NaphCare is hiring Licensed Practical Nurses to join our team of medical professionals in Grand Junction, Colorado, where we manage all aspects of healthcare for the individuals who are currently incarcerated in the Mesa County Jail. We currently have Part Time and PRN positions available!
*Our rates have recently been increased! Pay rates range from $36.00-$38.00 per hour plus a $6.00 shift differential for night shifts.*
As a Correctional Nurse, you will work alongside correctional officers and other medical professionals to provide care to the individuals in residence. Working within a controlled environment, where security is top priority and the days are never dull, you will have various duties to keep you engaged including performing physical assessments, administering medications, managing chronic illnesses, and responding to emergencies. This is a unique position where you can make a difference to this diverse population of patients with complex medical needs.

As an LPN, you will play a critical role in our continued mission to provide quality healthcare that protects the rights, dignity, and health of our unique patient population. Duties and responsibilities may include and not be limited to:
Performing patient assessments, interviewing patients about current health problems, medication history
Taking vital signs including blood pressure, temperature, and weight
Providing wound care, including cleaning and bandaging injured areas
Administering medication, including injections of medications and monitoring reaction to meds
Maintaining medical records, including entering information into our electronic health record, TechCare
Observing patient health and mental health status
Providing emergency first aid
Providing patient education and discharge/release planning
Graduate from a Licensed Practical Nursing program
High school diploma or equivalent (GED)
Actively state-certified as a Licensed Vocational Nurse or Licensed Practical Nurse
Must possess an active BLS CPR Certification (American Heart Association required)
Able to maintain confidentiality of all proprietary and/or confidential information
Display integrity, professionalism and be able to adhere to a Code of Conduct and comply with all facility(s) correctional healthcare policies, procedures, and legal requirements
Must pass the facility’s criminal background check and drug screening
Physical Capabilities:
Must have the ability to stand for long periods, lift and move patients (up to 50 pounds), bend, squat, reach, push, pull, walk significant distances, and perform physically demanding tasks like CPR, all while maintaining good balance and endurance throughout a shift.

Who are you:
Porch Light Health is looking for a Certified Physician Assistant, and/or Nurse Practitioner to work in the specialty of Addiction Medicine. We encourage new grads to apply as we will train you in the Addiction Medicine specialty. This job is 40 hours per week, clinic-based, located in Pueblo, Colorado. The medical Provider will see patients in clinic as well as via telehealth. You would be helping persons suffering from addiction or substance use disorder such as alcohol, opioids, stimulants, and benzodiazepines, etc. No prior addiction medicine experience is necessary. Emergency Room, Primary care, or psych experience is helpful. Must have an interest in helping underserved populations and be willing to learn new concepts. The ability to administer intramuscular medication by injection is required.
Key Responsibilities:
Patient Care: Provide clinical assessment, diagnosis, and ongoing management of patients seeking MAT for opioid or alcohol use disorder.
Medication Management: Prescribe and monitor MAT medications (e.g., oral and injectable buprenorphine and naltrexone), ensuring appropriate dosing, adherence, and monitoring for potential side effects.
Comprehensive Assessments: Conduct thorough medical histories, physical examinations, and diagnostic evaluations to assess patient needs and treatment progress.
Collaborative Care: Work closely with a multidisciplinary team including physicians, counselors, peer workers, and other healthcare providers to develop and implement individualized treatment plans.
Patient Education: Educate patients on the nature of substance use disorder and the MAT process, and provide counseling on lifestyle changes, coping mechanisms, and return-to-use prevention.
Follow-Up Care: Monitor and evaluate patient progress, adjusting treatment plans as needed, and coordinating care for any comorbid conditions.
Documentation: Maintain accurate and timely electronic health records (EHR) for all patient encounters, treatment plans, and medication management.
Compliance & Regulations: Ensure compliance with all federal and state regulations, including MAT guidelines, HIPAA, and appropriate documentation of controlled substances.
Crisis Intervention: Provide crisis intervention and de-escalation techniques when necessary, ensuring patient and staff safety at all times.
Quality Improvement: Participate in continuous quality improvement efforts to enhance patient care outcomes and clinic operations.
Qualifications:
Education: Master's or Doctoral degree in nursing (APRN), physician assistant (PA), or other related field.
Licensing & Certification: Active, unrestricted state license as a Nurse Practitioner (NP) or Physician Assistant (PA). (Board Certification) Active DEA registration status.
Experience: Minimum of 1 year of clinical experience, with at least 0.5 years in addiction medicine, specifically in MAT clinics.
Training & Knowledge: In-depth knowledge of MAT protocols, medications, and treatment for opioid, stimulant, benzodiazepine, and alcohol use disorders. Experience with behavioral health and counseling approaches is a plus.

Ability to work in a fast-paced, team-oriented environment.
Exceptional communication skills, with the ability to build rapport with patients and collaborate effectively with healthcare teams.
Understanding of regulatory compliance and documentation requirements in MAT settings

Job Duties / Responsibilities
The Licensed Social Worker Mental Health Counselor is responsible for assessing, diagnosing, and treating patients in residence with mental illnesses, substance use disorders, and behavioral issues ensuring they receive quality care that protects their rights, dignity and health. Duties and responsibilities may include and are not limited to:
Collaborating with medical, mental health and correctional staff to enhance quality patient care while maintaining a safe and secure work environment
Performing and documenting mental health evaluations, progress notes, individual and group counseling notes, individual treatment plans, addressing criminogenic needs and promoting positive behavioral change to prepare residents for release
Participating in educational and training activities related to mental health care for patients, security personnel, and healthcare staff
Documenting and managing case files, patient progress, preparing reports, and participating in hearings
Initiating and/or participating in patient education and treatment programs, such as developing educational materials and providing group therapy
Monitoring compliance with site-specific and company policies and procedures, taking appropriate corrective action, and ensuring compliance with ACA, NCCHC standards
Experience / Requirements / Qualifications / Skills
A minimum of 3 years’ clinical experience in a mental health care or acute care setting, with correctional healthcare experience preferred
Must have knowledge and experience in the identification and treatment of mental health and co-occurring substance abuse disorders, discharge planning, principles of social work, basic principles of individual and group behavior, and a working knowledge of psychotropic medications
Correctional counselors must be able to remain calm under pressure while assess patients’ situations, underlying causes of their behavior and be able to develop interventions and treatment plans
Effective communications skills are required for building patient rapport and relaying information and treatment plan with other medical and mental health care professionals
Strong organizational skills are needed to keep track of patient treatment plans and progress notes

We are seeking: a *full-time licensed psychologist* at our Colorado Springs location. Our psychologists typically perform three to four evaluations per week, and feedback is provided to all clients that we evaluate. We mostly focus on serving Medicaid clients and other disadvantaged populations. We believe that our services are essential for clients to receive targeted treatment and resources that are designed to meet their unique needs.

Brain and Body Integration (BBI) is an outpatient private practice. Working in a private practice has a lot of benefits in comparison to other outpatient settings. In private practice, you have a flexible schedule. You dictate when you work and when you perform evaluations. And once you are finished with an evaluation, you can write up the report from home. There is no need to stay in the clinic if you don’t have to! In addition, as a private practice we do not have a complicated bureaucracy to navigate. If we want something to change, or we need to advocate for our clients, we can do it and have answers the same day. There is no need for you to wait for your boss to contact their boss who has to contact their boss, etc. We know what our clients need, and since we are not connected to a large organization, we can make the decisions we need to make to create more informed care for our clients. In addition, we have a team of psychologists that you can collaborate with from many different backgrounds. We currently have 15 psychologists from forensic, school, clinical, and neuropsychological training backgrounds.
When you work at our private practice, you can choose what type of work/life balance that is best for you. You can work as much or as little as you want. We do not set a productivity expectation for you, and we do not limit how much time you can take off. This flexibility allows our team to tailor their schedule to the unique needs of their life, without having to constantly worry about a supervisor denying their request for time off or a reduced workload.
We also perform a variety of evaluations such as psychological, neuropsychological, learning disorder, adaptive functioning, and neurodevelopmental evaluations. We work with many different referral sources, so every day you have a different referral question to answer. This keeps our diagnostic minds sharp, and it prevents us from having repetitive referral questions day after day. Our practice also encourages clinicians to balance their expertise with an evaluation report that is succinct, to the point, and directly answers the referral question. We don’t want you to spend your time writing a novel for an evaluation report. We want you to spend less time writing, and more time engaging in clinical or non-clinical activities that you enjoy.
Lastly, we believe that excellent training and onboarding breeds long-term success and happiness within our practice. We make sure you have the training you need for any assessment that you are not familiar with, or any technological platform you have not worked with before. We then break up the first nine months of your employment into three rotations. Each rotation focuses on a specific evaluation type. This way, you don’t have to learn a novel battery for multiple evaluation types within the first few weeks of your employment. You can take your time to become familiar with the different types of evaluations and their associated batteries. In addition, during this process you will pair with the president of BBI in order to receive consultation and support as you acclimate to a new work environment.

Deliver contraceptive counseling, education, and method provision.
Participate in abortion care through counseling, referrals, scheduling, and/or direct provision.

Partner with patients, care teams, and community agencies to support holistic care.
