Where You Will Work:
Help patients transition safely through their hospital stay. Partner with patients, families, and the care team to assess needs, coordinate services, plan discharges, and support utilization management.
We’re Looking For:
License/Registration/Certification: Registered Nurse License issued by the state in which the Team Member practices.
Level of Education: preferred Bachelor’s Degree in Nursing
Years of Experience: 2 years of clinical nursing experience
What You’ll Do:
- Conduct comprehensive assessments and create timely care plans & interventions
- Collaborate and negotiate with patients/families and the multidisciplinary team to meet goals and preferences
- Provide continuity of care and discharge planning aligned with regulatory standards
- Coordinate smooth transfers and handoffs across units and settings
- Deliver case management services (healthcare access, finances, housing, family dynamics, illness adjustment) using community resources
- Initiate internal/external referrals; document discharge planning and UM activities accurately and on time
- Perform concurrent/retrospective reviews using approved criteria
- Partner closely with Social Work and support staff to implement safe, effective plans of care
- Identify and remove barriers to discharge; expedite care delivery and report care coordination/UM outcomes
- Collaborate with managers, physicians, medical directors, and physician advisors on status, progression of care, and regulatory compliance
- Complete UM activities (payer updates, data collection, denials coordination, avoidable delay management); deliver CMS notices within required timeframes
- Build relationships with community agencies; collaborate with Ambulatory Care Management/Continuing Health
- Educate staff on admission status, acute care criteria, UM, care coordination, discharge planning, and regulatory requirements
- Provide age-appropriate care per departmental standards

Summary:
Performs routine clinical and administrative duties in support of assigned area or office.Responsibilities:
Round with patients in practice to ensure Excellent Patient Experience is met.
Under the supervision of physician, physician assistant, nurse practitioner or nurse, assists with performing patient assessments and treatments.
Obtains and accurately documents patient vital signs, height and weight, takes brief history on current illness or injury, current medications, allergies, required screenings, and other pertinent information as appropriate.
Activates of My Chart Accounts where appropriate. Ensures Telehealth connectivity for scheduled appointments and trouble shoots as needed.
Follow-up on reported issues and assists with service recovery.
Maintains equipment and medical supplies, including reordering, stocking, cleaning/disinfection per protocol, and troubleshooting. Maintains examination areas.
Manages and facilitates all in-basket messages and patient phone calls.
Monitors and communicates changes in patient condition. Documents patient care provided. Provides discharge instructions and follows-up with patient as needed. Recognizes and seeks help in emergent situations.
Performs administrative or clerical duties as assigned, including scanning, reception, scheduling, data entry, and patient registration.
Performs basic diagnostic procedures and specimen collection, including phlebotomy, splint application and procedural tray/room set-up.
Reviews patient charts to ensure imaging is available and all pertinent information to maximize appointment.
Reviews/Documents Social Determinants. Provides education/resources as needed.

What You Will Do:

* Perform the HBO treatment and monitor the patient during therapy.
* Perform dressing changes and/or wound care within scope of practice, as ordered and participate in clinic when HBO volumes are below par.
* Take serial photographs of the wound for reporting of the patient's progress.
* Evaluate the patient outcome and meet with the manager and the hyperbaric physician to
discuss patient treatment and adjust treatment as necessary.
* Consult with hyperbaric physician regarding any adverse reactions.
* Document treatment notes and maintain patient treatment/diagnostic records.
Minimum Qualifications:

Stakeholder Communication & Reporting
- Prepare and distribute monthly capital project status reports to the Director of Engineering, CFO, and clinical leadership including schedule, budget, risk, and action items.
- Maintain the capital project portfolio dashboard for executive visibility.
- Serve as the primary point of escalation for clinical department managers on construction-related impacts to operations.
MINIMUM QUALIFICATIONS
Education: Bachelor's degree in Construction Management, Engineering, Architecture, or a related field required
Experience: 5+ years of project management experience on healthcare construction or renovation projects; hospital environment experience required
Licenses / Certifications: PMP (Project Management Professional) preferred; CHFM preferred; OSHA 30 preferred; ICRA/ILSM certification preferred
Tools / Technology: Procore or equivalent construction management platform, MS Project or Smartsheet, AutoCAD / Bluebeam Revu, Brightly Worxhub CMMS, MS Office 365
PREFERRED QUALIFICATIONS
- PMP (Project Management Professional) certification from PMI.
- CHFM (Certified Healthcare Facility Manager) certification.
- Demonstrated experience managing AHCA-permitted healthcare renovation projects in Florida.
- Familiarity with FGI Guidelines for Design and Construction of Hospitals (latest edition).
- Experience managing projects involving active clinical spaces including operating rooms, ICUs, and sterile processing departments.
- LEED AP or equivalent sustainability credential.
