Serves as lead responsible for monitoring, maintaining and coordinating work assignments. Performs ongoing audits of select inpatients and outpatient accounts and provides training to ensure optimum reimbursement and hospital coding compliance. Ensures accurate and appropriate information is documented, coded, and entered into the system(s) in order to meet departmental, hospital and outside agency requirements. This includes appropriate reimbursement, verification, compliance, and charging with the various coding guidelines and regulatory agencies. Responsible for obtaining accurate and complete documentation in the medical record for accurate coding assignment, severity of illness and risk of mortality for each medical record.MINIMUM QUALIFICATIONS:

EXPERIENCE:

1. Monitors, maintains and coordinates work assignments.
2. Processes daily reports.
3. Performs training and in-service education and serves as a coding expert.
4. Performs audits around charging and coding, as assigned.
5. Works with revenue cycle teams to ensure that accurate, timely coding and optimum reimbursement occurs.
6. Assists the management team in the day-to-day operations of the department, as it pertains to reimbursement, coding, abstracting, productivity, quality and education.
7. Assists with development of coding related policies, procedures, query development, work queues and training materials in conjunction with management.
8. Reviews and accurately interprets medical record documentation from all accounts in order to identify all diagnosis and procedures that affect the current inpatient stay or outpatient encounter and assigns the appropriate ICD-10, CPT, or modifier codes for each diagnosis and procedure that is identified.
9. Assures that quality and timely coding, charging and abstraction of accounts are completed daily for assigned specialty areas.
10. Maintains and enhances current levels of coding knowledge through quality review, attendance and participation at clinical in-services and coding seminars, internal meetings, study of circulating reference materials, and inclusion of updates to coding manuals.
11. Assures the accuracy, quality, and timely review of data needed to obtain a clean bill.
12. Contacts physicians or any persons necessary to obtain information required to accurately code assignments. Works and communicates with other offices in any manner necessary to facilitate the billing process.
13. Monitors on an on-going basis provider documentation. Performs audits to assess provider coding accuracy and follows up with provider education as needed.
14. Provides assistance to Revenue Cycle Operations in claim development functions to resolve problem patient accounts.
15. Interacts with physician and non-physician providers to maximize correct coding initiatives.
16. Analyzes & resolves issues of missing charges and problem accounts by researching information regarding department reimbursement.

1. Prepares room, equipment, materials, as needed for patient exams, including sterile technique. Cleans and disinfects rooms and equipment following each patient exam, using current infection control policies.
2. Performs examinations per Physician’s orders and explains procedures to the patients. Documents any variations from protocol due to patient’s condition or other unknown cause and relays patient history and clinical data to Radiologist.
3. Observes patient and equipment and reports any unusual occurrences or changes. Transports and assists patients as needed.
4. Documents status of patient in reference to radiology/CT procedure on patient’s chart.
5. Documents variances repeats and other such occurrences to help maintain high quality health care and opportunity for quality improvement.
6. Maintains stock supplies, performs general office duties, responsible for proper film labeling and following prescribed techniques to assure acceptable quality and consistency.
7. Practices safe radiation exposure and protection as well as general safety for patient’s, visitors, and staff at all times.
8. Participates in the patient safety training and patient protection, including the needs and methods to report medical/healthcare errors and abuse.
9. Produces diagnostic quality images utilizing CT equipment and effectively judges CT images for proper technique to best demonstrate anatomy and pathology.
10. If applicable, performs diagnostic studies using mobile radiographic equipment in emergency or patient rooms as ordered by physician. Performs diagnostic studies in surgical suite with C-Arm and other equipment.
11. Provides care specific to the needs of neonatal, pediatric, adolescent, adult and geriatric patients.
12. May perform IV insertion/blood draws.
13. Administer IV/Oral/Rectal contrast for CT imaging procedures per radiologist protocol/department manual and ordering physician/care provider order.
14. Utilize PACS to ensure exams are completed and available for the radiologist to read and for the referring physician/care provider review.
For facilities with specialty accreditation requirements:
1. Completion of annual required education related to specialty accreditation as defined by accreditation standards. Training may be completed through CBLs, trainings, In-services, and competency validation.

Position Title
Patient Scheduling Associate
Liberty Hospital Sports Medicine
Position Summary / Career Interest:
The Patient Scheduling Associate provides the highest level of customer service as they assist patients with routine scheduling of new and follow-up appointments requested by patients and providers and may also contact patients who have received a specialty referral. The Patient Scheduling Associate also assists patients with insurance registration with The University of Kansas Health System's outpatient clinics. This work may be done away from the front desk of a clinical area and could include work in a call center setting taking large volumes of patient phone calls. (Employees in the call center setting may have the opportunity to work remotely from home after demonstrating consistent competency with job duties.) In the clinical location, this role is responsible for front desk operations including greeting and checking in customers and visitors, copay collection, completing MyChart and EMR (Electronic Medical Record) appointment requests, processing external medical records and helping the physician care team prepare for daily patient appointments.
Responsibilities and Essential Job Functions
- Position responds to a high-volume of incoming telephone calls. Answers phone calls in a timely manner; provides complete and accurate exam related information to the patient regarding prep and other exam related notes; keeps work area organized and scheduling information readily available; maintains productivity within benchmark levels. Works to achieve a coordinated patient itinerary sometimes consisting of multiple appointments.
- Educates patients on providing medical records and other medical documentation needed for TUKHS appointments. Verifies patient demographics and insurance information and verification, completing the Medicare Secondary Payer Questionnaire (MSPQ), scanning document preparation, updating documentation and processing of referrals/order/appointment requests. Scheduler maintains discretion and protects patient confidentiality and privacy. Secures patient signatures for consent and financial forms. Familiarity with insurance coverage types (HMO, PPO, VA, Medicare). Identifies insurance plans and specific procedures which need to be pre-approved. Pre-certifies required tests and procedures. Preauthorization of clinic specific visits and procedures as needed.
- Pre-Visit Scheduling Schedule internal and external incoming department referrals. Complete order entry using hospital information system or manual order forms (test requisitions) including cancellation or modification of orders as required. Review all clinic orders for date, time, signature, and pagers. Verifies referring physician orders against scheduled appointments for accuracy.
- Supports both front desk and scheduling (telephone and MyChart) workflows. May assist patient with MyChart activation and support, including how to complete check-in online prior to the appointment. Responsible for supporting patient through self-check-in utilizing MyChart and the EMR. Ensure proper front desk coverage until last patient is dismissed from the clinic and any follow-up visits are scheduled. Responsible for outpatient clinic direct rooming
- Schedules/reschedules/cancels in person appointments, telehealth visits, surgeries, procedures and/or ancillary services and all incoming department referrals using Epic Cadence decision trees in a courteous, professional and timely manner; accommodates all scheduling requests; prioritizes exams based on clinical indications and contraindications; confirms patient appointments.
- Responsible for collecting all point of service collections due per the EMR generated patient estimate including copay, co-insurance, and deductibles. Follows and completes all standard registration documentation and scanning process in the Health System EMR. Efficiently navigates medical records and ensure patient information is up-to-date and accurately entered in the correct location.
- Effectively communicates Health System Policies to patients and patient advocates. Uses reference documents and online knowledge base tools to clearly articulate accurate information regarding SHC services.
- Has a working knowledge of patient related policies and procedures as well as an understanding of the University of Kansas Health System services and programs. Employees are expected to follow all health system policies and procedures: personal protective equipment (PPE) guidelines, attire, etc.
- Follows all regulatory and compliance standards, documented protocols and guidelines, daily maintenance of workspace and following environmental health cleaning guidelines.
- Maintains knowledge and understanding of scheduling protocols and guidelines, patient registration, pre-certification requirements and basic levels of health insurance and billing procedures. The Registered Nurse (RN) is a professional caregiver who assumes responsibility and accountability for assessing, planning, implementing, and evaluating the care of patients. Minimum Qualifications

- Oversees clinical research, evaluation and analysis of the effectiveness of evidence-based treatment. Determines effectiveness of interventions and other program components in working with patients.
- Works with site based and regional teams to meet local, state, and federal compliance standards, guidelines, and practices, including The Joint Commission (TJC).
- Provides psycho-diagnostic services to patients and their families and identifies and administers appropriate diagnostic materials. Provides quality individual, couple, family and/or group therapy, which is focused on the patient's identified diagnosis and needs, and provides crisis intervention to new and current patients.
- Performs human resources responsibilities for staff which include interviewing and selection of new employees, promotions, staff development, performance evaluations, compensation changes, resolution of employee concerns, corrective actions, terminations, and overall employee morale.
- Develops and recommends operating and capital budgets and controls expenditures within approved budget objectives.

- Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Licensure, Registration, and/or Certification Required:

The Patient Service Representative (PSR) serves as the first connection between Intermountain and patients. Emergency Dept Unit Clerk FT 7a-7pThe ED Unit Clerks serve as the department facilitators, aiding in all processes outside of direct patient care to ensure patient care can happen correctly and efficiently. This is achieved by working very closely with the ED Charge Nurse and the ED Flow Coordinator to help manage the department and patient flow within the department. The ED Unit Clerk is responsible for all clerical duties within the department; including answering a 13-line phone system, all encounter scanning, copying, and faxing. The ED Unit Clerk works directly with the physicians within their assigned team to facilitate and assist in various tasks to aid in patient care, such as, paging and contacting other physicians, obtaining medical records, and arranging transportation to various radiology departments. The ED Unit Clerks are solely responsible for monitoring the 13-bed Patient Safety Surveillance monitoring system, and for arranging all EMS transportation out of the Emergency Department. Employees providing direct patient care must demonstrate competencies specific to the population served.High school diploma or general education degree (GED) required.

Involves regularly lifting of bulky or moderately heavy weights (i.e., up to 50 pounds), and occasionally assisting with heavier tasks or expending the equivalent effort in pushing, pulling, or otherwise handling material, equipment, and other objects.
• Answers/Manages 13 incoming phone lines which entails fielding public and in-house calls. ED Unit Clerks take Incoming Patient Report calls for patients being sent to the ER from doctor’s offices, nursing facilities, and home health employees. Facilitates all patient radiology transfers/transports by keeping in contact via phone with CT, MRI, X-Ray, and Ultrasound.
• Solely responsible for the paging and contacting of any and all physicians on call for every consulting and emergent medical service that BMH offers 24 hours per day. Utilizes SmartWeb and Emergin paging systems, as well as ITSM and Conveyant phone databases.
• ED Unit Clerks are responsible for calling and initiating hospital call downs for the ED. This includes tier 1 and tier 2 trauma calls, AMIs, and Strokes, as well as calling down Code Black Level 1 and 2 for the hospital.
• Monitors and operates the 13 bed camera Patient Safety Surveillance program for patients who require extra visual and auditory monitoring.
• Responsible for scanning all encounter level documents into patient charts for emergency room physicians, nurses, and ED Techs. Also provide encounter scanning for Case Management/MSW, for all law enforcement documentation and subpoena information, and for the ED Pharmacy Department. This ensures the proper documentation is compiled for patients, creating the most accurate and complete medical record.
• Arrange and facilitate all patient EMS transfers out of the ED including transfers to nursing facilities, home to patients’ current residence, to KNI for imaging not offered at Bronson, and to KPEP for those patients who qualify. ED Unit Clerks are the only ED employees trained to use the Bronson Transfer Tracking Application.
• Facilitates and monitors the ED’s admission process. Responsibilities range from ensuring the ED Physician pages the correct admitting provider, to broadcasting to the team when the patient has a clean and ready inpatient hospital bed assignment. This helps ED staff expedite moving patients to inpatient status in a timely manner in order to reach SBAR and GEMBA goals, as well as provide the best possible care to patients.
• Facilitates ordering priority 1 blood and initiates massive transfusion protocols with the blood bank. ED Unit Clerks also do the “running” for massive transfusions, which entails retrieving the products from the blood bank and delivering back to the ED.
• Responsible for obtaining medical records from any and all outside hospitals for continuation of patient care for ED Physicians. Collects and encounter scans all requested records into the patients chart.
• Create a daily Physician call schedule for all consulting and emergent services offered by BMH. This involves maintaining and collecting all on-call schedules for all relevant offices and medical groups in the Kalamazoo/BMH area.

Function as a Patient Safety Assistant for suicide precautions only, which includes,
- Safe patient handling required.
- Responsibilities include observation and reporting patient activities and behaviors and appropriate response intervention to assure patient safety. The Patient Safety Assistant will demonstrate appropriate communication skills with patient/patient family and other healthcare providers, and practice confidentiality and safety procedures as defined by agency. The Patient Safety Assistant provide services in accordance with policy and procedure manuals. Employees providing direct patient care must demonstrate competencies specific to the population served.
- Maintain appropriate knowledge level to assist in patient turns and assist in patient transfer to commode/bathroom.
- Uses correct body positioning, proper body mechanics to avoid injury, utilizes safe work practices (e.g. pushing vs. pulling, patient transfer/lifting devices, etc.) and uses personal protective equipment when necessary.
- 80% Observation and prevention of unsafe patient behavior/activity.
- 10% Communicating with the responsible RN about patient status and pertinent input to facilitate care planning.
- 5% Accompanies patients for diagnostic tests occasionally.
