What Your Day Looks Like
You will join a team who loves what they do and are excited to learn and grow alongside you. Our multidisciplinary model means you'll have the opportunity to collaborate with Speech, OT, and ABA, gaining new perspectives and building a richer understanding of every child you serve.
Day to day, you can expect 30-60 minute one- to- one sessions that foster strong client and caregiver relationships, up to two evaluations per week that keep the work varied and engaging and built in documentation time so your evenings and weekends are yours to recharge. Teammates can expect a productivity of 72.50% of their day.

*Duties*
Provide skilled Occupational Therapy service, on an intermittent basis, to patients in their own homes, in accordance with the physician’s orders.
*Responsibilities*
* The Physician in evaluating the patient’s level of function by applying diagnostic and prognostic procedures.
* Conduct initial and ongoing comprehensive assessments of the patient’s needs including Outcome and Assessment Information Set (OASIS) assessment.
* Evaluate the patient’s his/her significant other(s) and the home situation to determine what instruction will be required, what assistance will be available for the family in caring for the patient and what other agency and community services will be required.
* Ensure that the physician’s orders are appropriate and discuss necessary changes.
* Assist in development and implementation of the interdisciplinary patient care plan as it pertains to Occupational Therapy.
* Guide and instruct the patient in prescribed therapeutic self-care and creative activities that are directed toward improving independence, physical and mental independence.
* Instruct patient in care and use of wheelchairs, braces, splints, prosthetic and orthotic devices.
* Teach supervise and counsel family and patient in the total Occupational Therapy program and other related problems of the patients at home.
* Establish goals based on the patient’s current level of functioning, potential for improvement and perform re-evaluation as indicated.
* Make arrangements for out-patient services procedures, which cannot be given at home.
* Evaluate the effectiveness of occupational therapy intervention and updates the patient care plan as needed.
* Prepare clinical/progress notes on the date of visit and incorporate same in the patient’s record within 48 hours.
* Communicate with the physician on a regular basis regarding patient’s status, obtaining additional orders if necessary.
* Communicate with other team members when appropriate and instruct them in Occupational Therapy techniques that they may use while working with the patient.
* Supervise the Home Health, (per Agency policy) when Occupational Therapy is the only professional discipline involved in the patient’s care.
* Supervise the Occupational Therapy Assistant every 60 days (as per Agency policy)
* Confirm on a weekly basis the schedule of visits with the clinical manager in order to coordinate the patient’s visit schedule.
* Participate with staff, patients and physicians in discharge planning activities and complete a Occupational Therapy Discharge Summary.
* Assume responsibility for self-development by continually striving to improve his/her healthcare knowledge through educational programs, attendance at workshops and conferences. Active participation in professional and related organization meeting and individual research and reading.
*Assist in Agency Admission Process.*
* Obtain a medical history as it relates to present condition.
* Evaluate the patient and the home situation to determine what instruction and assistance will be available from the family in caring for the patient and what other agency and community services will be required.
* Ensure that the physician’s orders are appropriate and discuss necessary changes.
*Job Conditions*
* The ability to drive and the ability to access patient’s homes, which may not be routinely wheelchair accessible are required.
* Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient’s condition and to perform patient care.
* On occasion, maybe required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds.
* Must be able to effectively communicate in English.

*Qualifications*

The Registered Dietitian / Nutritionist is responsible for nutrition-related needs encompassing menu review and nutrient analysis, nutrition education and counseling, food safety inspections, program support, and staff training. This position serves two agencies and requires *site visits across Berkshire and Franklin Counties and the North Quabbin area.* The ideal candidate is passionate about nutrition, committed to serving older adults, and skilled in both direct care and community education.
*Responsibilities*
* To meet the needs of the agency, responsibilities include but are not limited to:
*1. Safety & Compliance*
* Conduct food safety and sanitation inspections twice yearly at each Dining Center, Luncheon Club, and at the Home Delivered Meal (HDM) kitchens.
* Monitor certification of Choke Saver and ServSafe for each site.
* Conduct once-yearly food safety and sanitation inspection of the caterer's Boston-based kitchen to ensure compliance with state regulations; perform follow-up inspections as necessary.
* Conduct site visits and inspections for broader program compliance across service areas.
*2. Menus & Nutrient Analysis*
* Create, review, and/or edit monthly menus HDM and Congregate meals based on Standards and Policies for the Massachusetts Elderly Nutrition Program.
* Enter menu information into the statewide Computrition software program.
* Analyze the nutritional value of all HDM and Congregate meals using Computrition.
* Evaluate the nutritional content of therapeutic and other special meals provided by caterers.
* Support menu planning across program sites to ensure high nutritional standards are met.
* Perform nutrient analysis and maintain accurate, timely reports.
* Send monthly menus to meal sites and media.
*3. Education & Community Outreach*
* Prepare and submit an annual nutrition education plan to the AGE state nutritionist.
* Conduct two nutrition education programs per year at each dining center and luncheon club; all presentations must include a lesson plan and evaluation component.
* Deliver nutrition education to community members and staff at local venues throughout the Berkshire community.
* Develop and submit lesson plans, handouts, and evaluation tools.
* Write or revise and edit a monthly nutrition education column for the back of the printed HDM menu.
* Write or revise and edit newspaper articles for _The Good Life_ and contribute articles to the Berkshire Senior newspaper.
* Serve as the agency's primary resource on nutrition-related topics.
*4. Counseling & Direct Care*
* Review and approve therapeutic meal requests for consumers.
* Counsel prospective consumers with life-threatening allergies.
* Conduct nutrition assessments and provide nutritional counseling through:
* Home visits
* Telehealth (video)
* Phone consultations
* Enter results into the Wellsky A&D database.
*5. Training & Staff Development*
* Lead ongoing trainings for staff and volunteers on nutrition-related topics.
* Support quality improvement practices within the Nutrition Department.
*6. Administrative*
* Attend meetings and conferences designated by the Nutrition Program Director, including the Executive Office of Aging and Independence (AGE) quarterly nutritionist group meetings.
* Prepare monthly activity reports and other reports as requested.
* Perform other related duties as assigned by the Nutrition Program Director, other supervisory staff, or a member of the management team.

* Adheres to CMS guidelines by providing initial and ongoing assessments to determine the level of functioning, including OASIS assessment at appropriate time points.
* Gathers background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient’s physical and psychosocial functioning.
* Develop and revises the plan of care in consultation with the physician and other care team members.
* Responsible for planning, evaluation, and treatment implementation of an assigned caseload.
* Preparing clinical progress notes and evaluate outcomes.
* Able to complete clinical documentation within 24 hours of patient contact.
* Supervising therapy assistants and home health aides as appropriate.
* Educate patients and/or families about proper safe use and recommend methods for the installation of adaptive equipment.
*Physical Therapist (PT) Qualifications and Skills:*
* Currently licensed by the Massachusetts Board of Physical Therapy (PT) Examiners.
* Experience in acute care settings or Home Health is preferred but not required.
* Able to maintain strict confidentiality about protected and sensitive information.
* Must be able to solve problems independently and confidently by applying analytical and logical thinking.

What You'll Do
Serve as the attending physician for patients in each assigned SNF.
Collaborate with NPs/PAs to manage daily patient care and oversee clinical supervision.
Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
Participate in Theoria's Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
Your Qualifications
Physicians (MD/DO) must hold an active U.S. medical license and be board-certified in an appropriate specialty. *Essential Functions*:
* Performs complex tasks associated with the operation of high field strength magnets and related equipment to produce special radiographic images of specific body parts.
* Screens patients for MR safety prior to procedure performance.
* Prepares facilities in accordance with procedural requirements; calibrates and adjusts equipment; prepares contrasts and materials; cleans and disinfects equipment in accordance with sterile procedures and standards.
* Determines appropriate methodology and techniques to use; arranges immobilization and/or support devices for proper patient positioning; adjusts shields; selects appropriate imaging device.
* Obtains patient history, explains procedures to patient, addresses patients’ concerns, and follows established pre-screening policy and procedure.
* Follows prescribed procedure in the administration of MRI study or radiography.
* Monitors the patient’s physical condition during the course of the procedure; report patient change of status or emergency situations to clinical staff, supervisor and/or cardiologist.
* Evaluates MRI scans for technical quality and appropriately label.
* Maintains accurate patient documentation, including computerized documentation.

* *Qualifications and Requirements:*

* Maintains state license as a general radiographer in the state(s) in which employed and practices.
* One year of current experience within the last three years in a comparable job classification required.
* Maintenance of current Cardio Pulmonary Resuscitation (CPR) for HealthCare Workers or Basic Cardiac Life Support (BCLS) certification for HealthCare workers.
* Educated on and compliant with HIPAA regulations; maintains strict confidentiality of client information.
* Understanding of MRI processes and procedures and able to effectively carry out with minimal supervision.
* Ability to operate MRI imaging equipment and software.
* Demonstrates an understanding of human cross-sectional anatomy, physiology, pathology, pharmacology and medical terminology.
* Excellent written and verbal communication skills, with the ability to clearly communicate detailed information to customers, coworkers and management.
