Consider joining our team to provide social work services for applicants and residents of the Minnesota Veterans Home-Silver Bay by providing assessments, clinical support and analysis to ensure residents' needs are addressed and by planning and implementing social service programs for residents of the home. This position supports the Admissions Coordinator to determine proper placement of applicants requesting admission and will work effectively with the Home Administrator to ensure the delivery of social services meets regulatory requirements and agency standards.
This position is designated as full-time and primarily day shift. This position is eligible for full employer contribution toward health and dental insurance benefits.

Qualifications
Minimum Qualifications
Current licensure in Minnesota as a Licensed Social Worker (LSW) or ability to obtain licensure by date of hire
One year of Social Work experience
Skills in completing social work assessments sufficient to evaluate clients' social service needs
Human relations skills sufficient to develop and maintain effective working relationships with other staff, residents, resident representatives, and community groups
Preferred Qualifications
Experience coordinating placements in senior housing or long-term care services
Experience in a long-term care or skilled nursing care facility
Experience working with Veterans
Knowledge of federal and state regulations related to skilled nursing facilities
Experience in electronic charting systems, such as PointClickCare
Additional Requirements
This position requires successful completion of the following:

Severe mental illness affects over 15 million adults in the United States. Schizophrenia. Schizoaffective disorder. Bipolar I. Treatment-resistant depression. These are the conditions the rest of healthcare has systematically failed to build for.
What makes this problem different from most of healthcare is that the tools to solve it do not exist. The clinical protocols, the measurement science, the precision medicine, the technology to deliver coordinated longitudinal outpatient care for this population. None of it has been built. Who We Are
Amae Health is a Series B, venture-backed Public Benefit Corporation that delivers specialty outpatient care for people living with severe mental illness, including schizophrenia, schizoaffective disorder, bipolar I disorder, and treatment-resistant depression. The Role
We are hiring a Psychiatrist (PT) to join our clinic opening in Orange, CA. You would be part of an interdisciplinary team dedicated to delivering personalized, compassionate, high-quality care for individuals with SMI.
What You'll Do
Provide behavioral healthcare management to Amae Health patients; monitor and manage care through an array of treatment modalities as patients progress through phases of recovery
Develop individualized treatment plans during the engagement and stabilization phase, focused on specific psychosocial and medical needs that might limit recovery
Provide continuous, regular consultations as patients navigate recovery, both in person and occasionally virtually
Liaise with patient care teams and collaborate with behavioral health providers to ensure an integrated, cohesive approach to care
Address medical needs to mitigate negative impacts of social determinants of health
What You'll Have
Board-Certified or Board-Eligible Psychiatrist, with a CA license and DEA that are active, unrestricted, and in good standing

What You’ll Do
Clinical Responsibilities
Perform evaluations, reassessments, and progress monitoring of pediatric patients
Develop and implement individualized treatment plans and goals
Provide skilled interventions to improve ADLs, sensory integration, motor skills, cognition, and functional independence
Assess and recommend adaptive equipment and home modifications
Educate and support families and caregivers for carryover
Coordinate care with physicians and interdisciplinary teams
Document all care in compliance with agency and regulatory standards
Leadership & Supervision Responsibilities
Supervise and mentor a team of Occupational Therapy Assistants (COTAs)
Maintain oversight of assistant caseloads and documentation compliance
Conduct monthly direct observation visits and performance support
Provide ongoing coaching, training, and competency development
Review and co-sign documentation, including treatment plans and notes
Support onboarding, training, and development of new clinicians
Assist with caseload growth, staffing support, and coverage needs
Participate in discipline meetings, QA/QI initiatives, and special projects
Qualifications
Required:
Active, unrestricted Occupational Therapist (OT) license – Colorado
Graduate of an accredited Occupational Therapy program
Current CPR certification
Valid driver’s license and reliable transportation
Strong clinical judgment, communication, and leadership skills
Preferred:
Pediatric and/or home health experience
Previous supervisory or mentoring experience
