Baptist Health Pharmacy services are committed to ensuring the optimal safe and effective use of medications for all patients through advanced technology and innovative application of skilled personnel that will improve clinical outcomes and community health in a value-based environment.
What You'll Do:
Provide pharmaceutical care to support antimicrobial stewardship for Baptist Health Floyd patients in Critical Care including optimizing medication therapies
Drug preparation and distribution for medications at Baptist Health Floyd
Evaluate, input, and verify new medication orders
Assist providers and other healthcare professions by serving as a drug information resource
Manage Pharmacy inventory
What You Need:
Graduate of an accredited College of Pharmacy with a minimum of a BS in Pharmacy with a PharmD (Preferred)
Indiana Pharmacist Registration
Hospital experience and completion of an ASHP-accredited Pharmacy Residency (Preferred)
Board Certification in Critical Care or equivalent (Preferred)
ACLS Certification (Will Provide)

This position performs nonprofessional direct and indirect patient care activities including personal, physical and comfort needs of patients, under the supervision of a registered professional nurse. 1. Observes and reports all patient concerns/changes in condition to a registered nurse immediately.
2. Assists the Registered Nurse with admission, transfer, discharge and general patient care duties.
3. Records the vital signs, assists in personal hygiene and other patient comfort measures. Records oral intake and outputs (temperature, blood pressure and other monitoring).
4. Assists in specimen collection and other activities of assigned patients.
5. Assists in indirect patient care by maintaining a clean and safe environment to help prevent and/or control infections and to provide for safety of patients, visitors and employees.
6. Reports all necessary equipment repairs to appropriate personnel according to departmental procedure in a timely manner.
7. Prepares patients for examinations and diagnostic procedures according to departmental procedure.
8. Communicates with RN regarding any nutrition issues, such as poor appetite or verbalized lack of understanding of diet changes.
10. Assists department with administrative tasks to support the department operations.
11. Answers patient call bell and relays message to appropriate personnel

1. Completion of annual required education related to specialty accreditation as defined by accreditation standards. Training may be completed through CBLs, trainings, In-services, and competency validation.

This position comprehensively plans for the coordination of care for the WVU Medicine patient population across the continuum. 2. Current social worker licensure as required by the state where work is being performed.

9. Communicates with the multidisciplinary team and post-acute providers when applicable, any complex family dynamics that may directly impact patient care and transition/discharge planning
10. Initiates and facilitates referrals to post-acute services- including but not limited to- Homecare, Durable Medical Equipment, Hospice Care, Long Term Acute Care Facilities, Acute Rehab Facilities, and Skilled Nursing Facilities
11. Communicates all necessary information regarding transition/discharge plan to the multidisciplinary team, patient and family. Assists other team members to understand and appreciate a patient and/or family’s reaction to a serious illness and/or chronic illness/disease as well as to understand other environmental factors affecting care, treatment and compliance.
12. Provides timely and comprehensive documentation of interactions with patient and/or families and all transition/discharge planning activities and progress according to regulatory policies and procedures.
13. Working knowledge of patient’s current medical insurance coverage and limitations and the precertification requirements for Durable Medical Equipment (DME), post-acute placements, infusions, transfers etc.
14. Assists patient/families with completion of medical power of attorney, health care surrogate, and advanced directives
15. Utilizes clinical skill and expertise to provide assessment, intervention, and where appropriate, reporting for complex abuse, neglect, Foster Care, adoption, Mental health placement, homelessness, domestic violence, and sexual assault situations.
16. Collaborate for appropriate resource and financial management which may include but is not limited to-financial assistance coordination/referrals, entitlement program coordination/referrals, or patient benefit coordination
17. Communicates with the Care Management Resource Center and/or third party payors to facilitate covered day reimbursement certification for assigned patients and discusses payor criteria and issues on a case by case basis with clinical staff (Ie: peer to Peer) and follows up to resolve problems with payors as needed
18. May require occasional coverage to outpatient services including but not limited to LVAD assessment/services, Transplant psychosocial assessments and services and for coordination of discharge services with the outpatient organ transplant/LVAD clinic staff.
19. Uses quality screens in the electronic record to identify potential issues including but not limited to- avoidable delays and readmissions
20. Educates hospital staff and physicians to payer regulations and managed care principals to prevent denials
21. Fosters the integration of staff and/or students into the healthcare team
22. Exhibits professional behavior on a consistent basis
23. Required on call and weekend/holiday rotations as needed

Employees providing direct patient care must demonstrate competencies specific to the population served.
Associates degree from an accredited school of Ultrasound/Sonography or completion of an accredited sonography program or Must have a minimum of one of the following a Certificate of Completion of a Hospital base program or Associate’s degree from an accredited school of Radiology Technology.
Six months to a year of on the job clinical experience and training.
BLS required within 6-12 months after hire. Some positions may require additional certifications.
For general ultrasound positions:
Technologists are required to be registry eligible or registered ARDMS in OB/GYN and Abdomen (which includes Physics board exam). Upon hiring, registry-eligible technologists must attempt all three registries (Physics, OB/GYN and Abdomen) within nine months of hire and successfully complete all three ARDMS registries (Physics, OB/GYN and Abdomen) within 12 months of hire.
For dedicated breast center positions:
Technologists are required to be registry eligible or registered ARDMS in Breast (which includes Physics board exam) or registry eligible or registered in Breast Sonography through the ARRT. This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. The 8th floor Med/Surg specializes in caring for a variety of both medical and surgical patients. We provide inpatient care to patients with multiple diagnoses including sepsis, diabetes, electrolyte imbalances, gastrointestinal, respiratory, cardiac-tele monitored, infectious, kidney, and liver diseases. We also offer surgical care for pre/ post orthopedic surgeries, general surgeries such as bowel resections/ ostomies, pre/post laparoscopic procedure, bladder surgeries, and plastics. For history of present illness, past medical history, review of systems, medications, social history, family history, allergies or other elements of the patient chart: The physician or licensed independent practitioner will elicit the information as usual and Scribes will record the information in the chart for approval.
2. The Scribe may accompany the physician or licensed independent practitioner in the patient examination area in order to transcribe a history and physical examination as given by the patient and physician or licensed independent practitioner.
3. The Scribe documents in the medical chart any procedures performed by the physician or licensed independent practitioner, or nurses.
4. The Scribe transcribes any consultations or discussions with family and/or the patient’s private physician or the on-call physician.
5. Scribes may gather laboratory results, radiology results, medical records and other data for review by the physician or licensed independent practitioner. Data should not be entered into the EMR, chart, or otherwise incorporated into the medical record until it has been reviewed and approved by the physician’s or licensed independent practitioner.
6. Scribes must continuously check on the progress of this data in order to get the patient’s workup complete so that the physician or licensed independent practitioner is able to make treatment decisions.
7. The Scribe may not provide verbal orders or relay verbal orders between the physician or licensed independent practitioner and other patient care providers. The physician or licensed independent practitioner must communicate orders directly to other care providers.
8. Scribes can make and answer calls for the physician or licensed independent practitioner, however Scribes may not take medical data over the phone.
9. Scribes may not give medical advice, communicate medical information or care plans of any kind directly with the patient or patient’s family or friends.
10. Scribes can communicate waits and delays as directed by the RN or physician or licensed independent practitioner.
11. Scribes may print the physician’s or licensed independent practitioner process at the physician or licensed independent practitioner’s direction.

The Registered Nurse (RN) is a professional caregiver who assumes responsibility and accountability for assessing, planning, implementing and evaluating care of patients.
