But you know her.
You review her medications, assess her symptoms, talk with the nursing team, and recognize a change that needs attention before it becomes something bigger.

Twin Cities Physicians is looking for a PA-C to bring community-based primary care to older adults throughout *Willmar and surrounding Western Minnesota communities*.

PA school prepared you to assess, diagnose, treat, problem-solve, and make clinical decisions.
We want you to use those skills.
You'll manage medically complex older adults with conditions like *diabetes, CHF, COPD, hypertension, dementia, and other chronic illnesses*, while also addressing acute changes in condition and preventative care needs.
But unlike a traditional clinic environment, you'll have the opportunity to understand the bigger picture.
You'll know the nurses caring for your patient.
You'll understand their living environment.
You'll communicate with families and caregivers.
You'll see what happens after a hospitalization instead of simply reading about it in the chart.

Healthcare access looks different outside the metro.
For older adults with mobility limitations, chronic illnesses, cognitive decline, or transportation challenges, getting to a traditional medical clinic isn't always simple.
So instead of asking them to come to us, *we go to them*.
You'll travel throughout Willmar and surrounding communities providing primary care within *assisted living, memory care, independent living, and skilled nursing settings*.
Yes, that means time on the road.
But the destination matters.
At the end of that drive is a community that needs a consistent provider and patients who benefit tremendously from having medical care brought directly to where they live.
*Your Office Is the Community*
Every day can look a little different.
One morning you may be managing chronic conditions and completing routine follow-ups.
That afternoon, a nurse may ask you to evaluate a resident with a sudden change in condition.
Another patient may have just returned from the hospital and needs medications reconciled and their treatment plan coordinated.
You'll be expected to *think clinically, prioritize appropriately, and take ownership of what happens next*.
You'll also have an experienced clinical organization behind you when collaboration or additional resources are needed.
*What You'll Be Doing*
* Provide comprehensive primary care within *senior living and long-term care communities*.
* Diagnose and manage *acute and chronic medical conditions*.
* Perform assessments, preventative care, and routine follow-up visits.
* Develop individualized treatment plans.
* Recognize *subtle changes in condition* and intervene proactively.
* Manage chronic diseases and medically complex patients.
* Coordinate care following hospitalizations, rehabilitation stays, and specialty visits.
* Collaborate with *physicians, nurses, therapists, pharmacists, caregivers, and senior living teams*.
* Educate patients and families regarding diagnoses, medications, and treatment plans.
* Help reduce *avoidable emergency department visits and hospitalizations* through proactive medical management.
* Maintain accurate and timely clinical documentation.

We welcome Physician Assistants with experience in:
* Primary Care.
* Family Practice.
* Internal Medicine.
* Urgent Care.
* Hospital Medicine.
* *Geriatrics.*
* Post-Acute Care.

* Home Health.
* Community-Based Medicine.
You don't need to come directly from senior living.
We're looking for a PA-C who enjoys *clinical problem-solving, medically complex patients, independence, and relationship-based care*.
*What We're Looking For*
*Required Qualifications*
* Graduate of an accredited Physician Assistant program.
* Active Minnesota PA-C license in good standing.
* Strong assessment, diagnostic, and *clinical decision-making skills*.
* Confidence managing patients with multiple chronic conditions.
* Ability to work autonomously while collaborating with physicians.
* Strong communication and care coordination skills.
* *Passion for caring for older adults.*
* Reliable transportation and willingness to travel throughout *Willmar and surrounding Western Minnesota communities*.
*Preferred Qualifications*
* Experience caring for *medically complex older adults*.
* Primary care or chronic disease management experience.
* Geriatric, long-term care, or community-based medicine experience.
* Assisted living, memory care, or skilled nursing experience.
* Advanced wound care experience.
* *Wound Care Certification (WCC)* is a plus.
*Be the Provider They Don't Have to Leave Home to See*
