This role is responsible for developing and implementing comprehensive, individualized care plans that align with senior participants’ personal health goals, proactively addressing changes in their conditions, and facilitating preventative primary care services as well as on-call and home visits.
*Duties*
* Conducts comprehensive History and Physicals on new participants and current participants in any care setting (clinic, member’s home in the community, nursing home, hospital, other care settings).
* Provides evaluation and treatment of participants during acute illness or the emergence of new complaints or symptoms.
* Determines the need for and facilitates referrals to other medical specialists when the participant's condition warrants.
* Evaluate the participants’ plan of care and revise it as appropriate.
* Provides preventive health maintenance for participants, including immunizations, screenings, and monitoring of pertinent indicators.
* Prescribes and adjusts medications within the scope of prescriptive authority and current practice guidelines.
* Orders, reviews, and provides instruction to participants regarding therapeutic diet.
* Orders, facilitates, and reviews diagnostic examinations such as laboratory, radiology, etc.
* Participates in on-call rotation and conducts home visits and nursing home visits as needed.
* In collaboration with PCP, manages the care of participants residing in contracted provider facilities – hospitals, nursing homes, and assisted living.
* Provides primary care coverage for other primary care physicians at assigned PACE Centers. May provide temporary coverage of assignments at other PACE Centers within the region as assigned. nce with medical imaging is advantageous.
*Requirements*
* Current and valid registration to practice as a Family, Adult, or Geriatric Nurse Practitioner in Indiana/KY
* Valid and up-to-date DEA certificate is required PRIOR to hire date.
* AANP or ANCC Board Certification required for IN/KY.
* Basic Life Support certified by the American Heart Association or American Red Cross required
* Minimum experience working as an Adult, Family, or Geriatric NP, must be at at least 2 years, including one (1) year of experience working with frail elderly, preferably in a long-term care or primary medical care setting.
* Willing to travel up to 50-60% or as needed.

General Summary:
Under the direction of the Site Manager and CT Scan Supervisor, demonstrates competency and actively functions in CT/General Radiology procedures.
Responsibilities:
1. Prepares procedure room and patient for all studies. Properly disinfect all equipment after each patient, following cleaning policies and protocols. All dirty linen should be disposed and replaced after every patient. Monitor patient flow for efficient daily operations.
2. Educates patient regarding ordered procedure and answers any related questions. Obtain patient history and written consent for exam. Verify patient information, check for appropriate ICD-10 codes. Utilize appropriate patient safety screening/consent forms to ensure all safety precautions have been addressed prior to procedure.
3. Performs procedures in accordance with established protocols. Positions patients and selects appropriate equipment necessary to accomplish procedure depending upon the age of the patient and the physical capabilities. Monitors patient during procedures; notifies Radiologist of reactions or problems. Assist the radiologists when necessary. Every effort must be made to minimize patient discomfort.
4. Demonstrate competency on all general diagnostic imaging equipment in CT. Ensure quality images for all completed procedures prior to releasing patient by reviewing or sending images to the PACS System, depending on type of procedure.
5. Carries out duties and assumes responsibility for portions of the quality assurance program, as directed by the equipment vendors. Maintain competence with HIS, RIS, PHS and PACS.
6. Run a pending work report daily to ensure all exams have been properly charged.
7. Maintain adequate supplies. Responsible for stocking the room, as assigned, at the beginning of each shift and throughout the day to ensure that the room is always ready to perform the required procedures. Be aware of supply needs and par levels, and report all items that fall below par level to the designated person.
