-based practices for illness and disease prevention and control, health promotion, and safe work practices.
- Completes rounding with the providers/leaders/nurses for infective patients in the acute care and clinic settings making practice recommendations as needed.
- Participates in interdisciplinary team rounding to identify infection prevention opportunities with follow up to action plans.
- Regularly attends unit/department huddles and meetings to share information on infection prevention practices and infection trends.
- Participates in investigations of unplanned infective events or infection risks and reports findings to involved units/departments, such investigations may include learning from defects or root cause analyses.
- Identifies potential or actual serious conditions or workplace hazards and acts effectively to prevent or resolve.
- Consults with facilities management and contractors for building projects to ensure regulations for infection prevention are in compliance.
- Participates in nursing meetings as appropriate.
- Participates in Medical Staff meetings as appropriate.
- Participates/leads regional infection prevention workgroup.
- Oversees infection prevention in the owned critical access hospitals as determined by the manager of quality and safety. This may be market specific.

Schedule:
- Full time, mostly day shift
- 4 10 hr days per week plus call
Responsibilities:
- Provides direct patient care activities in the Cath Lab & other applicable procedural areas implementing an outcome-based plan of care
- Identifies learning needs and implements a plan for the patient, conducts patient and family teaching and documents teaching done
- Appropriately uses equipment required to perform Cath Lab procedures
- Understands varying treatment needs and approaches for patients across the life span, as appropriate to the department
- Performs independently in the role of scrub tech, circulator, and hemodynamic monitor and obtains data collection with physiologic calculations specific to the procedure performed and documents
- Initiates BLS/ACLS protocols as appropriate in cardiac emergencies
- Provides care utilizing critical thinking skills based on patients' needs and acuity, and is consistent with policies and procedures of the department

- Facilitates medication administration, initiates and manages IV therapy

Qualifications
Required
- Associate degree from a Physical Therapist Assistant program approved by the American Physical Therapy Association (APTA).

- Strong communication and documentation skills.

Assist in the departmental decisions with implementing the philosophy, objectives, standard of care, policies and procedures of the Nutrition Department and MercyOne North Iowa Medical Center.
ESSENTIAL FUNCTIONS:

Works with vendor invoices to ensure orders are delivered accurately. SKILLS AND ABILITIES REQUIRED:

WORKING CONDITIONS AND PHYSICAL DEMANDS:

Periodically (11%-20%) lifts food or equipment up to 50 pounds. Eye/hand coordination is needed to train and demonstrate production procedures and equipment. A minimum one-year point of sale system and computerized food system software experience. Serve Safe Certified and/or Certified Professional Food Manager Certification preferred. - Current Iowa RN license, (MercyOne Genesis, excluding Aledo and DeWitt, - current Registered Nurse (RN) license in either Iowa or Illinois is required at the time of hire. A secondary RN license for the other state must be obtained within 12 months of the hire date.)
- Proof of completion of Mandatory Reporter – Child and/or Dependent Adult Abuse training within three (3) months of hire. Colleagues employed and working solely in facilities located in Illinois are exempt from this requirement in accordance with applicable state requirements.
- Experience leading complex cases, care planning discussions, or interdisciplinary efforts without formal supervisory authority.
- Demonstrated success coordinating care for patients with significant medical, psychosocial, behavioral, and system complexity during hospitalization.
