ERCC serves residents receiving short-term rehabilitation and long-term nursing care. As an LPN, you’ll help provide the attentive, consistent care they depend on—whether they’re working toward recovery or making ERCC their home.
Working within your licensed scope of practice and facility procedures, you’ll:
· Provide nursing care according to residents’ individualized care plans.
· Administer medications and treatments as ordered.
· Monitor residents’ conditions and promptly communicate changes to the appropriate clinical team members.
· Contribute to care planning and coordinate with nursing staff and other care team members.
· Maintain accurate, timely documentation.
· Follow infection-control, medication safety, and resident safety procedures.
· Communicate respectfully with residents and families while protecting privacy and dignity.

It is our goal to provide the finest Orthopaedic care possible. This philosophy requires that all office staff and providers be sensitive and responsive to patients’ needs and preferences. To ensure that we hire and retain the quality of staff needed for implementing our philosophy of service, we have adopted the following job description for this position. The position requires that the employee be available approximately forty hours per week and that also the employee be flexible in his/her schedule to provide efficient service for the group and work additional hours as needed. The employee however, will be expected to work no more than five days per week. Many of the responsibilities require that you have a working knowledge of computers and the ability to learn the proper use of the programs utilized in this office, or those that which may be necessary to meet the needs of the practice. The hours, pay scale, and benefits will be defined as shall be agreed to.
*BASIC FUNCTION:* Under the supervision of the Director of Physical Therapy, the Physical Therapist shall provide all responsibilities necessary for the successful operation of the Physical Therapy Department for Arrowhead Orthopaedics.
*RESPONSIBILITIES:* Work is primarily performed in the Physical Therapy Department located within one of Arrowhead Orthopaedics offices. There will be contact on a daily basis with confidential files pertaining to patients’ care, and the Physical Therapist must be able to handle this information with the highest degree of privacy, discretion, and professionalism. The Physical Therapist is responsible for accomplishing the duties set forth below:
*General Activities (There may be other duties required of this position not listed below):*
* Identifying patient care issues; providing information and commentary; recommending options and courses of action; implementing physical therapy directives.
* Assesses patient health by interviewing patients; performing physical examinations; obtaining, updating, and studying therapy histories.
* Assesses abnormal conditions by reviewing physical therapist's interpretations of patient evaluations and test results.
* Documents patient care services in electronic medical record system.
* Performs therapeutic procedures by administering manual exercises; instructing, encouraging, and assisting patients in performing physical activities, such as non-manual exercises, ambulatory functional activities, and daily-living activities; and in using supportive devices, such as crutches, canes, and prostheses.
* Instructs and counsels patients by describing therapeutic regimens; giving normal growth and development information; promoting wellness and health maintenance.
* Provides continuity of care by developing and implementing patient management plans.
*Essential Functions:*
* Attend to the therapy needs of patients as scheduled.
* Perform 2-3 procedures and 1-2 modalities per patient whenever called for and if possible. Lower levels of performance could result in an increased patient load.
* Complete all PT/OT evaluations and daily notes within 2 working days.
* Complete EEFs, and DASH, and cosign’s within 2 working days.
* Complete Discharge (DC) list monthly
* Maintain full compliance with CMS-Medicare regulations, and other regulations of third party payors.
* Maintain complete documentation of services rendered, and communicate periodically with the billing staff to ensure the proper and timely billing of all services rendered.
*RELATIONSHIPS:* The Physical Therapist should observe and conduct the following relationships:
* The employee shall be responsible directly to the Director of Physical Therapy. The employee shall report to the Director of Physical Therapy and his/her assignees any progress, reports, requests, concerns, problems, and/or expectations in relation to the responsibilities of the position. Such communication may be verbal or written as deemed necessary by the employee or as directed by the Director of Physical Therapy or his/her assignee.
* The employee shall interact with other office and clinical staff in the Practice. Such interactions should be collegial, professional, and contributing to the safety and pleasantness of the work environment at Arrowhead Orthopaedics.
* The employee shall interact with Doctors, licensed physical therapists, students conducting internships, physical therapy aids, and patients. Such communication should always reflect the total commitment of the office to quality care and customer satisfaction.

We are looking for an experienced clinical psychologist to provide consultation and evaluation services in an outpatient setting to patients who need mental health services as a result of a work injury. In this role, you will provide psychotherapy for individuals and groups while utilizing a wide range of intervention modalities that support positive mental health & enhanced individual functioning. As a member of a multi-disciplinary team, you will also work closely with Primary Treating Physicians supporting a Functional Restoration Program (FRP). *RESPONSIBILITIES:*

* The SNF Rounding Clinician is required to obtain a ReSkin Medical approved wound care certification, if not already wound care certified, within one year of their start date with ReSkin Medical. The approved certifications are: WCC, CWS, WOCN.

* Masters Degree from an accredited Nurse Practitioner program

The pediatrician must have a medical doctor (MD) or doctor of osteopathy (DO) degree from a school accredited by accrediting institutions recognized by the U.S. Department of Education, unrestricted California medical license, active DEA license, current board certification by the American Board of Pediatrics. Must have or willing to obtain current CPR Certification from the American Heart Association or Red Cross prior to orientation. The Pediatrician provides primary and urgent patient care in the ambulatory clinic setting. The pediatrician evaluates children 0-19 years of age, instituting appropriate diagnosis, treatment, follow-up, and referral for consultations when indicated. This position is an integral part of the health care team, which includes other physicians, nurse practitioners, nurses, and other support staff. Must possess a valid California driver’s license. Travel required between the clinics as needed by department. _Must be able to work with the Indian community, and be sensitive to the Indian Culture and its needs._
*Major Duties and Responsibilities:*
Duties of this position shall include, but are not limited to the following: (in appraisal format)
1. TECHNICAL SKILLS: Demonstrates proficiency in history taking and physical examinations of children. Demonstrates competence for all specialized procedures for which privileging is requested. Appropriately documents all procedures in the medical records.
2. DIAGNOSTIC SKILLS: Appropriately interprets historical and physical examination data to arrive at accurate diagnosis. Orders laboratory and radiology studies when necessary to confirm diagnosis, or to rule out other differential diagnoses. Utilizes in-house resources prior to referring patient outside of RSBCIHI health care system. Appropriately documents all findings in the medical record.
3. THERAPEUTIC SKILLS: Accurately identifies and orders appropriate treatments. Utilizes Pharmacy formulary for pharmacological options. Orders appropriate follow-up for patient to assess results of treatment. Refers patient when necessary for treatments not available directly from RSBCIHI. Appropriately documents all plans and orders in the medical record.
4. HEALTH CARE GOALS OF RSBCIHI: Recognizes that the health care objectives of RSBCIHI emphasize primary care, health promotion, and disease prevention. Implements these objectives in the routine care of patients in our clinics. Maintains awareness of specific goals via participation in Medical Department meetings and referring to the Program Administration Manual and Medical Department Manual. Utilizes internal resources first and refers to outside health providers only for those services not available directly from RSBCIHI, or when internal services are not available in a timely manner.
5. NATIVE AMERICAN CULTURAL SENSITIVITY: Has an awareness and appreciation for the diversity of Native American cultural and spiritual values, recognizes how these values affect our interactions with Native Americans in the medical setting.
6. LEADERSHIP AND TEAM SKILLS: Functions as an effective member of the health care team, which includes nursing, clerical, pharmacy, and laboratory staff. Effectively communicates with all team members and coordinates everyone’s efforts to provide both efficient and high quality care to our patients. Sets the pace for efficient patient flow through the clinic.
7. ADMINISTRATIVE RESPONSIBILITIES: Participates in Medical Department staff meetings and clinic site meetings. Volunteers for, or accepts when asked by the Medical Director, special assignments, e.g. Medical Department Quality Management Sub-committees, Pharmacy and Therapeutics Committee, etc.
8. PRODUCTIVITY: Delivers patient care in a manner that is thorough and elicits patient compliance within RSBCIHI guidelines:
30 minutes for routine care of patients with chronic illnesses, and minor acute problems.
60 minutes for well child checks and new patients.
Average 12-14 primary care patient visits per 8-hour shift.
Completes all pharmacy refills in a timely manner.
Will participate in the after-hours on-call medical provider coverage rotation schedule every three months as staffing dictates.
Qualifications
*Education:*
Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an institution accredited by an accrediting agency recognized by the U.S. Department of Education.
*Licenses and Certifications:*
* Valid California Medical License
* DEA Registration Certificate
* American Board of Pediatrics Board Certification
* Valid CPR (BLS Provider) certification through the American Heart Association (AHA) or the American Red Cross. Certification may be obtained prior to orientation.
* California Driver’s License with a safe driving record, DMV record required. Must be insurable by the program’s insurance carrier.
*Experience:*
Two years of experience in an ambulatory care patient setting desirable.
*Ability To:*
Must be able to work with the Indian Community, and be sensitive to the Indian culture and its needs.
*Physical Demands:*
Must have excellent physical and emotional health. The assignment will be carried out in a medical clinic setting. There is frequent exposure to contagious diseases. Walking and standing is frequent for long periods of time. Some sitting and lifting are required. The incumbent will be required to assist patients who are frail or handicapped. Driving will be required between clinics.
