Twin Cities Physicians offers Physician Assistants a different kind of practice.

You'll provide *on-site primary care to older adults throughout Willmar and surrounding Western Minnesota communities*, following the same patients over time instead of constantly handing their care to someone else.

You get to see whether the treatment worked.

Whether the medication adjustment helped.

Whether the patient recovered after hospitalization.

Whether the plan actually made their life better.

*You don't just make the decision. You get to see what happens because of it.*

Follow the Patient, Not Just the Visit

Our providers care for older adults in *assisted living, memory care, independent living, and skilled nursing communities*.

These are often medically complex patients managing multiple chronic conditions, medications, specialists, and transitions of care.

You'll become their primary care provider.

That means managing diabetes, CHF, COPD, hypertension, dementia, and other chronic conditions while also addressing acute concerns, preventative care, medication management, and post-hospital follow-up.

But you're not starting from scratch every time.

*You know what you tried last time. You know what changed. And you decide what comes next.*

Complex Patients Need Providers Who Think Beyond Today's Visit

Geriatric medicine requires strong clinical judgment.

A change in appetite may mean more than a change in appetite. New confusion may have several possible causes. A medication that solves one problem may complicate another.

You'll work directly with *community nurses and caregivers who see your patients every day*, giving you additional clinical context that can be difficult to obtain during a traditional office visit.

When something changes, you can assess what's happening and determine the appropriate next step.

Sometimes that's treatment in place.

Sometimes it's medication management or additional testing.

And sometimes the hospital truly is the appropriate destination.

*Your job is to make the right clinical decision for the patient in front of you.*

What You'll Be Doing

* Provide comprehensive *on-site primary care* within senior living communities
* Manage an ongoing panel of *older adult and medically complex patients*
* Diagnose and treat acute and chronic medical conditions
* Complete assessments, annual wellness visits, preventative care, and follow-ups
* Manage medications and chronic disease treatment plans
* Recognize and respond to *changes in patient condition*
* Follow patients after hospitalizations and specialty visits
* Coordinate care across multiple disciplines
* Collaborate with physicians, nurses, therapists, pharmacists, caregivers, and families
* Help identify concerns early and *reduce avoidable hospitalizations and readmissions*
* Maintain accurate and timely clinical documentation

You Don't Need a Geriatrics Background

Maybe you've spent the last several years in *urgent care* and you're tired of never knowing what happened after discharge.

Maybe you're in *hospital medicine* and would like to see patients outside the walls of the hospital.

Maybe you're practicing in *family medicine or primary care* but want more autonomy and fewer of the constraints of a traditional clinic.

Or maybe you already know that caring for older adults is where you belong.

Experience from *primary care, family medicine, internal medicine, urgent care, hospital medicine, geriatrics, post-acute care, skilled nursing, home health, hospice/palliative care, or community-based medicine* can translate extremely well.

What We're Looking For

*Required:*

* Graduate of an accredited *Physician Assistant program*
* Active *Minnesota Physician Assistant license*
* Strong assessment, diagnostic, and clinical decision-making skills
* Confidence managing patients with multiple chronic conditions
* Strong communication and care coordination skills
* Interest in caring for *older adults*
* Reliable transportation and willingness to travel throughout *Willmar and surrounding communities*

Experience with medically complex adults, chronic disease management, geriatrics, senior living, or community-based medicine is highly valued. *Wound Care Certification (WCC)* is a plus.

Autonomy Without Going It Alone

You'll spend much of your day practicing independently in the field, but you'll have an established *physician-led organization* behind you.

You get meaningful clinical ownership without having to build the infrastructure yourself.
