Our patient population ranges from pediatrics to geriatric depending on the location to include both orthopedic and neurological conditions. What you will do:

As a Physical Therapist, you are responsible for direct patient care.

Formulate comprehensive treatment program by evaluating patient’s past medical history, disease/condition, impairments, disability and functional/developmental status

The program supports the largest number of deliveries in the Capital region, many of which are high-risk pregnancies and multiple births. The family centered team YOU will work with includes physicians, licensed nurse midwives, dedicated perinatal nurses, social work and spiritual care services, skilled techs, and lactation consultants. What you will do:
Nurses working in Mother/Baby unit have the support of comprehensive orientation developed by a clinical nurse specialist in maternal/child nursing, on-site BLS and neonatal resuscitation classes, as well as on going clinical education. The staff is highly involved in quality improvement activities, program development, recognition activities, and team building. The clinical ladder program provides the opportunity for professional growth through mentoring, opportunities for leadership, and generous educational funding.
Responsibilities:
- The RN is responsible for assessing the needs of each patient to which he or she is assigned
- Planning the nursing care needed to meet those needs, including spiritual, emotional and physiological needs. Once planned, the RN is responsible for seeing that the plan of care is carried out through performance and or leadership of supervised personnel and that the plan of care is evaluated
- Directly accountable to the Nurse Manager for the quality of patient care delivered

Position Summary:

- Closely collaborate with primary care clinicians in the medical group setting to coordinate patient physical and mental health care.
- Screen, assess, and provide psychoeducation to patients for common mental health disorders. Facilitate patient engagement and follow-up care for a designated case load.
- Create a treatment plan and provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.

Track patient follow up and clinical outcomes using a registry, in addition to documenting in the electronic medical record.

Responsible for identification, investigation, reporting, prevention and control of healthcare associated infections at one or more of the Saint Alphonsus Medical Centers and their associated clinics.
The Infection Control Specialist is responsible for conducting/assisting with educational programs related to infection control and prevention, developing policies, and implementing performance improvement measures that ensure the medical centers meet the Infection Control requirements of CDC, OSHA, TJC and other federal and state regulatory agencies. Some travel between sites may be required.
REQUIREMENTS:
Bachelor's degree in health care related field required. A combination of bachelor’s degree in any field and current Certification in Infection Control (CIC) may substitute in lieu of health care degree requirement.
Certification in Infection Control (CIC) required within three years of employment. 3 years of acute care experience required. Experience in performance improvement, facilitation and adult education required. Experience in statistics and research methodology preferred.
Knowledge of federal, state, and local laws, regulations and guidelines that affect infection control. Knowledge of infection control requirements of CDC, OSHA, TJC, APIC recommendations and ability to interpret them in clinical settings. Knowledgeable of epidemiologic principles, communicable diseases, pharmacokinetics, microbiology, pathophysiology, sterilization, sanitation, and disinfection practices.
WHAT YOU WILL DO:
Demonstrates self-direction in providing systematic surveillance of patients to identify hospital acquired infections through established criteria. Maintains databases related to surveillance and process/outcome metrics. Analyzes, displays and communicates infection prevention data. Identifies persons who are at high risk for infections or pose an infection risk to others and initiates preventative measures. Coordinates appropriate investigation and intervention when a patient or employee is exposed to a potentially infectious agent.
Provides consultation on standards interpretation and implementation to hospital departments for the safety of health care services in relation to infection control. Participates as a member or consultant in organizational decision making, including the development of policies and procedures that affect the delivery of patient care. Demonstrates ability to work effectively with professional and supportive personnel throughout the medical centers to promote safety in relation to infection control.
Identifies areas for performance improvement or study in the realm of infection prevention and control and leads/participates in performance improvement teams, as appropriate. Initiates changes in clinical practice based on current concepts and findings from regulatory and accrediting agencies, professional practice guidelines, and other research and /or studies.

What you will do:
Responsible for collaboration, communication & facilitation of optimal provision of care through a computerized &/or digital platform supporting daily management of care.
